Performance Work Statement Steam Quality Testing for SPS Engineering Service Safety Shop Hampton VA Medical Center Background: The Hampton VA Medical Center located at 100 Emancipation Drive Hampton Virginia 23667 requires a Steam Quality Test to be performed on the steam line that serves steam sterilization equipment in the Sterile Processing Service (SPS), Building 110A, CG48A.
1. General. Requirement: In the base year of the contract, contractor will install steam sample points in the defined testing location and provide all personnel, labor, equipment, and supplies to perform an effective and seamless installation. The contractor shall provide all necessary time, personnel, labor, equipment, and supplies, to perform steam testing for dryness and non-condensable gases at the steam line serving steam sterilization equipment located in Building 110A, CG48A within the SPS at the Hampton VA Medical Center. The contractor will provide a formal sample report indicating the results of the test. The work outlined in this Scope shall be performed annually for one base year and four option years.
2. Coordination. The Engineering Service Safety Officer (ext. 2085) will serve as the focal point and point-of-contact (POC) for this contract.
2.1. The contractor must pre-coordinate and preschedule work with the COR prior to arriving on station. Two-week notice indicating the exact date and time of arrival is required.
2.2. This task shall be conducted and accomplished during normal business hours i.e. from 0800 AM to 1630 PM, Monday through Friday. Work accomplished outside these hours must be approved by the COR.
3. Government Provided Tools, Materials and Equipment. This is a turn-key project with the Contractor providing all labor, tools, materials, and equipment.
4. Period of Performance. Performance of Steam Testing shall occur in October of each calendar year.
5. Specifications. The Contractor shall perform all work required to complete the requirements outlined in section, 1. General. to include the following:
5.1. During the base year of the contract, the contractor will perform all work necessary to safely install steam sample points on the steam line serving steam sterilization equipment in the Sterile Processing Service (SPS), Building 110A, CG48A. Prior to the installation of sample points the contractor will coordinate with the Contracting Officer’s Representative to receive approval on the location of the sample point location. The contractor will install sample points adequate to meet the requirements of this contract, turn-key and without additional or unforeseen costs to the government.
5.2. Measurement of steam quality within the SPS at the Hampton VA Medical Center as determined by the level of dryness and percentage of non-condensable gases (NCGs). Steam dryness is measured in the percentage of moisture by volume.
5.3. In the final report, the results of the steam quality test should be compared to the following capability requirements had by the Hampton VA Medical Center: Manufactures of steam sterilization equipment recommend steam with 97% to 100% dryness. Non-Condensable Gases (NCGs) is below 3.5% by volume to prevent the creation of air pocket; these can block the steam from directly contacting the Reusable Medical Equipment’s (RMEs) and and prevent effective sterilization.
5.4. Before arriving on-site, the contractor must coordinate arrival with the VA COR in the Safety Office to either be escorted or receive a visitor badge.
5.5. The steam quality report should be provided to the COR within 30 days of the steam testing.
5.6. This service shall be performed once per year for an initial year and 4 option years.
5.7. Performance of Steam Testing shall be performed in October of each calendar year.
5.8. Steam testing, mobilization, materials, time, equipment, and coordination will be the responsible of the contractor and this service will be entirely turn-key.
5.9. The contractor shall remove all trash, materials, and debris generated by their work from the service site and provide for disposal of all waste generated. The site shall be left in the condition it was in prior to the contractor’s arrival.
5.10. Submit invoicing in-accordance with contract instructions.

6.4. Parking will be in designated lots. Consult with the POC as to allowable parking and material drop-off.
6.5. The Contractor will be responsible for ensuring that all the employees and other personnel visiting the work site have and are using the appropriate Personal Protective Equipment (PPE) that addresses the hazards found at the site. The Contractor will be responsible for the upkeep, issue, and training for the PPE used and maintaining all required documentation.
6.6. Since the incidents of September 11, 2001, the Federal Government has begun to implement new security measures to help protect federal facilities. The Contractor will provide to the POC a list of employees expected to work on this contract. This list will be on company letterhead and contain the employees' name, driver's license number, address, and nationality. This list will be verified and signed by a corporate officer. These personnel are subject to a background check and fingerprinting to be eligible to receive a security badge.
6.7. (a) Definitions. As used in this clause – VA Information system is the same as information system and means, pursuant to 38 U.S.C. 5727, a discrete set of information resources organized for the collection, processing, maintenance, use, sharing, dissemination, or disposition of information whether automated or manual.
VA sensitive information means all VA data, on any storage media or in any form or format, which requires protection due to the risk of harm that could result from inadvertent or deliberate disclosure, alteration, or destruction of the information and includes sensitive personal information. The term includes information where improper use or disclosure could adversely affect the ability of VA to accomplish its mission, proprietary information, records about individuals requiring protection under various confidentiality provisions such as the Privacy Act and the HIPAA Privacy Rule, and information that can be withheld under the Freedom of Information Act. Examples of VA sensitive information include the following: individually-identifiable medical, benefits, and personnel information; financial, budgetary, research, quality assurance, confidential commercial, critical infrastructure, investigatory, and law enforcement information; information that is confidential and privileged in litigation such as information protected by the deliberative process privilege, attorney work-product privilege, and the attorney client privilege; and other information which, if released, could result in violation of law or harm or unfairness to any individual or group, or could adversely affect the national interest or the conduct of Federal programs.
(b) General. Contractor personnel assigned to work for or on behalf of VA must undergo a background investigation commensurate with the risk and sensitivity level designation associated with the work to be performed at the level indicated in the contract. The Contractor and subcontractors shall comply with VA Directive/Handbook 0710, Personnel Security and Suitability Program, which can be accessed at: https://vaww.va.gov/vapubs/index.cfm.

The following sensitivity designations have been assigned for the identified Tier 3 and Tier 5 required by this contract:

The results from a background investigation are used to determine if an individual’s fitness is sufficient for that individual to perform work for or on behalf of VA in the position identified in this contract. Contractor fitness determinations are made in accordance with 5 CFR Part 731.202.
Fitness requirements for employment are separate and distinct from job qualifications. If a Contractor or subcontractor employee is found to be unsuitable or unfit to provide services under this contract, the Contractor shall immediately remove the employee from working on this contract and take those necessary steps that restrict the employee’s logical access to VA data, information, VA sensitive information, or information technology or VA information systems containing such data or information.
The Contractor shall advise the employee that they are not permitted to access any VA controlled building or real property in relation to this contract. The removal of an unfit Contractor or subcontractor employee does not alleviate the Contractor from satisfying the requirements of this contract. The Government will not reimburse the Contractor for any costs associated with the recruitment/replacement of an employee or subcontractor employee who is found to be unfit.
(e) Identification Cards. The Government will provide a Personal Identification Verification (PIV) card or other identification card, as necessary, to fit Contractor personnel who require physical access to VA facilities and/or logical access to VA data, information, VA sensitive information, or information technology or VA information systems containing such data or information. Contractor and subcontractor personnel shall prominently display their PIV/identification card on their persons while working at a VA facility and shall present their PIV/identification card for inspection upon request by a VA official. Within 48 hours of reporting the lost/stolen PIV/identification card, the Contractor shall submit to the Program Manager an incident report that describes the relevant facts and circumstances regarding the loss/theft. If the loss/theft was reported by the Contractor to the local police, the Contractor shall further submit a copy of the final police report to the Program Manager within 48 hours of the report being made available by the local police department. The Government will not reimburse the Contractor for any costs that result from lost/stolen PIV/identification card(s).

The report must provide the status of each contractor/subcontractor employee who is required to have a PIV/identification card during the performance of the contract. The report shall identify the Contractor and the contract number, and list the following status for each contractor/subcontractor employee who holds a PIV/identification card under this contract:
