B.3 STATEMENT OF WORK

Introduction: The Fayetteville VA Coastal Health Care Center 2300 Ramsey Street, Fayetteville NC, 28301 requires Service for testing steam quality at access points, with a direct (dead weight) calorimeter at the following locations:

Fayetteville Veterans Affairs Medical Center (VAMC)Fayetteville Health Care Center (HCC)
2300 Ramsey St.7300 S. Raeford Rd.
Fayetteville, NC 28301Fayetteville, NC 28304

Wilmington Health Care Center (HCC) 1705 Gardner Rd.

Wilmington, NC 28405

1. Background:

Testing steam quality compliance with Veterans Health Administration (VHA) directive 1116 (2) listed AAMI standards. Steam Quality Test methodology is described in ANSI/AAMI ST79 Comprehensive guide to steam sterilization and sterility assurance in health care facilities. Testing and resolution of steam quality issues saves cost, time, effort, and reduces risk.

2. Statement Of Work:

2.1. The contractor shall furnish labor, parts, supplies and equipment essential for conducting preventive maintenance on the equipment. Services must be executed in alignment with the terms and conditions outlined in this statement of work and must adhere to all technical specifications set forth by the manufacturers, thereby preserving the original equipment warranty.

2.2. Test steam quality at access point, with a direct (dead weight) calorimeter. The test requires a sampling tube (we provide) inserted in the steam supply pipe. A few pounds of steam are condensed in a water reservoir. Weight and temperature changes allow the steam quality to be calculated (also called “dryness fraction” in the standards).

2.3. Measure condensates return temperature to check for evidence of back pressure. Inspect return system.

2.4. Electronically verify steam system drainage and measure insulation performance at:

a. steam supply piping (to boiler room, powerhouse, or district heat valve).

b. intermediate 50-60 psi reducing station piping.

c. run out piping, and sterilizer take-off piping.

2.5. Inspect the entire steam supply for drain legs that might be forgotten, missing, disconnected, rerouted, clogged, have failed (or worn out) valves, or be installed incorrectly.

2.6. Measure water quality of condensate, wash water, and final rinse.

2.7. Check boiler operation records and procedures for TDS control, bottom, and surface blowdown performance.

2.8. The Contractor shall perform to the standards in this contract.

2.9. This SOW requires the work to be performed by qualified and certified Steam Quality Professionals.

2.10. The Contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved service personnel to the Contracting Officer (CO) and/or the Contracting Officer’s Representative (COR). Contractor shall revise this documentation whenever service personnel are assigned or re-assigned to the FVAMC.

2.11. The Contracting Officer or COR reserve the right to request authentication of factory training certificates or credentials from the Contractor at any time for any personnel who are servicing FVAMC equipment.

2.12. The Contractor shall be required to sign in with the COR after the contract is awarded, before beginning a service call, emergency service, etc.

2.13. Upon completion of work the Contractor shall leave a copy of the service report with the using service and submit to the same location as designated above a service report annotating what service, calibration, etc., were performed and whether the work was done.

2.14. The service designee and the FSE (Field Service Engineer) attest that the completed work is in satisfactory working order and functions at an acceptable level of performance shall sign this report. The contractor shall sign out at the designated location before leaving.

NOTE:

· Failure to furnish a service report or obtain a signed copy of the service report could result in delay of payment or non-payment. The COR or Alternate COR in the using service has the authority to approve/request a service call from the contractor.

3. Unscheduled and Emergency Service: All Work will be performed during the normal VA business hours (8:00 - 16:30) Monday through Friday, except Federal holidays, unless otherwise specified. The Contractor may work outside normal business hours by arrangement with the COR if such services are provided without additional charge to the Government.

4. Performance Working Hours:

· Government normal duty hours are from 0800-1630, Monday through Friday, excluding Federal Holidays observed by the Veterans Administration are listed below and revised to include any other day specifically declared by the President of the United States to be a national holiday.

The offeror further acknowledges that this certification is subject to Title 18, United States Code, Section 1001, and, as such, a false, fictitious, or fraudulent certification may render the offeror subject to criminal, civil, or administrative penalties, including prosecution.
