X The purpose of this amendment is to answer solicitation questions and correct the solicitation as needed. See following:

a. Solicitation Questions and Answers are attached.

b. Becuase of the quantity and depth of questions a revised solicitation is attached with all changes noted in Green.

c. The Solicitation proposal due date is extended to 23 Aug 2019 at 2:30 pm ET.

Mario L. Santiago Contracting Officer Attachment 1- Solicitation 36C24619R0161 Questions and Answers Psychiatric Physician Services for Fayetteville VAMC

1. Question: FBO submission and archive deadline shows 7/31/19, however SF1449 indicates 8/6/19 but no time listed. Please clarify.

Answer: Proposals are due changed to 23 Aug 2019 at 2:30pm.

1. Question: SOW page 7, item 1.4 indicates: "The contract psychiatrist may also be utilized in the Inpatient Psychiatric Unit at Fayetteville, NC in the event a staff psychiatrist reports to duty prior to completion of the contract period. It is the VA’s intention to solicit quotes under the Federal Supply Schedule 621 and to award the contract that is determined to be the best value to the Government and pending availability of funds." This appears to be in conflict with this open market solicitation. Will you consider removing this 1.4 section? Otherwise, it implies Vendors would be required to meet the Federal Supply Schedule 621 I requirements in order to submit a proposal for this opportunity.

Answer: The wording “under the Federal Supply Schedule 621” has been replaced by “via Open Market”.

1. Question: SOW page 13, item d. indicates "Name of individuals providing services under the contract and their applicable licenses must be submitted to the Contracting Officer with the offer, including individual Social Security Numbers, dates of birth, place of birth (city, state and country) and addresses in order to comply with VA Credentialing and Privileging requirements" However, few candidates will be willing to provide this information at the proposal phase, but would comply upon award. Please consider removing this requirement for the proposal?

Answer: This wording and requirement is provided in all of our Locums solicitations. Part of the Technical Evaluation is the review of the physician’s CVs, which are requirement as part of your proposal.

1. Question: SOW on page 8, Section 4, item a. indicates: "Contractor personnel shall be Board Certified Psychiatrist proficient in the diagnosis and treatment of psychiatric conditions....and licensed in State of North Carolina, commonwealth of the US or D.C...." However, in other sections of the solicitation it indicates Board Eligible candidates will be considered. Please clarify if you are asking for only Board Certified candidates for this solicitation and whether or not ANY State Medical license is accepted including U.S. Territories and D.C.?

Answer: A global find and replace was performed and all references to Board Certified now includes Board Eligible.

1. Question: SOW indicates travel to CBOC's may be required. I do not see a CLIN for mileage between facilities listed. Will you consider adding verbiage to contract to indicate mileage will be reimbursed if the provider(s) on contract are assigned to work at one of the CBOC clinics listed? (Separate from the CLIN 1002 on the pricing sheet.)

Answer: If a physician is assigned to work at on of the CBOC clinics mileage is authorized. But the expectation is and was that the Physician would be assigned for a longer period or even permanently, so they would acquire residence in the CBOC location.

1. Question: Is there an incumbent vendor providing these services? If so, what is the contract award number?

Answer: Yes, 36C24618C0008.

1. Question: SOW on page 9 indicates 12-15 patients per day (to include weekends on a rotational basis) to inpatients. Please clarify the patient volume expected per day for outpatient is 12-15 and it could be 12-15 for inpatient coverage on rotating weekends? If not the same patient count for inpatient on weekends, what are those inpatient volumes? And, please clarify if any inpatients are to be seen during normal clinical hours in addition to the 12-15 daily outpatient count?

Answer: Patient expectation for outpatient volume is 12-15 per day for the 8:00 AM – 4:30 PM shift. Locums don’t rotate weekends nor cover the inpatient unit, to include during normal clinical hours.

1. Question: What is rotation schedule? 1 in 2 or 1 in 3, etc.

Answer: Locums don’t rotate weekends. They work weekdays as noted above covering outpatient veterans’ appointments & walk-ins.

1. Question: We noticed Past Performance Questionnaires are being requested. Due to fast turn-around and challenges with obtaining these from our clients, will you consider using our CPARS and a listing of our contact for recent/current contracts in lieu of the PPQ's?

Answer: Past Performance Questionnaires are required for all of our requirements. We also may us CPARS, your submittal and we may call your references.

1. Question: (Past Performance Questionnaires are very difficult to obtain from our commercial clients and Federal clients. In most cases, our clients are required to have legal department review before the client can respond (subject to legal approval) which can take as long as 3 weeks. In other cases, our clients refuse to complete these as it is of no benefit to them and is time consuming which takes them away from their current job tasks. In addition, they consider these as a liability concern. Since there is no FAR requirement requiring PPQ's, we appreciate your consideration if we cannot include these in our proposal but you will still be able to accept our proposal for evaluation.)

Answer: Past Performance Questionnaires are required for all of our requirements. We also may us CPARS, your submittal and we may call your references.

1. Question: Is Board Certification a requirement?

Answer: See first paragraph of the Performance Work Statement, i.e., Six (6) “Board Certified/Board Eligible Psychiatrist”.

1. Question: What is the minimum # of psychiatrists required in order to submit?

Answer: See first paragraph of the Performance Work Statement, i.e., Six (6) “Board Certified/Board Eligible Psychiatrist”.

1. Question: Can the contracting team please comment on / provide context for why the hours/travel, lodging & Per diem/Rental car units are all different from Option years 1 & 2?

Answer: We realize that it takes time to credential and get candidates through the PIV process at the beginning of the contract period before they can work. These figures are estimated and will be appropriately adjusted as needed.

1. Question: Will this be Outpatient and Inpatient? If so, can the contracting team please comment on what percentage of each can be expected?

Answer: Outpatient

1. Question: Will there be any call hours associated with this project?

Answer: Locums don’t rotate on weekends, so no call hours will be required

1. Question: Is there an existing contract currently in place that covers the services outlined in this solicitation?

Answer: Yes, 36C24618C0008.

1. Question: If there is an existing contract and vendor currently performing these services. Does this new solicitation vary in terms of the scope of work?

Answer: Each requirement we solicit stands on its own. This requirement may or may not have enhancements or reductions to the current or last contract.

1. Question: Are the not to exceed (NTE) costs for CLIN 0002 to be considered for the “total base year price’ on page 5 of the RFQ?

Answer: Yes

1. Question: Are the NTE costs from CLIN 1002 to be consider for the ‘total option year 1 price’ per page 5 of the RFQ?

Answer: Yes

1. Question: Is there a maximum a vendor can be reimbursed for relocation per page 6: ‘Payment for relocation shall be based on actual expenses incurred” Answer: Yes, “and shall be reimbursed at rates not to exceed the maximum locality per diem rates (the combination of lodging, meals and incidentals) in effect at the time of travel, as set forth in the Federal Travel Regulations. Travel shall be approved in each case. All invoices for travel reimbursement shall be accompanied by supporting receipts. Airline tickets shall be non-refundable only.”

1. Question: Page 7 reads ‘Contract psychiatrist may also be utilized at any of the four (4) Community Based Outpatient Clinics (Hamlet, Wilmington, Jacksonville and Robeson County) or at the FVAMC, Fayetteville, NC

a. Question: Should a provider be expected to work in any of these locations any day?

i. Example: Will the provider be asked to work Monday at the Main Campus in Fayetteville then Tuesday in Jacksonville followed by Hamlet on Wednesday. This would make for a logistical challenge for lodging.

Answer: Locum provider will be assigned to one location only. Locums will not be expected to work at multiple locations during the work week.

b. Question: Will the VA be agreeable to a provider being assigned a single location for a 3-month period to offer the provide stability?

Answer: Not needed. Locums are assigned to single location only.

1. Question: Section 1.4 on page 7 mentions the 621-I FSS. Can you confirm this is an open market procurement not subject to the 621-I FSS?

Answer: The wording “under the Federal Supply Schedule 621” has been replaced by “via Open Market”.

a. Question: FSS pricing is also noted on page 61, will the VA remove the references to the 621-I?

Answer: This was changed via amendment 0001.

1. Question: Since this is not applicable to the 621-I FSS would the VA be willing to consider pricing responses that ‘ALL INCLUSIVE firm-fixed prices including per diem, housing, and costs of all required security clearances’ and remove the CLINs 0001 and 1001?

Answer: No, we do not do Cost Type contracts.

1. Question: Page 61 reads: ‘No mileage reimbursement will be authorized for personnel living within the 50-mile radius of Salem VAMC in Salem, VA.’. Should this reference the Fayetteville VAMC and associate clinics?

Answer: Yes, it should be Fayetteville. This was corrected via Amendment 0001.

1. Question: Is this requirement consistent with a non-personal services agreement that is listed on page 7: ‘Other duties, as assigned by mutual agreement between the locum tenens and the Chief and/or Medical Director, Mental Health Service Line”. Considering the non-personal service nature of this project wouldn’t this be more appropriate between the vendor and the COR?

Answer: Agree. This has been changed to COR or Contracting Officer.

1. Question: Page 6, section 6 states ‘Services shall include Psychiatric coverage Monday through Friday from 8:00 a.m. to 4:30 p.m. for 12-15 patients each day and to include weekends for inpatient coverage on a rotational basis.

25. Question: Can the VA clarify the hours?

Answer: Monday – Friday 8:00 AM to 4:30 PM

25. Question: Are weekend hours required?

Answer: No

25. Question: What are the weekend hours?

Answer: N/A

25. Question: Are there any hours required Monday through Friday after 4:30pm?

Answer: By exception and on occasion based off needs of situation

25. Question: Considering patient abandonment concerns what are the procedures for a provider if a consult request or other clinical request (walk-in) requires the provider to work past 4:30pm?

Answer: In the event of an unscheduled walk in that requires the provider to work beyond their normal schedule with direct patient care then that time would be paid. Documentation is not considered direct patient care.

25. Question: Is there on-call Answer: No

1. Question: Page 61 states ‘have a minimum of 3 years of experience in the field.” Can the VA expand upon what would be considered 3 years of experience? Would residency training meet this requirement?

Answer: No residency training does not meet the requirement. Locum candidate must have minimum 3 years of post-residency experience.

1. Question: Are past performance questionnaires required?

Answer: Past Performance Questionnaires are required for all of our requirements. We also may us CPARS, your submittal and we may call your references.

27. Question: Is it the responsibility of the vendor to ensure the questionnaires are completed and returned to Mr. Lapp?

Answer: No, you fill out the top portion of the PPQ and send to your evaluator. The instructions tell them to send their evaluation to Larry Lapp.

27. Question: What is the due date for past performance questionnaires?

Answer: Prior to your proposal due date to be considered.

27. Question: Will subcontractor past performance be considered equal to prime vendor past performance?

Answer: Yes.

1. Question: The Schedule B, CLIN 0002/1002 allocates $4,452.00 per week. I have a few questions concerning the calculation.

28. Is $4,452 based on $106 lodging per day x 6 FTE x 7 days per week?

28. If this is the calculation, per diem is not included. Is that correct?

Answer: Schedule B, CLIN 0002/1002 allocations are incorrect. New figures have been added to the attached Schedule.

For your convenience a revised copy of the solicitation follows with all changes noted in Green.
