Subject: Asbestos Testing Services Summary Sheet Upgrade Fire Alarm System, Project #658-19-105 Salem VA Medical Center in Salem, Virginia H&P Project 20192002
Mr. Collins, Hurt & Proffitt, Inc. (H&P) was requested to conduct sampling of asbestos containing materials (ACM) in the areas that will be impacted by the work for the Fire Alarm System Upgrade Project. For the surveys, H&P was provided with construction drawings showing the locations where work is planned noting the specific work scope in the respective area. Based on our review of the drawings, sampling was performed on targeted suspect ACM in each work. The targeted sampling was limited to surfaces where components will be mounted and excluded those that could be affected by running of lines since their routing will be determined in the field by the installation contractor. Below is a summary of the materials that were identified as asbestos containing by sampling and analysis listed per building. For a detailed listing of all materials sampled, refer to the respective building’s Asbestos Survey Report.
Building 04
Asbestos containing materials were not identified in the areas and materials sampled during this survey.
Building 05
Asbestos containing materials were not identified in the areas and materials sampled during this survey.
Building 09
Asbestos (3% Chrysotile) was identified in the white mastic on ducting collected on the basement level in Room 2. All similar white duct mastic should be presumed to be asbestos containing.
Building 11
Asbestos (3% Chrysotile) was identified in the white mastic on ducting collected in the 2nd floor hall by Room 223. All similar white duct mastic should be presumed to be asbestos containing.
mailto:coliins@avetechdg.com
A trace (<1%) level of asbestos was detected in the caulking in the brick mortar joints located in the 1st floor exterior open porch area. This level is below the regulatory threshold under EPA regulations for defining a material as regulated and “asbestos containing”. However, OSHA has regulations for the removal and disturbance of trace levels of asbestos; therefore, H&P recommends that the owner assume that all similar caulking in the area is asbestos containing at a trace level.
Building 13
Asbestos containing materials were not identified in the areas and materials sampled during this survey.
Building 14
Asbestos containing materials were not identified in the areas and materials sampled during this survey.
Building15
A trace (<1%) level of asbestos was detected in the plaster skim coat on the wall in the 1st floor hallway.
This level is below the regulatory threshold under EPA regulations for defining a material as regulated and “asbestos containing”. However OSHA has regulations for the removal and disturbance of trace levels of asbestos; therefore, H&P recommends that the owner assume that all skim coat in the area is asbestos containing at a trace level.
Building16
Asbestos containing materials were not identified in the areas and materials sampled during this survey.
Building 75
Asbestos (5% Chrysotile) was identified in the white duct mastic collected in the 2nd floor hall by Room
252. All similar white duct mastic should be presumed to be asbestos containing.
Asbestos (3% Chrysotile) was identified in the white duct mastic collected in the basement level in Room
12. All similar white duct mastic should be presumed to be asbestos containing.
A trace (<1%) level of asbestos was detected in the drywall joint compound collected on the 1st floor hallway area by the stairwell. This level is below the regulatory threshold under EPA regulations for defining a material as regulated and “asbestos containing”. OSHA has regulations for the removal and disturbance of trace levels of asbestos; therefore, H&P recommends that the owner assume that all joint compound in the area is asbestos containing at a trace level.
Building76
Asbestos (2% Chrysotile) was identified in the drywall joint compound collected in the basement level in the Locksmith Room. All similar wall joint compounds should be presumed to be asbestos containing.
Building 145
Asbestos containing materials were not identified in the areas and materials sampled during this survey.
Presumed Asbestos Containing Materials
During the conduct of this survey, sampling was limited to those materials which were within the areas designated by the client, which were safely accessible, and were able to be sampled without damaging systems or structures. Additionally, the facility and multiple rooms were occupied and in use at the time or our survey; which limited our ability to access all areas. As such, other materials not represented in the respective building’s survey should be presumed to be positive, unless sampling is conducted and they are shown to be negative for asbestos. Some components that were not accessible and/or could not be sampled include:
Electrical equipment wiring (i.e. control panels) and switches and panel backer boards;
Fire doors where magnetic locks will be installed. If the scope of work will disturb the inner door core, then this material must be presumed to be asbestos containing and should be treated as such;
unless sampling and analysis is performed and confirms the absence of asbestos in the material.
Thermal systems insulation that may be present in wall cavities or above ceilings and; if the scope of work will disturb the material, then it must be presumed to be asbestos containing and should be treated as such, unless sampling and analysis is performed and confirms the absence of asbestos in the material.
If work is performed that will impact suspect materials that have not been sampled, it is recommended that they be sampled by a Virginia licensed Asbestos Inspector prior to disturbance.

Subject: Building 4 Asbestos Testing Services Upgrade Fire Alarm System, Project #658-19-105
Hurt & Proffitt, Inc. (H&P) was requested to conduct sampling for asbestos containing materials (ACM) that will be impacted by the work for the fire Alarm System Upgrade Project. This survey was conducted in Building 4 at the Salem Veterans Affairs Medical Center in Salem, Virginia. The sampling was performed by H&P representatives Mr. Roy Wriston (Virginia Asbestos Inspector License number is 3303 003648) and Ms. Mary Beth Wriston (Virginia Asbestos Inspector License number is 3303 003466) on December 16-17, 2019 and January 10, 2020. This letter and attachments represent H&P’s report for the above-referenced project.
1.0 Asbestos Survey Procedures and Findings
H&P was provided with construction drawings showing the general locations where work is planned noting the specific work scope in the respective areas. Based on our review of the drawings, sampling was performed to target suspect ACM in each work area based on the work task type. The targeted sampling was limited to surfaces where components will be mounted and excluded materials that could be affected by running of lines since their routing will be determined in the field by the installation contractor. Therefore, the approach of the survey was to sample representative suspect ACM in the locations designated on the drawings in order to provide an understanding of:
Materials that are asbestos containing and require abated prior to disturbance;
Identify those that are not asbestos containing via sampling and analysis.
The survey included the collection and submittal of 50 individual samples of suspect asbestos containing materials (ACM) that were analyzed for a total of 75 layers. The sampling was limited to non-invasive means and was performed in accordance with state and federal regulations.
The suspect asbestos samples were submitted for analysis by EPA Method No. 600/R-93/116 and 600/M4-82-020 polarized light microscopy (PLM). All samples were analyzed by SanAir Technologies
Laboratory of Powhatan, Virginia, a NVLAP accredited laboratory licensed to perform asbestos bulk analysis within the State of Virginia. The sampling results are shown in the following table. Copies of the laboratory Asbestos Analysis Bulk Report and Chain of Custody forms are included as an attachment to this report

2.0 Analysis Results
Based on the results of sample analysis, asbestos containing materials were not identified in the areaas and materials sampled during this survey.
2.1 Presumed Asbestos Containing Materials
During the conduct of this survey, sampling was limited to those materials which were within the areas designated by the client, which were safely accessible, and were able to be sampled without damaging systems or structures. Additionally, the facility and multiple rooms were occupied and in use at the time or our survey; which limited our ability to access all areas. As such, other materials not represented in this survey should be presumed to be positive, unless sampling is conducted and they are shown to be negative for asbestos. Some components that were not accessible and/or could not be sampled include:
core, then this material must be presumed to be asbestos containing and should be treated as such;
unless sampling and analysis is performed and confirms the absence of asbestos in the material.
Thermal systems insulation that may be present in wall cavities or above ceilings and; if the scope of work will disturb the material, then it must be presumed to be asbestos containing and should be
If work is performed that will impact suspect materials that have not been sampled, it is recommended that they be sampled by a Virginia licensed Asbestos Inspector prior to disturbance. We note that during renovation or demolition activities, it is not uncommon to reveal materials that were not accessible during the survey or their presence could not otherwise be known.
3.0 Applicable Regulations
Following are some applicable regulations pertaining to asbestos containing materials.
3.1 EPA/NESHAP Regulations for Asbestos Containing Materials
The U.S. Environmental Protection Agency promulgated the National Emission Standards for Hazardous Air Pollutants (NESHAP) [40 CFR Part 61], which addresses the application, removal, and disposal of asbestos-containing materials (ACM). Under NESHAP the following categories are defined for asbestos-containing materials:
Friable - When dry, can be crumbled, pulverized, or reduced to powder by hand pressure.
Non-friable - When dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure.
Category I Non-friable ACM - Packings, gaskets, resilient floor coverings, and asphalt roofing products containing more than 1% asbestos.
Category II Non-friable ACM – Any material, excluding Category I Non-friable ACM, which contains more than 1% asbestos.
Regulated Asbestos Containing Material (RACM) – One of the following:
1. Friable ACM
2. Category I Non-friable ACM that has become friable.
3. Category I Non-friable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading.
4. Category II Non-friable ACM that has a high probability of becoming, or has become, friable by the forces expected to act on the material in the course of demolition or renovation operations.
Under NESHAP, the following actions are required:
1. Prior to the commencement of demolition or renovation activities, the building owner must inspect the affected facility or part of the facility where the demolition or renovation activities will occur for the presence of asbestos.
2. Remove all RACM from the facility before any activity begins that would break up, dislodge, or similarly disturb the material or preclude access for subsequent removal.
3. RACM need not be removed if:
a) It is Category I non-friable ACM that is not in poor condition.
b) It is on a facility component that is encased in concrete or other similar material and is adequately wet whenever exposed.
c) It was not accessible for testing and was therefore not discovered until after demolition began and because of the demolition the material cannot be safely removed.
d) It is Category II non-friable ACM and the probability is low that the material will become crumbled, pulverized, or reduced to powder during demolition.
Note: Based on our experience, we recommend that all ACM be removed prior to renovation or demolition due to the fact that disturbance can damage them, create potential exposure to workers and occupants and make them friable. This recommendation is made as a best practice to reduce potential exposure and limit liability.
3.2 OSHA
The Occupational Safety and Health Administration (OSHA) regulates employee exposure to asbestos under 29 CFR 1926.1101 and 29 CFR 1926.1001. Work associated with known or suspect asbestos containing materials (ACM), including trace (<1%) levels of asbestos, must be conducted according to these regulations in addition to the noted EPA regulations where applicable.
If the materials that have been identified as asbestos containing will be disturbed or are in poor condition, it is recommended that the asbestos be removed by an appropriately trained and licensed asbestos abatement contractor in accordance with Virginia, EPA and OSHA regulations.
9.0 Limitations
This report summarizes the results of our sample collection and analysis of the suspect ACM materials at the facility based on our understanding of the requested scope of work. The findings prepared by H&P are based upon our observations and the analytical analysis of the samples collected at the time of our field inspection. The services performed were provided in accordance with generally accepted environmental industry standard practices. No warranty, expressed or implied, is made. Our observations, conclusions and recommendations are based upon conditions readily visible at the site at the time of our visit, the results of testing and analysis and information provided to us by others.
Our conclusions and recommendations are based on the guidelines presented by the EPA, Commonwealth of Virginia and OSHA. Any conditions discovered which deviate from the data contained in this report should be presented to us for our evaluation. This report has been prepared for the exclusive use of the client/or their agents. This service was performed in accordance with generally accepted environmental practices. Our conclusions and recommendations are based, in part, upon information provided to us by others; we have not verified the completeness or accuracy of the information provided by others, unless otherwise noted.
During H&P’s non-invasive inspection, accessible areas were visually surveyed for the presence of ACM.
Areas reviewed were limited to those identified by the Client and that could be readily and safely accessed. Our conclusions and recommendations are based on the results of our testing and cannot be used to form a professional opinion of conditions in other areas beyond those from which the samples were collected. It is possible that inaccessible areas, such as behind walls or above ceilings, may not have been surveyed and therefore conditions in these areas are unknown.
During this study, suspect asbestos samples were submitted for analysis at an NVLAP-accredited laboratory via polarized light microscopy. As with any similar survey of this nature, actual conditions exist only at the precise locations from which samples were collected. Certain inferences are based on the results of this sampling and related testing to form a professional opinion of conditions in areas beyond those from which the samples were collected. It is also understood that this is a limited survey so that it is possible that concealed materials may be present that were not accessible to us. No other warranty, expressed or implied, is made.
H&P assumes no responsibility regarding response actions initiated as a result of our findings; nor liability for the duties and responsibilities of the Client or building owner with respect to compliance with applicable regulations. Compliance with regulations are the responsibility of the Client or building owner in accordance with local, state, and/or federal requirements.

Fibers smaller than 5 microns cannot be seen with this method due to scope limitations. The accuracy of the results is dependent upon the client’s sampling procedure and information provided to the laboratory by the client. SanAir assumes no responsibility for the sampling procedure and will provide evaluation reports based solely on the sample and information provided by the client. This report may not be used by the client to claim product endorsement by NVLAP or any other agency of the U.S. government. Samples are held for a period of 60 days.
For NY state samples, method EPA 600/M4-82-020 is performed.
Polarized- light microscopy is not consistently reliable in detecting asbestos in floor covering and similar non-friable organically bound materials. Quantitative transmission electron microscopy is currently the only method that can be used to determine if this material can be considered or treated as non-asbestos containing.

Subject: Building 5 Asbestos Testing Services Upgrade Fire Alarm System, Project #658-19-105
Hurt & Proffitt, Inc. (H&P) was requested to conduct sampling for asbestos containing materials (ACM) that will be impacted by the work for the Fire Alarm System Upgrade Project. This survey was conducted in Building 5 at the Salem Veterans Affairs Medical Center in Salem, Virginia. The sampling was performed by H&P representatives Mr. Roy Wriston (Virginia Asbestos Inspector License number is 3303 003648) and Ms. Mary Beth Wriston (Virginia Asbestos Inspector License number is 3303 003466) on December 17-18, 2019.
The targeted sampling was limited to surfaces where components will be mounted and excluded materials that could be affected by running of lines since their routing will be determined in the field by the installation
The survey included the collection and submittal of 26 individual samples of suspect asbestos containing materials (ACM) that were analyzed for a total of 37 layers. The sampling was limited to non-invasive means and was performed in accordance with state and federal regulations.
The suspect asbestos samples were submitted for analysis by EPA Method No. 600/R-93/116 and 600/M4-82-020 polarized light microscopy (PLM). All samples were analyzed by SanAir Technologies analysis within the State of Virginia. The sampling results are shown in the following table. Copies of the laboratory Asbestos Analysis Bulk Report and Chain of Custody forms are included as an attachment to this report

Based on the results of sample analysis, asbestos containing materials were not identified in the areas and materials sampled during this survey.
As such, other materials not represented in negative for asbestos. Some components that were not accessible and/or could not be sampled include:
core then this material must be presumed to be asbestos containing and should be treated as such;
unless sampling and analysis is performed and confirms the absence of asbestos in the material.
We note that during renovation or demolition activities, it is not uncommon to reveal materials that were not accessible during
3.1 EPA/NESHAP Regulations for Asbestos Containing Materials
4. Category II Non-friable ACM that has a high probability of becoming, or has become, friable by the inspect the affected facility or part of the facility where the demolition or renovation activities will
3. RACM need not be removed if:
a) It is Category I non-friable ACM that is not in poor condition.
b) It is on a facility component that is encased in concrete or other similar material and is adequately wet whenever exposed.
c) It was not accessible for testing and was therefore not discovered until after demolition began and because of the demolition the material cannot be safely removed.
demolition due to the fact that disturbance can damage them, create potential exposure to workers and occupants and make them friable. This recommendation is made as a best practice to reduce potential exposure and limit liability.
3.2 OSHA
The Occupational Safety and Health Administration (OSHA) regulates employee exposure to asbestos under 29 CFR 1926.1101 and 29 CFR 1926.1001. Work associated with known or suspect asbestos these regulations in addition to the noted EPA regulations where applicable.
Compliance with regulations are the responsibility of the Client or building owner g.whitt@handp.com or Mary Beth Wriston at 540-588-0941 or via email at m.wriston@handp.com. We appreciate the opportunity to serve as your environmental consultant.
