Request for Information- 36C24725Q0884

1. We wanted to confirm if the two new grossing stations that will be installed have already been purchased? Confirming if this is only uninstalling and installing or it also includes purchase of the grossing stations.

This procurement encompasses the following:

· Acquisition of two new grossing stations

· Removal and disposal of existing grossing stations

· Complete turn-key installation and commissioning of the two new grossing stations

1. Scope of Work, Item #5 (Page 8) This section is incorrect and needs to be redlined and amended to read:

“The connection to the facility will be completed by the Mopec contractor only if the facility rough-ins match the provided drawing. Mopec will not modify the facility rough-ins.”

The Contractor shall perform a preliminary site assessment to evaluate existing conditions and develop detailed plans for the removal and installation procedures. This site survey must be coordinated with the designated Point-of-Contact (POC) and conducted between September 8, 2025, and September 12, 2025.

Contractor shall provide:

•all requisite supervision
•personnel
•travel logistics
•tools
•equipment
•consumables
•materials
•technical expertise
•certified credentials

For Unit 1, Contractor will:

•initiate installation with the smaller unit located on the right side of the room
•acquire the new unit from the hospital’s receiving dock and transport it to the main front entrance
•remove the existing unit through the hospital’s front entrance, and install the new unit via the same access point
•position, elevate, and level the new unit; connect electrical power, drainage, and hot- and cold-water supply lines
•connect two flexible ducts to the existing vent hood, securing them with appropriate clamps, and attach them to the existing penetrations
•perform operational testing to verify system functionality, subsequently commissioning the system for active use
•following the successful commissioning of the first unit, proceed with the installation of the second unit

For Unit 2, Contractor will:

•remove the secondary unit, performing a detailed disassembly due to the extended countertop affixed to its right end
•modify the existing vent hood to facilitate the integration of two flexible duct connections
•cap the drain and water supply lines associated with the removed unit for temporary closure
•fabricate a stainless-steel countertop to replace the countertop associated with the cabinets remaining after system removal
•modify the existing countertop attached to the removed sink system, if feasible; otherwise, a new countertop shall be fabricated
•upon completion of vent hood and countertop modifications, the contractor will retrieve the second unit from receiving, transport it to the main front entrance, and install it following procedures identical to those used for the first unit

For both Units:

· The contractor will be tasked with decontaminating and disposing of biohazard components.

· The contractor will oversee any qualifications, re-qualifications, or testing and balancing of the exhaust fan.

· A one-year warranty on parts and labor for the supplied materials will be provided.

· The contractor will be accountable for all necessary asbestos abatement

1. Existing Equipment Details Without knowing the make and model of the existing equipment, we cannot provide an accurate quote. Please provide photos of the current units and their facility connections. If the old units are Fisher GrossPath, the rough-ins will not align, and installation cannot proceed immediately after removal.

Please provide photos of existing equipment, including the existing utility connections of these units, along with their make and model #’s.

Unit 1 grossing station: model MB600, serial number MB600J34

Unit 2 custom grossing station:

The connections shall be inspected and verified by the contractor during the site visit.

1. Delivery & Freight Requirements

3. This will be a ground delivery and will require a liftgate. Freight must be Pre-Paid and Add (PPA).

3. Cones or similar barricades are required to block traffic; this means we must purchase or rent and should be excluded from Mopec’s scope, this should be the GC issue as it is his work site.

3. Mopec does not work in the street to uncrate units we can work in a parking lot where the units can be delivered (near the front door).

Can you verify if this will require a lift gate and ground delivery? If that is the case, we will need more information to accurately quote freight.

This will be a dock delivery.

1. On-Site Fabrication Mopec does not modify or fabricate countertops on site.

The modification and/or fabrication of the countertops on the side housing the existing custom grossing station is an obligatory specification/requirement.

1. Emergency Call Provision The provision for a technician to arrive within 2 hours of a service call must be reviewed by Scott. Mopec does not currently provide 24-hour service to meet a 2-hour emergency response requirement.

7. Training Requirements We can meet the Sample 1 training requirement. Our lead installers hold OSHA 30 certifications, and we can request OSHA 10 certified workers as needed.

* Ignore the placeholder text related to the Katron system, as it was inadvertently retained from the form template.

The Contractor shall furnish, upon request, copies of all necessary training documentation (e.g., HVAC, biohazardous waste handling), valid licenses, certifications (e.g., NSF, OSHA, HVAC), and applicable permits.

TRAINING REQUIREMENTS:

HVAC Certified Technician Certified Ventilation Specialist

8. Site Access Requirements Please confirm any site access requirements beyond checking in with engineering upon arrival. Are there orientations or special requirements due to this being a VAMC site? Also, please confirm if this project is subject to prevailing wage requirements.

Any modifications or supplementary requirements pertaining to site access will be addressed and finalized during the site inspection.

** Site survey must be coordinated with the designated Point-of-Contact (POC) and conducted between September 8, 2025, and September 11, 2025, final questions will be taken September 12. Any questions beyond this date will not be accepted.** POC: Knightner, Joielle Email: Joielle.Knightner@va.gov image4.jpeg image1.jpeg image2.jpeg image3.jpeg
