This notice is separate from any notices required for utility shutdown described later in this section. All work to be coordinated around an operational medical center as to minimize impact to patient care.

This Scope of Work is for One (1) base year and four (4) option years

Scope of Work: The contractor shall provide all labor, personnel, supervision, equipment, tools, and all materials necessary to perform preventive maintenance, repair and certification on the chemical fume hoods/biological safety cabinets and Laminar Flow devices and assess the Primary Environmental Controls for the associated spaces where aforementioned equipment is located in the Durham Veterans Affairs Health Care System. The frequency of the preventative maintenance and certification is semi-annual.

Testing Standards: All testing of biosafety cabinets to be performed in accordance with criteria established by the National Sanitation Foundation (NSF) International standard number # 49-2002e, Annex F, or the prevailing NSF standard at time of inspection. All fume hood testing to be performed in accordance with criteria established by ANSI/ASHRAE 110, or the prevailing standard at time of inspection. All testing of laminar flow units to be done in accordance with criteria established by IEST-RP-CC002.2, Federal Standard 209E, ISO 14644 and the OEM specifications. All testing of animal isolator racks/units to be performed in accordance with criteria established by IEST-RP-CC002.2, Federal Standard 209E and the OEM specifications. Hoods are to be certified semiannually. Primary Environmental Controls test to be performed per ANSI/AIHA Z9.5-2003 American National Standards for Laboratory Ventilation.

Height: Height shall be based on the dimension from the bottom most part of the sash to the work surface located in a straight plane directly beneath the sash for vertical sashes, or the opening between horizontal tracks on horizontal sashes.

Width: Width shall be based in the interior dimensions of the design sash opening.

The average face velocity shall be determined by the method described in the ANSI/ASHRAE 110-1995 Method of Testing Performance of Laboratory Fume Hoods. Face velocity measurements shall be made by dividing the hood opening into equal area grids with sides measuring no more than 12 in. (30.5 cm). The tip of the probe shall be positioned in the plane of the sash opening and fixed (not handheld) at the approximate center of each grid. Grid measurements around the perimeter of the hood opening shall be made at approximately 6 in. (15.2 cm) from the top, bottom, and sides of the opening enclosure. The average face velocity shall be the average of the grid velocity measurements. Each grid velocity shall be the average of at least 10 measurements made over at least 10 seconds. The plane of the sash shall be located at the midpoint of the sash frame depth. Manufacturer’s specifications of each piece of equipment shall also be used for testing.

Face Velocity Measurement. A 1.0 ft2 imaginary grid pattern shall be formed by equally dividing the design hood opening into vertical and horizontal openings. Velocity readings shall be taken with a calibrated anemometer. For VAV hoods, measurements should be made at 25, 50, and 100% sash openings. A hood is considered certified when the average face velocity at the manufacturer recommended working sash height is between 80 – 120 feet per minute.

Routine performance tests shall be conducted in accordance with the SOW. A hood that is found to be operating with an average face velocity more than 10% below the designated average face velocity shall be labeled as OUT OF SERVICE and corrective actions shall be taken by VA Engineering staff to adjust and/or replace belts. Each hood shall be posted with a notice giving the date of the routine performance test, the measured average face velocity and initialed by the technician. If it is taken OUT OF SERVICE, it shall be posted with an out-of-service notice.

PART 2 PRODUCTS – NOT USED97
PART 3 - EXECUTION97
3.1GENERAL REQUIREMENTS97
Establish and maintain an effective quality control (QC) system. that complies with the FAR Clause 52.246.12 titled “Inspection of Contract”. QC consists of plans, procedures, and organization necessary to produce an end product which complies with the Contract requirements. The QC system covers all design and contract operations, both onsite and offsite, and be keyed to the proposed design and contract sequence. The project superintendent will be held responsible for the quality of work and is subject to removal by the Contracting Office or Authorized designee for non-compliance with the quality requirements specified in the Contract. In this context the highest-level manager responsible for the overall contract activities at the site, including quality and production is the project superintendent. The project superintendent maintains a physical presence at the site at all times and is responsible for all contract and related activities at the site, except as otherwise acceptable to the Contracting Officer.97
3.2CQC PLAN:98
A.Submit the CQC Plan no later than 15 days after receipt of Notice to Proceed (NTP) proposed to implement the requirements of the FAR Clause 52.246.12 titled “Inspection of Contract”. Design and/or contract will be permitted to begin only after acceptance of the CQC Plan or acceptance of an Interim plan applicable to the particular feature of work to be started. Work outside of the accepted Interim CQC Plan will not be permitted to begin until acceptance of a CQC Plan or another Interim CQC Plan containing the additional work scope is accepted.98
B.Content of the CQC Plan: Include, as a minimum, the following to cover all contract operations, both onsite and offsite, including work by subcontractors, fabricators, suppliers, and purchasing agents:98
C.Acceptance of Plan: Acceptance of the Contractor’s plan is required prior to the start of design and contract. Acceptance is conditional and will be predicated on satisfactory performance during the design and contract. The Government reserves the right to require the Contractor to make changes in the CQC Plan and operations including removal of personnel as necessary, to obtain the quality specified.100
D.Notification of Changes: After acceptance of the CQC Plan, notify the Contracting Officer or Authorized designee in writing of any proposed change. Proposed changes are subject to acceptance by the Government prior to implementation by the Contractor.100
1.3COORDINATION MEETING:100
After the Precontract Conference Post-award Conference before start of design or contract, and prior to acceptance by the Government of the CQC Plan, meet with the Contracting Officer or Authorized designee to discuss the Contractor’s quality control system. Submit the CQC Plan a minimum of 5 business days prior to the Coordination Meeting. During the meeting, a mutual understanding of the system details must be developed, including the forms for recording the CC operations, design activities (if applicable), control activities, testing, administration of the system for both onsite and offsite work, and the interrelationship of Contractor’s Management and control with the Government’s Quality Assurance. Minutes of the meeting will be prepared by the Government, signed by both the Contractor and Contracting Officer or Authorized designee and will become a part of the contract file. There can be occasions when subsequent conferences will be called by either party to reconfirm mutual understandings or address deficiencies in the CQC system or procedures which can require corrective action by the Contractor.100
1.4QUALITY CONTROL ORGANIZATION:101
A.Personnel Requirements: The requirements for the CQC organization are a Safety and Health Manager, CQC System Manager, a Design Quality Manager (if applicable), and sufficient number of additional qualified personnel to ensure safety and Contract compliance. The Safety and Health Manager shall satisfy the requirements of Specification 01 35 26 Safety Requirements and reports directly to a senior project (or corporate) official independent from the CQC System Manager. The Safety and Health Manager will also serve as a member of the CQC Staff. Personnel identified in the technical provisions as requiring specialized skills to assure the required work is being performed properly will also be included as part of the CQC organization. The Contractor’s CQC staff maintains a presence at the site at all times during progress of the work and have complete authority and responsibility to take any action necessary to ensure Contract compliance. The CQC staff will be subject to acceptance by the Contracting Officer or Authorized designee.
