Questions Responses Would the Government be willing to extend the submission due date? Yes. See amendment.

This section indicates that the period of performanced will begin on 9/1/2026.

Will there be an implementation period? Implementation period will be 60 days from award.

Given the requirement for custom kits and the specified delivery timelines, will the Government commit to purchasing defined quantities of each pack based on anticipated annual usage to enable offerors to maintain inventory? If so, please provide the quantities the Government will commit to purchasing for each pack.

The # of packs listed is the estimated annual qantities.

Will the government allow at least 14 weeks as a implementation period prior to the start of the contract? 60 Days will be alloted for performance to begin.

Please confirm the UOM. For instance, please confirm that the item should be priced by the Case (CS). Please also confirm how many Packs (PK) should be included in each Item Number.

Items shall be priced per case. See attachement B, Line Item Spreadsheet, column L for the # of kits per case.

Does the Government anticipate adding more ship to locations? No. There are 3 ship to locations.

Does the quantity section include estimates for emergency orders? Yes

Could you provide the estimated quantity for emergency orders, or alternatively, the emergency order quantity from the prior year? There were no emergency orders.

Could you please provide the historical frequency and pattern of emergency orders? N/A

Could you please provide past monthly usage for each item?

The quantities provided are estimates for annual usage.

Monthly averages are not available as surgical cases fluctuate.

Could you please confirm this requirement apply the same for emergency orders? Will the emergency orders be required any time of the day for order placement?

Yes.

Please confirm whether the requirement that the VA ‘will not wait for the pack to be shipped to the distribution center’ means that custom kits must be shipped directly from the entity assembling the kit to the VA facility (i.e., drop shipment), rather than through an intermediate distribution center. If not, please clarify what fulfillment models are permitted and whether use of a distribution center is allowed under any circumstances.

Drop ship to the VA facility.

Would the Government allow Return Authorization / Shipping Label confirmation within eight (8) hours instead of four (4) hours? No .

Could you please clarify what equipment the Government expects to be brought to the VA facility? The surgical packs. No additional equipment is required.

Please confirm that the Total Shoulder Pack is not included in the scope of this solicitation. Based on SF1449, p. 74, Section 2.1.1.7 (Pack Components), the

Total Shoulder Pack does not appear to be listed.

Could you please confirm whether column L in Attachment B represents kits per case? Yes. Column L is kits per case.

Could you please confirm whether column J in Attachment B reflects annual usage? Yes. Column J represents estimated annual usage.

Could you please clarify what “NSP” stands for in column N of Attachment B? Not Seperately Priced.

Could you please provide the quantities for each component included in the pack?

See Attachment A, Bill of Materials for Surgical Packs, for the qantities of each component.
