A.1 52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014)
UNVERIFIED SDVOSBS WILL NOT BE CONSIDERED FOR AWARD
ADDENDUM to FAR 52.212-2 EVALUATION—COMMERCIAL ITEMS
(a) The Government will award ONE (1) Base + 4 Option Year Firm Fixed Price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
Past Performance Technical Capability Price Past Performance is more important than price. Technical Capability is more important than price. Past Performance is equal to Technical Capability and when combined is more important than price. All evaluation factors other than non- cost factors are significantly more important than cost or price.
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
EVALUATION CRITERIA for Advanced Life Support (ALS) Stretcher transportation, and Nonemergent Medical Services: Basis for award will be made by evaluating proposals using the following criteria listed.
Factor 1. Past Performance Past performance evaluation results in an assessment of the offeror’s probability of meeting the solicitation requirements. It considers each offeror demonstrated recent and relevant record of performance in supplying products and services that met the contract requirements:
There are two aspects to the past performance evaluation:
(a)Relevancy of contractual work performed and;
b) How well the contractual work was performed Relevant is defined as three (3) contracts awarded within the last five (5) years that include services that are similar is complexity, dollar value, contract type and degree of subcontract and teaming. The second aspect of the past performance evaluation is to determine how well the contractor performed on the contracts.
The government will review past performance information and determine the quality and usefulness as it applies to performance confidence assessment. Ensure your submissions include a point of contact who can speak on behalf of the company, the company name, address, telephone number(s), email address and contract dates. We have attached a past performance evaluation form for you to send to your references.
The Government may contact references, other than those identified by the offeror, and use the information received to evaluate the offeror’s past performance. The information evaluated may include data on efforts performed by other divisions, critical subcontractors, or teaming partners, if such resources will be brought to bear or significantly influence the performance of the proposed effort. Special emphasis will be placed on performance related to areas covered in the PWS for this solicitation. The government will also use information obtained from government data bases such as CPARS, PPIRS, and any other data base as necessary.
In the case of an offeror without a record of relevant performance history or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on performance history.
Factor 2. Technical Capability Technical Capability - Offerors will be evaluated based on the capability of the items offered to the need of the Government in accordance with the PERFORMANCE WORK STATEMENT. Each proposal will be evaluated against the Performance Work Statement and the stated evaluation criteria.
Subfactor 1: Technical Narrative- The Narrative must clearly demonstrate the contractor's understanding of the requirement and technical capability to perform services according to the PWS.
However the narrative shall include listing of all vehicles, minimum of eight (8), that conform to the requirements listed in the performance work statement(PWS), a copy of the Fleet Maintenance Plan and a copy of the last vehicle inspection report. Further, the Offeror shall submit a photocopy of the business license with the Alabama Department of Public Health and proof of Liability insurance. May not be met through a proposed subcontractor but only through the resulting prime contractor.
Subfactor 2: Staffing Plan: The offeror shall submit EMT documentation on all personnel proposed to perform under the resulting contract. The listing shall be accompanied by a photocopy of ALL EMT licenses for the State of Alabama and the Driver’s record. EMT’s may be employees of the resulting prime.
Subfactor 3: Transition Plan: The offeror must submit documentation demonstrating how they will provide satellite offices in Birmingham and Huntsville, (or closest county available) Alabama within 60 days of award.
Factor 3. Price The price will be evaluated based on the overall price to determine if the proposed price is fair and reasonable. Unreasonable prices may be grounds for eliminating an offeror from competition.
· Price will be considered in the overall evaluation of the offered services to ensure award is made to the offeror presenting the best value to the Government. Acceptance may be made to an offeror that is not the lowest priced.
