The Department of Veterans Affairs, VISN 6 Network Contracting Office (NCO) is conducting Market Research to find qualified small business manufactures and distributors, registered under NAICS code 336211; with size standard of 1,245 employees, capable of supplying a Turnkey Mobile Lithotripter unit to provide non-surgical treatment using Extracorporeal Shock Wave Lithotripsy (ESWL) of kidney stones at Salisbury VA Medical Center. This requirement will consist of approximate 30-60 cases per year.
ITEM INFORMATION:
0001- the Extracorporeal Shock Wave Lithotripsy (ESWL) must be self-contained and will be located on VAMC Station grounds on an outdoor prepared site/pad.
All interested parties shall respond, in writing via email to gordon.burns@va.gov no later than 1pm EST August 17, 2024. No telephone inquiries will be accepted. In response, please include the following:
· Company/individual name,
· Address,
· Point of contact with phone number
· Information describing your interest,
· Indicate FSS/GSA contract number or Open Market, as applicable
· FSS Category you qualify for
· Tax ID number
· DUNS number
· Capabilities statement
· Company Socio-Economic SIZE (example…large, small, SDVOSB, VOSB, WOSB etc.)
This requirement may be subject to the non-manufacturer rule. If the contractor can provide the above items, the contractor shall provide a letter from the large or small business manufacturer stating that they are an authorized distributor/licenser of their products. Failure to submit an authorized distributor letter shall result in the product(s) potentially offered to be considered “grey market” and shall not be considered technically acceptable.
Interested firms are reminded that in accordance with FAR 4.1201(a) Prospective contractors shall complete electronic annual representations and certifications at the System for Award Management (SAM) as a part of required registration in conjunction with FAR 4.1102(a) that states, Prospective contractors shall be registered in the (SAM) database prior to award of a contract or agreement. Therefore, due to time constraints, vendors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award.
