B.2 STATEMENT OF WORK

I. Executive Summary The US Department of Veterans Affairs (VA) is committed to eliminating homelessness amongst our Veteran population. This goal is supported by VA Central Office (VACO) and the White House. It represents one of the VA’s most significant efforts to achieve the President’s goal of ending homelessness among Veterans.

A significant aspect of this plan is the construct and design of a continuum of care approach to alleviate homelessness among Veterans- an approach that fosters partnership between community providers and the Veterans Affairs Medical Center as well as community base and comprehensive response to meeting the different and frequently complex needs of homeless Veterans. As a result, the need for a Safe Haven has become an integral part of the continuum.

The Veterans Affairs Medical Center seeks to partner with community providers to establish a Safe Haven: a type of supportive housing that serves hard to reach homeless Veterans with severe mental illness and/or co-occurring disorder who are on the street and have been unwilling to participate in supportive services.

II. Acronyms and Abbreviations

1. VA- Veterans Affairs

2. HCHV-Health Care for Homeless Veterans

3. VHA- Veteran Health Administration

4. HUD definition of chronically homeless: Person with a disability who has been homeless for one full year or four episodes of homelessness in the past three years.

III. Background The VA has implemented several different types of housing and services geared at ending homelessness among Veterans. However, despite the Department of Veterans Affairs best efforts and intentions, analysis have shown that the special needs of some Veteran homeless subpopulation are often unmet or only partially addressed. This frequently applies to homeless Veterans with severe and persistent mental illness or co-occurring substance abuse disorder. Often the most visible, yet hardest to reach homeless Veteran population. The opportunities for these individuals to move beyond homelessness are reduced due to the nature of their disability which disrupts their judgment, motivation and social skills. Because of their complex needs and frequent resistance to both mental health and or homeless services a comprehensive and flexible array of services and supports need to be readily available to them. Having a Safe Haven as an option will provide a safe non-threatening place for Veterans to become stable.

IV. Purpose The Purpose of the Safe Haven is to provide low barrier, non-threatening, 24/7 hours transitional housing for Veterans with severe Mental Health disorder or Co-occurring disorder. This facility will serve the hard to reach homeless Veterans with severe mental illnesses who are on the streets and have been unable or unwilling to participate in supportive services.

For some persons with mental illnesses who have been living on the streets, the transition to permanent housing is best made in stages, starting with a small, highly supportive environment where an individual can feel at ease, out of danger, and subject to no immediate service demands. Safe Havens do not require participation in services and referrals as a condition of occupancy. Rather, it is hoped that after a period of stabilization in a Safe Haven, residents will be more willing to participate in services and referrals and will eventually be ready to move to more traditional forms of housing. Safe Havens can serve as an entry point to the service system and provide access to basic services such as food, clothing, bathing facilities, Telephones, storage space and a mailing address.

This facility shall provide:

1. Three meals per/day as well as laundry accommodation

2. Medication monitoring by trained staff.

3. Intensive case management which includes assessment of treatment and housing goals at intake and weekly follow up by a Licensed Clinical Social Worker.

4. Provides 24-hour staffed residence for up to 6 months.

5. Provide private or semi-private accommodations

6. Activity center for residents and or structured day program

7. Outreach Safe Havens target the most difficult to serve chronically homeless population. Individuals who are chronically homeless tend to have mental illnesses or co-occurring disorders that make them inherently distrustful of people and programs.

8. Intake/Assessment Certain information must be gathered about program participants by a Licensed Clinical Social Worker to comply with VA reporting and data collection system requirements (including HMIS), and to assess client needs. However, clients may have a number of problems communicating important information to program staff, due to lack of knowledge, stigmatization, or mental health issues.

9. Engagement The philosophy behind the Safe Haven model dictates that housing and services be delivered in a low demand, coercion-free environment. Yet supportive services are crucial to achieving long-term housing and occupational stability for the target Veterans.

VI. Scope of Work The contractor shall provide 24-hour staffed low demand, transitional residence in Greater Metropolitan area of Baltimore City. The facility must provide up to five (5) beds clustered exclusively for Male Veterans; The Program must provide onsite laundry, three meals/day, medication monitoring, computer access for Veterans, and access to daytime activities. VA may complement the non-VA community-based provider’s program with added treatment services such as participation in VA outpatient programs.

1. The contractor shall target chronically homeless (as defined by HUD) with serious mental illness (as defined by VHA) and substance use problems.

2. The contractor shall allow VA staff to inspect the facility and/or review Veteran participant treatment protocols at any time determined necessary by VA.

3. The contractor shall offer a low-demand environment with a minimum set of rules designed to re-establish trust in the chronically homeless Veteran and engage the Veteran in needed treatment services.

a. Acceptance of treatment cannot be a condition of admission or continued stay.

b. Abstinence from alcohol or drugs cannot be a condition of treatment or continued stay.

c. Infractions of rules should be used as opportunities for engagement.

6. The provider must have the ability to accommodate immediate admissions 24 hours per day, seven (7) days per week. All admissions must have concurrence of VA. If a Veteran presents at the facility during weekends or at night, the Safe Haven provider may accept the Veteran, however, VA staff have right of approval/disapproval for payment.

7. The contractor and VA liaison shall develop a highly individualized treatment plan with each Veteran to increase housing stability, income and benefits, improvement in living situation, and follow-up treatment for identified problem areas (alcohol, drug, mental health, medical, social, vocational, family).

8. The contractor shall encourage Veterans to work towards transitional or permanent housing within six (6) months of admission. All extensions beyond six (6) months will require approval of the VA.

9. The contractor shall ensure that the facility meets all applicable life/safety codes and other standards/compliance requirements as determined by VA, including Americans with Disabilities Act accessibility.

10. The contractor selected for this specialized contract is preferred at least two (2) years of direct experience as a provider of a Safe Haven Program or a program demonstrating the low-demand model.

11. The contractor shall provide newly admitted Veterans with all necessary furnishings and clean linens. Basic toiletries will be provided for all Veteran residents.

12. The contractor shall ensure that all meals provided are nutritious and that food items will remain available throughout the day. Three (3) meals per day need to be provided.

13. The contractor shall ensure that the facilities are well maintained and clean.

14. The contractor shall participate in annual homeless events sponsored by the VA Maryland Health Care System, i.e., local Stand Down events.

15. The contractor shall be knowledgeable of local VA and non-VA Homeless Programs and maintain a willingness to collaborate with other Providers as it benefits the Veterans.

16. The contractor shall have a site currently available to implement the Safe Haven Model.

17. The Contractor shall provide a locked and secured area for medications. This location shall only be accessible by the contractor staff. It is recognized that Veterans will take their own medications, but they shall be supervised and documented by trained contractor staff while obtaining and consuming their daily medications.

18. The Contractor shall comply with the VA Veteran's Bill of Rights as set forth in section 17.34a, title 38, Code of Federal Regulations.

19. The Contractor shall assist in preparing reports, maintaining records and databases to track referrals, acceptances, denials, discharges, and follow-up post discharge.

20. The Contractor shall provide monthly invoices in a timely manner, no later than the fifth day of the month, in order to receive timely compensation for the services identified in the contract.

21. The Contractor shall employ trained professional staff to carry out the requirements of the contract. There shall be an identified professional specifically assigned to the contract who shall be the point of contact for the HCHV staff. This shall include but not limited to; Licensed Clinical Social Worker and trained Case Aide , e.g peer support specialist, case manager assistant, social services aide, behavioral health aide.

22. The Contractor, namely the identified key personnel of Licensed Clinical Social Worker, shall provide documentation to the VA HCHV staff of services provided to each Veteran on a monthly basis, outlining progress/barriers toward individualized treatment goals provided no later than the tenth day of each month.

23. The Contractor, namely the identified key personnel of Licensed Clinical Social Worker, shall provide the VA HCHV staff an individualized services plan with-in seven days of admission and discharge summary with-in three days of discharge.

24. The Contractor’s per diem rate established by this contract shall include the services listed in this document and also include all services and supplies normally provided other clients by the agency without extra charge.

25. The Contractor shall make available documentary information required for the VA to conduct utilization review audits for the mandated national evaluation study as required by section 2 of Public Law 100-6; to verify quality of care for Veterans, to assure confidentiality of Veteran record information, and to determine the completeness and accuracy of financial records.

26. The Contractor shall collaborate with the Veteran and HCHV staff to conduct treatment and discharge planning reflecting a team assessment of health, social and vocational needs and the involvement of Veteran's family and appropriate community resources in resolving problems and setting goals.

27. The Contractor shall provide transportation to and from the VA Maryland Health Care Services facilities for scheduled appointments or services as well as search for transitional or permanent housing and address other care needs. It is recommended that the Contractor facilities be provided no more than 50 miles from the Baltimore VAMC.
