1. Is the contractor collecting the samples, or will the contractor provide supplies/shipping materials/shipping labels for VA staff to collect the samples?

See Paragraph 3.1 of the revised Performance Work Statement. The contractor is responsible for providing the sample collection supplies and shipping boxes. A designated government personnel will collect the sample for shipment to contractor’s address

2. How many sample locations are there at each site? The price schedule seems to indicate 60 for Durham and 36 for Greenville, but the table on page 14 of the RFQ and on the amendment indicates 60 are for Greenville and I assume the 36 are for Durham.

See the revised Price Schedule Paragraph 3.1 of the revised Performance Work Statement. There are five (5) sample locations at the VAMC Durham site and 3 sample locations at the Greenville CBOC site.

3. The table in the amendment indicates that samples will be collected annually. Is that correct? Some of the tests listed in the table are recommended to be tested monthly, others annually, and other quarterly until within limits for four quarters--then annually. The quantity of samples of 60 and 36 for each site seems excessive for once a year. Please confirm the number of sample locations and the frequency of testing for each test.

See Paragraph 3.1 of the revised Performance Work Statement. These numbers are based on points of contact (spouts) for water testing. There are five (5) sample collecting locations at the VAMC Durham site and 3 sample collecting locations at the Greenville CBOC site. Each individual sample test for each sample collecting location shall cover all ten (10) test requirements listed in Tables 1 and Table 2 of the revised Performance Work Statement.

B.3 PERFORMANCE WORK STATEMENT

1. CONTRACT TITLE. Critical Water Systems Testing and Reporting for Durham VA Health Care System Sterile Processing Services.

2. BACKGROUND/DATES OF SERVICE (PERIOD OF PERFORMANCE).

The National Program Office for Sterile Processing Service (NPOSP) and the Office of Healthcare Environment and Facilities Program (HEFP) require the testing of critical and utility water systems impacting patient care. The Durham VA Health Care System (DVHCS) have a need for water sampling testing services to meet the water testing requirements for sterile processing services in accordance with VHA Directive 1116 (2) AAMI TIR-34 Testing Protocol. The contractor shall provide sample testing and reporting services in accordance with the performance work statement. The sampling collections shall occur at the VA Medical Center in Durham, NC and Greenville CBOC, NC.

3.1 SAMPLE COLLECTION, SHIPPING AND REPORTING

The contractor shall provide the sample collection supplies (sample collection containers and shipping boxes) to the contracting officer representative (COR). The COR or a designated Government personnel shall collect and ship the sample to the contractor’s address for testing. The Government shall cover the samples tests postage/shipping cost to the contractor’s address.

The contractor shall provide five (5) samples tests per month for Durham VAMC and three (3) samples tests per month for Greenville CBOC. Each individual sample test for the respective facility shall cover all ten (10) test requirements in Tables 1 for VAMC Durham and Table 2 for Greenville CBOC.

The contractor shall provide the sample test result reports to the COR three to five business days after receiving test sample. The sample test result reports shall be provided to the COR via email.

5. TESTING STANDARDS AND PERFORMANCE MONITORING:

Sterile Processing Service and COR shall ensure that the contractor’s performance of the critical water system testing, and reporting requirements are performed in accordance with the performance work statements.

The contractor shall submit a detailed and analytical sample test result reports to the COR for a review of completed tests. The COR shall review the reports to ensure that the contractor’s performance is in accordance with the terms and conditions of the contract and the Quality Assurance Surveillance Plan (QASP).
