B.2 STATEMENT OF NEED (SON)
TITLE OF PROJECT
UpToDate Anywhere database.
BACKGROUND
The James A. Haley Veterans’ Hospital, Tampa, FL (JAHVA) desires to provide a point-of-care clinical information product for use of its clinical staff in providing excellent patient care. The JAHVA Library Service requires an authoritative, comprehensive point-of-care clinical reference product, an essential tool for our JAHVH clinical staff. This product is needed to provide clinical staff with access to knowledge-based medical information in both desktop and mobile platforms. Staff will use this product to conduct health services research, prepare reports, and access other knowledge-based health information documents.
SCOPE OF WORK
Contractor database (program) must be Internet-based and able to be incorporated into the “Toolbar” of the hospital’s Computerized Patient Record System (CPRS). Individual access must provide direct access to continuing education credits (CME), and it must be accessible remotely using individual passwords. The Data base must include 10,000 clinical topics covered at a minimum and database needs to include recommendations and expert opinion which provides guidance for clinicians at the point of care.
a. Comprehensive point-of-care product.
b. Easily accessible from the CPRS “Toolbar.”
c. Remotely accessible with individual passwords.
d. Provides continuing education credits (CME) directly without third-party intermediaries.
e. User friendly and with a >75% acceptance rate of JAHVH clinicians.
D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES
a. This product will provide evidence-based, peer-reviewed, comprehensive topic reviews that are highly pertinent to James A. Haley Veterans Hospital clinicians at all designated locations (main hospital and satellite clinics). Content must be presented in a scholarly manner and format appropriate for a teaching hospital. Articles should provide narratives that discuss disease progression and treatment options; they should not rely merely on “short answer” or bulleted lists.
b. The product must provide full integration and interoperability with the National Library of Medicine’s PubMed LinkOut functionality, linking out to the full-text journal articles held in our JAHVH collection.
c. This product must provide integrated and direct links from topic search results to visual images in VisualDX, the diagnostic support tool accessible from CPRS and purchased by the national Library Network Office for all VA facilities. VisualDX provides clinicians with relevant images to assist in their diagnosis of skin conditions seen in primary care settings.
d. This resource should include a large file of knowledge-based topic reviews from a wide range of medical specialties, reflecting the needs of JAHVH patients and clinical staff, including, but not limited to the following:
Adult Emergency Medicine
Hospice and Palliative Care
Adult Primary Care and Internal Medicine
Hospital Medicine
Allergy and Immunology
Infectious Diseases
Anesthesiology
Nephrology and Hypertension
Cardiovascular Medicine
Neurology
Dermatology
Obstetrics, Gynecology and Women’s Health
Endocrinology and Diabetes
Oncology
Family Medicine and General Practice
Pulmonary and Critical Care
Gastroenterology and Hepatology
Psychiatry
General Surgery
Rheumatology
Geriatrics
Sleep Medicine
Hematology
e. This resource will also include evidence-based topic reviews designed for patient education and written at an appropriate reading level for patients. Use of these reviews will allow patients and doctors to actively participate in shared decision-making.
f. There must be published reports and data validating the ability of this product to positively impact quality of care and patient safety.
g. The product will provide IP authentication via networked hospital computers at these addresses:
JAHVH IP Addresses 152.130.7.197 152.131.7.197 152.132.7.197 152.133.7.197 In addition, the product must provide off-site access through the hospital’s password-protected Athens remote authentication product and mobile access using individual usernames and passwords.
h. The product will allow for multiple simultaneous users from any computer with Internet access.
i. The product will be updated regularly, with major updates occurring at least three times annually.
j. Clinicians reviewing topic articles will automatically receive American Medical Association (AMA) Category 1 Continuing Medical Education (CME), Maintenance of Certification (MOC), or MOC Part II CME credits through accreditation providers for physicians, nurses, and other clinical health care professions, subject to the requirements of the Accreditation Council for Continuing Medical Education (ACCME). Credits are granted automatically, so that clinicians do not need to contact third-party CME providers to receive their credits. The product will be an accredited CME provider for the American Medical Association, American Academy of Family Physicians, American Association of Nurse Practitioners, American Academy of Pediatrics, American Academy of Physician Assistants, American College of Emergency Physician, American College of Obstetricians and Gynecologists and American Osteopathic Association, in addition to other professional organizations.
k. JAHVH employees will be authorized users and may print individual articles for their personal educational use, and they send individual articles to colleagues
REPORTING REQUIREMENTS
The Vendor will respond to any technical issue or other question from JAHVH within 2 hours of notification.
GOVERNMENT RESPONSIBILITIES
The Government will not need to furnish any materials or equipment; employees will use existing hospital computers, their personal computers, or their personal mobile devices. Staff may also access the database remotely from any Internet-based computer, provided they have previously requested offsite access from a computer on the hospital network.
