See attached document for answers to solicitation questions.

Marchelle Peyton Contracting Officer Attachment 1- Solicitation 36C24618R0818 Questions and Answers Mobile PET/CT Scan Services for Hampton VAMC and Salem VAMC

1. Question: Do you want two review stations with one at each location or do you want two at each location?

Answer: Two at each location.

2. Question: Do you want two monitors for each station or one monitor for each station?

Answer: Two at each location.

3. Question: Would you consider 1 review station with appropriate software installed on facility desk tops? That option is much less expensive.

Answer: VA IT requires all software to be evaluated by them before they would install it on a VA Desktop or Laptop System. This would be required after award and would take some time to do. Therefore, the contracting officer has decided that it would be in the best interest of the Government to require the Contractor to supply both review stations with the appropriate software.

4. Question: Hampton VA patient numbers appear lower than anticipated. Does this location have an in-house PET/CT and the mobile is used to lower backlog?

Answer: No, Hampton VA does not have an in-house PET/CT. There number are the same as last year and they do not anticipate an increase or decrease, however we did experience upward and downward fluctuation in quantities from Facilities on that contract.

5. Question: I know this is a speculation question but the start date is Apr. 2019. Do you anticipate other locations being added to this contract before Apr? 2019?

Answer: No.

6. Question: Under the title “Instructions for proposal submission” is the following language:

Offerors shall provide one copy of their proposal via email to larry.lapp@va.gov (PDF or MS Word only) which includes separate volume sections addressing 1) Past Performance volume, 2) Technical volume and 3) Pricing volume. Each Volume shall be clearly marked with the Offerors Name, RFP Number, RFP Title, Officers DUNs Number and Socio-Economic Status.

Do you prefer one document with the volumes in 3 sections or three separate documents?

Answer: One submittal with 3 separate sections. One addressing Past Performance, one addressing Technical and one addressing Price.

7. Question: The volume of technical information required to satisfy the RFP is large (copies of all licenses and certifications for company and techs). If the VA email system is limited to a size of 5 MB or less, do you prefer all copies submitted with RFP (multiple follow up emails/attachments) or only upon award?

Answer: Great point, we have had this problem in the past. All required documents are required to be submitted with your proposal. Please forward your email proposal in a series of numbered emails if necessary.

8. Question: Under 2 – Factor 2 – Technical is this language:

The technical proposal shall be type printed on standard size white paper in 11 pitch New Times Roman font, on a single side. No notebooks and special binding methods are required. Clips or a single staple on the upper left corner is sufficient. Technical proposals are limited to no more than 50 pages and shall include the Offer Form, offerors experience, personnel, technical approach and product description as a minimum.

I assume the above language is from an old template and should be omitted, please confirm?

Answer: Ref: In the “Instructions for Proposal Submission”; Factor 2 – Technical, second paragraph is replaced by the below wording:

“The technical submittal shall be type printed on standard size white paper in 11 pitch New Times Roman font, on a single side. Technical submittals are limited to 50 pages and shall include the Offer Form, offerors experience, personnel, technical approach and product description as a minimum.”

9. Question: Are the telephone receptacles provided by each facility at the mobile pad waterproof outlets (Hubbell PH6495) mounted in weatherproof boxes (Hubbell Ph6619) and mounted within 50 feet of the mobile pad? If not, what type of telephone receptacles are provided at each facility?

Answer: Standard Telephone receptacles, mounted near mobile pad. Currently NON-Waterproof type.

10. Question: Are the data line receptacles provided by each facility at the mobile pad standard RJ45 connections with weatherproof covers and within 50 feet of the mobile pad? If not, what type of data receptacles are provided at each facility?

Answer: Standard RJ45 receptacles, mounted near mobile pad. Currently NON-Waterproof type.

11. Question: Is the electrical power provided by each facility 60Hz, configured in three-phase “wye” and within 50 feet of the mobile pad? If not, what type of electrical power is provided?

Answer: 60Hz, Three-phase in waterproof receptacle, near mobile pad.

12. Question: What are the anticipated work hours at each facility? The S.O.W. states 0800-1830 on page 17, while pages 29 and 78 state 0800-1630.

Answer: During the days the contractor is on station the hours of operation shall be 0800-1830.

13. Question: What is the required year of manufacture of the camera? The Pre-solicitation Notice states 2007 while page 11 of the S.O.W. states 2004.

Answer: Paragraph 3 of B.4 Performance Work Statement, first sentence is corrected as listed below.

“3. CONTRACTOR’S FURNISHED EQUIPMENT: Equipment shall include but not be limited to a Contractor furnished fully equipped PET/CT Mobile Trailer where studies will be performed on a PET/CT scanner manufactured after January 2007 and a PET/CT review station and fusion software to be placed within each medical center obtaining service under this contract.”

14. Question: How many workstations with software for the interpretation of PET/CT images are required to satisfy this bid? (The wording on page 13, item K is confusing. One at each facility for a total of two for the bid, or two at each facility for a total of four for the bid?)

Answer: Two at Hampton VAMC and two at Salem VAMC for a total of four.

15. Question: What is the anticipated award date?

Answer: Before 1 April 2019 with a performance period to start 1 April 2019.

16. Question: This is a 100% SDVOSB set-aside. Can the prime contract take on a sub-contract as long as at least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern.

Answer: Yes, however please refer to the below FAR Clause 52.219-14 “Limitations on Subcontracting (Jan 2017)” which was inadvertently omitted from the solicitation by reference under FAR Clause 52.212-5 and which is now included by reference under FAR Clause 52.212-5. This clause will provide proper guidance on the implementation of the 50 percent rule.

52.219-14 -- Limitations on Subcontracting (Jan 2017)

(a) This clause does not apply to the unrestricted portion of a partial set-aside.

(b) Applicability. This clause applies only to--

(1) Contracts that have been set aside or reserved for small business concerns or 8(a) participants;

(2) Part or parts of a multiple-award contract that have been set aside for small business concerns or 8(a) participants; and

(3) Orders set aside for small business or 8(a) participants under multiple-award contracts as described in 8.405-5 and 16.505(b)(2)(i)(F).

(c) By submission of an offer and execution of a contract, the Offeror/Contractor agrees that in performance of the contract in the case of a contract for --

(1) Services (except construction). At least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern.

(2) Supplies (other than procurement from a nonmanufacturer of such supplies). The concern shall perform work for at least 50 percent of the cost of manufacturing the supplies, not including the cost of materials.

(3) General construction. The concern will perform at least 15 percent of the cost of the contract, not including the cost of materials, with its own employees.

(4) Construction by special trade contractors. The concern will perform at least 25 percent of the cost of the contract, not including the cost of materials, with its own employees.
