The WJB Dorn (Columbia) VA Medical Center is looking for SDVOSB to establish a contract for Boiler Plant Preventative Maintenance Service Base plus 4

The Department of Veterans Affairs (VA), Veterans Integrated Service Network 7 (VISN 7), Regional Procurement Office East (RPO-East), 6439 Garners Ferry Road Columbia, SC 29209, location is issuing a Combined Synopsis/Solicitation for commercial services prepared in accordance with Federal Acquisition Regulation (FAR) This Request for Quote (RFQ) is issued in accordance with FAR 12 and in conjunction with Simplified Acquisition Procedures FAR 13.5. Evaluation of price and other factors will be performed in accordance with procedures outlined in FAR 13.106-3

This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-06 / 05-06-2020. In addition, offerors are responsible for continuing to monitor the websites for any/all future Amendments issued. Offerors shall sign copies of any amendment issued and submit with their package.

The WJB Dorn (Columbia) VA Medical Center is looking for SDVOSB to establish a contract for Boiler Plant Preventative Maintenance Service. This contract will a twelve (12) month base year with four (4) one-year option periods.

Contract Type: Firm Fixed Price

All Offerors must have an active Vendor Information Pages account (https://www.vip.vetbiz.gov/) reflecting they are currently registered as an SDVOSB.

FAR 52.212-1 Instructions to offerors-Commercial Items (MAR 2020) Addendum (Additional Information) is deleted in its entirety and is replaced with the following:

1. The offeror shall submit their quote on company letterhead. The offeror’s quote shall include the name, address, and telephone number of the offeror, the offeror’s DUNS# and ORCA document in SAM at www.sam.gov, and a completed copy of 52.212-3 offeror Representations and Certifications- Commercial Items.

2. Signed copy of the Combined Synopsis/Solicitation Notice and/or any amendments posted to the solicitation.

3.All offerors must be VIP verified SDVOSB concerns at the time of quote submittal and at the time of award. See VAAR 819.7003.
4.All offerors must be registered in SAM at the time of quote submittal.
5All offerors shall demonstrate ability to meet the Limitations on Subcontracting as described in 13 CFR 125.6 in accordance with VAAR clause 852.219-10. An offeror shall indicate in its quote whether it will (1) self-perform 100% of the requirement, or (2) subcontract any portion of the requirement. If the offeror intends to subcontract any portion of the requirement, the offeror shall provide a subcontracting plan that includes a percentage breakdown evidencing how the offeror will comply with the limitations. Failure to provide the required information may result in an offeror’s quote being rejected without further consideration.
6.Price: The offeror shall fill-in all unit prices and totals for each CLIN as well as the total amount of the price quote in the solicitation’s Price Schedule.
7.Technical Approach: The offeror shall provide a narrative demonstrating how it will meet the Boiler Plant Preventative Maintenance Services requirement as described in the solicitation’s performance work statement.
8.Experience: The offeror shall provide a narrative describing its specific experience providing Boiler Plant Preventative Maintenance Service. General statements claiming experience providing Boiler Plant Preventative Maintenance Service, that do not include specific references to prior contract work, or that do not include the information required below, are not acceptable. For each reference contract the Offeror submits as evidence of its experience providing Boiler Plant Preventative Maintenance Service. services, the Offeror’s narrative must clearly state the following information:
a.Boiler Plant Maintenance services designed to provide server and software maintenance
b.Boiler combustion control point calibration and combustion testing for each boiler as applicable
c.Boiler safety switch calibration and testing for each boiler
d.Boiler plant loop tuning for each system
e.Common plant I/O control and instrumentation preventive maintenance and calibration
f.Gas detection system preventative maintenance and calibration. the legal name of entity with whom the offeror held the contract.

h. the contract number(s);

i.a description of the specific tasks the offeror performed under that contract.
j.the dates during which the offeror performed the contract.
k.the name, phone number, and email address of a person at the entity with whom the offeror held the contract who can verify the information the offeror provides in the narrative description for this factor.
9.Past Performance: The offeror shall provide a minimum of one (1) past performance reference for similar services within the last four (4) years, either Governmental or Commercial entity, in response to this solicitation, to include place, size, and years of service performed. If no past performance is available, you must address this in your offer. Past performance will be reviewed as part of the responsibility determination only.

10. The offeror will submit one document in PDF form only, no word documents will be accepted Not providing all the required documents above along with not following the instructions for submission will result in the rejection of a quote

NOTE: This is the ONLY site visit scheduled for this project; individual or separate site visits will not be scheduled. This is not a mandatory site visit, but it is highly encouraged to attend before submitting a proposal.

WORK STATEMENT

BOILER PLANT MAINTENANCE

1. GENERAL:

Provide Annual Boiler Plant Control System Maintenance consisting of maintenance on four (4), Duel Fuel (#2 Fuel Oil / Natural Gas) Boilers and the associated systems covering several areas of the facility. Vendor will provide service calls by a qualified representative, who will make on-the-spot analysis and repairs to electric/electronic controls for the boilers and associated systems, as well as, provide emergency service and after-hours response if needed, to maintain system stability to the facility. This service is provided for the Department of Veterans Affairs, Columbia VA Health Care System (CVAHCS), 6439 Garners Ferry Road, Columbia, South Carolina 29209

2. BACKGROUND:

The CVAHCS is a 216-bed facility, encompassing acute medical, surgical, psychiatric, and long-term care. The hospital is located in Columbia, South Carolina and provides primary, secondary, and some tertiary care. Annually, the medical center serves approximately 65,000 patients. Satellite outpatient clinics are located in Anderson, Greenville, Florence, Orangeburg, Rockhill, Sumter, and Spartanburg, South Carolina.

3. PERIOD OF PERFORMANCE:

Contract POP: MAY 1, 2021 TO APRIL 30, 2022.

4. PLACE OF PERFORMANCE: CVAHCS, 6439 Garners Ferry Rd, Columbia, SC

5. PERFORMANCE: Provide annual Boiler Plant Control System Maintenance. Perform two (2) semi-annual testing and maintenance services IAW VHA Directive 1810, dated 11 Sep 2018. Inspection and testing must be conducted and documented as defined in the latest VHA Boiler Plant Safety Device Testing Manual. All inspections and testing must be done by qualified individuals annually. The Boiler Plant Control System maintenance will consist of maintenance on four (4) Duel Fuel (#2 Fuel Oil / Natural Gas) Boilers and the associated systems covering the following areas:

6. DAYS AND HOURS OF OPERATION:

Monday – Friday, 8:00 a.m. to 5:30 p.m.; excludes Federally Observed Holidays

7. POINT OF CONTACT/AUTHORIZED INDIVIDUALS:

8. INVOICES:

Payment will be made upon receipt of a properly prepared detailed invoice, prepared by the Contractor, validated by the Contracting Officer’s Representative (COR), and submitted to VA FSC, P. O. BOX 149971, AUSTIN, TX 78714.

A properly prepared invoice will contain:

· Invoice Number and Date

· Contractor’s Name and Address

· Accurate Purchase Order Number

· Supply or Service provided

· Date of Service (period of performance)

· Total amount due

11. RECORDS MANAGEMENT LANGUAGE FOR CONTRACTS

The following standard items relate to records generated in executing the contract and should be included in a typical Electronic Information Systems (EIS) procurement contract:

a. Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228.

b. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest.

c. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records.

d. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act.

e. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract.

f. The Government Agency owns the rights to all data/records produced as part of this contract.

g. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data.

h. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.].

i. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules.

j. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
