I. SCOPE: Contractor shall provide access for purchase, as needed, the Everything DISC workplace assessment tool, including all variations of the focus of the test and possible results reports on the Catalyst platform. The DISC assessment tool is manufactured by

Wiley and Sons Publishing.

II. DEFINITIONS/ACRONYMS:

• Department of Veterans Affairs (VA)

• Contracting Officer's Representative (COR): A representative from the requiring activity appointed in writing by the Contracting Officer (CO) to perform surveillance and to act as liaison to the contractor.

• Veterans Integrated Service Network 5 (VISN 5): Healthcare network consisting of five

VA hospitals within Maryland, West Virginia and the District of Columbia.

III. GENERAL REQUIREMENTS:

A. Introduction: The Department of Veterans Affairs and VISN 5 has a need to make available the DISC assessment tool to managers, leaders and their staff with the end goal to improve employee engagement. The tool is to only be used by individuals who are trained and permitted to provide and interpret the DISC assessment results.

B. Place of Performance: The vendor will provide individuals taking the DISC with an online link via a secure online platform. Results are shared to the test taker through the online platform (Catalyst) and via the DISC certified OD psychologist.

C. Background: VA is at a turning point and needs swift organization-wide change to reach the goals of providing high reliability, psychological safety, personalized, patient-driven, timely care to our Veterans. Through learning opportunities such as this, key leaders and staff within VISN 5 will develop a deeper understanding of each other, their mission and an appreciation of the diverse work styles that make up their team.

D. Performance Period: The performance period shall include a base and 4 option years.

Services shall consist of offering easy access to purchase the needed number of assessments as the need arises.

IV. SPECIFIC REQUIREMENTS/TASKS AND ASSOCIATED DELIVERABLES

The following is a list of the trainings, services and products which the VA can order under the sole source agreement:

Task 1: The contractor shall make available for purchase up to 110 Everything DISC

Workplace on Catalyst base experience (includes an assessment, access to the catalyst platform and a DISC profile report).

Task 2: The contractor shall make available for purchase up to 25 Everything DISC

Facilitator Reports.

Task 3: The contractor shall have available all of the subtests and reports available for purchase for the DISC, as published by the manufacturer Wiley and Sons Publishing.

All DISC assessments ordered shall be invoiced per person and include costs for any subtests, and reports, Contractor Qualifications

The vendor shall be an authorized provider of the Everything DISC assessment tool. The vendor shall be knowledgeable of the DISC in order to provider any needed guidance as to the best version of the DISC to use during an assessment request.

Task 1: The contractor shall be available to conduct a virtual orientation of the Catalyst platform with clear directions on how to purchase and or access the test material, the results and other reports that are available.

Task 3: The contractor shall assist the Program Manager and/or Facilitator with preparing the invitation for test taking participants and opening remarks, as well as guiding the

Program Manager and/or facilitator towards an implementation plan, as needed.

Task 4: The following is a general description of the training materials and support services that the contractor will be required to provide during the life of the resulting BPA:

a) The Contractor must:

• Make available for purchase, DISC assessment items for each VA trained instructor. The contractor will provide a mechanism for the VA DISC trained administrator to submit the names of the employees who are approved to take the test. The vendor will then provide individual links to each test taker who will be able to log in to the system and take the test. The system is otherwise known as Catalyst. All results will become property of the VA.

• Contractor must provide updated training material if there are changes to curriculum during the life of the contract.

• Make available, a support team that will work with the VA-employed instructors/trainers during the lifetime of the contract.

• The team leader/consultant shall be available by telephone or email to assist with any pre-course or post-course questions. Response time requirement is within 24 hours after initial notification during regular work week and within 72 hours during holidays and weekends.

b) The DISC assessment tool results shall:

1. Enable participants to understand their work engagement style using the results of the DISC assessment tool.

2. Help participants understand themselves and how their engagement style lines up with each of the Office of Personnel Management Executive Core Qualifications
