The purpose of this amendment it to post Questions and Answer, Site Visit sheets, extend the time for receipt of quotes, extend the time for RFIs, incorporate revised SOW and drawings.

1. The revised SOW is part of this amendment.

3. The site visit sheets are attached to this amendment.

4. The Questions and Answers are attached to this amendment.

5. The deadline for RFIs is March 10, 2025 at 3:00 PM EST.

STATEMENT OF WORK

ASBESTOS ABATEMENT

1.1 STATEMENT OF BID ITEMS(S)

A. Vendor shall furnish labor, materials, equipment, supervision and disposal services to abate asbestos containing materials from rooms in

Building 801, 1st Floor (1B104, and 1B105) at the VA Augusta Healthcare

System. Includes approximately 2,750 square feet of asbestos containing floor tile (up to 2 layers) and mastic. Includes notifications to authorities and proper disposal of hazardous materials. Includes air monitoring and sampling to ensure safety of employees once abatement is completed.

B. Contractor shall provide asbestos abatement plan for review by facility

IH.

C. All employees of vendor shall comply with VA security management program and obtain permission of the VA police, be identified by employer, and restricted from unauthorized access.

1.2 SECURITY REQUIREMENTS

A. Security Procedures:

1. Vendor’s employees shall not enter the site without appropriate coordination with COR. They may also be subject to inspection of their personal effects when entering or leaving the site.

2. For working outside the “regular hours” as defined in the contract, the Vendor shall give 3 days notice to the Contracting Officer so that security escort arrangements can be provided for the employees.

3. No photography of VA premises is allowed without written permission of the Contracting Officer.

4. VA reserves the right to close down or shut down the site and order

Vendor’s employees off the premises in the event of a national emergency. The Vendor may return to the site only with the written approval of the Contracting Officer.

1.3 OPERATIONS AND STORAGE AREAS

A. The Vendor shall confine all operations (including storage of materials) on Government premises to areas authorized or approved by the Contracting

Officer. The Vendor shall hold and save the Government, its officers and agents, free and harmless from liability of any nature occasioned by the

Vendor's performance.

1.4 INSTRUCTIONS

A. Vendor shall furnish reports of air quality and disposal certification as required.

1.5 WORK HOURS

A. Work can be done during normal working hours (7:30AM-4:00PM), Monday—

Friday, except federal holidays. If nights and weekends are needed, contractor must request through the COR 72 hours in advance. Shutdowns of any kind must be requested in writing 14 days prior.

Construction period of performance 20 calendar days from notice to proceed.

Questions and Answers

1. Do we have to hire our own Certified industrial Hygienist or does the VA hire one?

Contractor shall provide CIH.

2. Can a containment be put up on that hallway leading to the work area ? To facilitate the access of workers, materials and ACM debris removal from the exterior of the building to the work area vice versa?

Yes, containment can be put in the hallway, but must leave enough access for hospital occupants to utilize the hallway for entry and exit.

3. Will both rooms need to be completed at same time or separate?

Yes, both rooms can be completed simultaneously.

4. Is there a window or door that can be used to route the exhaust tubing of the negative air machines to outside the bldg.?

There is no window within the room. Routing will have to be coordinate with VA, and limiting disruption to facility operations.

5. The air monitoring and clearance testing to be supplied by the VA?

Air monitoring and clearance samples will be supplied by Contractor.

6. There appears to be two separate areas that require abatement, correct?

Yes, there are two separate areas.

7. The SOW also states that work must be completed within 20 calendar days from Notice to Proceed. Please verify that the notice to proceed will be provided after all submittals and ten-day notification have been completed and approved. That's a critical area as for start to completion. Can you verify the start to completion period?

20 days from notice to proceed.
