SECTION E

INSTRUCTIONS TO OFFERORS:

Contractor shall provide a quote for product and service indicated in the Schedule, including all licensing and process requirements indicated in the solicitation and Statement of Work (SOW). Quotes shall be sent by the date indicated in the solicitation to the Contracting Officer (CO) at darla.hurlock@va.gov. FOB for the duration of performance under any award executed as a result of this solicitation shall be Destination.

Prospective offerors may, but are not required to, visit the bulk oxygen tank site at the Salisbury VAMC, and shall email the Contracting Officer at darla.hurlock@va.gov to request a site visit; no telephone requests will be honored. In the site visit request, include the company name and name of the individual(s) participating; each company is limited to two (2) participants. Site visit request emails shall be received by the Contracting Officer no later than Friday, July 20, 2018, 1:00p.m. EDT.

The site visit will be held at the Medical Center in Salisbury, NC on Monday, July 23, 2018 from 2:00 p.m. to 3:00 p.m.; participants are requested to arrive by 1:45 p.m. at Building 21, Room BC102 and await a Logistics Service staff member to escort the group. Expenses incurred by the vendor for the site visit will not be reimbursed by the Government.

Any questions regarding solicitation requirements or facility specifications shall be made only to the Contracting Officer at darla.hurlock@va.gov; questions shall be received no later than Wednesday, July 25, 2018 by 8:00 a.m. EDT. All questions shall be sent via email; no verbal or faxed questions will be addressed by the Contracting Officer. Responses to relevant questions will be answered by amendment to the solicitation.

All required documentation regarding certification and registrations required to perform these services shall be submitted with the quote.

All price quotes shall include a narrative Past Performance statement, which will document three (3) recent (within 3 years) and relevant (of similar size and scope) contract references for same/similar jobs including Point of Contact (name and telephone number). This capability statement of performance is limited to three pages with 11pt font.

Contractor shall accept both Government purchase cards and Government purchase orders.

BASIS FOR AWARD;

Offers will be evaluated on a Lowest Price Technically Acceptable basis. Award shall be made to the lowest priced, technically acceptable quote that meets the solicitation requirements based on the pass/fail evaluation by technical experts against the following Technical Evaluation Criteria; ensure sufficient narrative is provided with the quote to determine acceptability:

1) Whether the offer meets or exceeds all requirements of the Statement of Work and solicitation; and

2) Whether the vendor’s past performance submission indicates an acceptable level of performance has been demonstrated on recent and relevant work.

The evaluation process shall proceed as follows:

Initially, quotes shall be ranked according to price, including option prices if applicable. An offeror’s quoted price will be determined by multiplying the quantities identified in the pricing schedule by the proposed unit price for each contract line item to confirm the extended amount for each item. The extended amounts will be added together to determine the total evaluated price.

Next, the lowest priced offer shall be evaluated for technical acceptability. Technical acceptability shall be evaluated on a pass/fail basis considering technical evaluation criteria stated above.

If the lowest priced offer is determined technically acceptable, that offer represents the best value to the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers.

If the lowest priced offer is not determined technically acceptable, the next lowest priced offer will be evaluated for technical acceptability and the process will continue in order by price until an offer is judged technically acceptable.
