Questions for 36C24519Q0342:
| 1. | In reference to the excel file |
| a. | What do the 1120-line items represent? (Inventory list of 1 facility; Inventory of all facilities; estimated workload per vendor visit; etc.) The items in the spreadsheet are a list of items that are currently used at each site. |
| b. | Since there is no price/cost schedule in the solicitation document, what tabs on the excel sheet must be filled out as part of the vendor response? (The second tab labeled "Suggested Repairs CLINS FY1" is blank) A new spreadsheet is being provided with this amendment; the suggested repair clins tabs for each year should be completed. Contractors are highly encouraged to add additional repairs which might be needed for each type of instrument. |
| c. | If a vendor is unable to provide a price for one of the 1120-line items, will their bid be considered nonresponsive? Yes, the quote will be technically unacceptable if a contractor is unable to provide all of the required services through either self-performance or through a sub -contractor. |
| d. | If a vendor offers the instrument tray service at a flat rate per day that is cheaper than the itemized list, how do we provide that response? Please complete the attach spreadsheet. However, the contractor can provide another alternative pricing structure that incorporate servicing all listed items. Instrument Repair can be provided as a flat fee for instrument sharpening and repair, also case cart repair as well. |
| 2. | Page 7, bullet (3c) references repair and maintenance for case carts. There is no line item for case carts on the excel sheet. How should a vendor provide their pricing for this service? If a vendor provides this service as a flat rate per day up to (x) amount of carts, how do we provide that response? |
See answer to letter D.
| 3. | Page 8, bullet (3h) references that the vendor will be responsible for etching and entering data into Censitrac: a quote from Censitrac for 100% etching when we get the system in for use. Mobile will etch if needed as part of the Instrument Repair Contract. |
| a. | What instruments will need to be etched? (Complete inventory from each facility or just new additions/replacements in the trays serviced during the visit?) Will only need replacement instruments etched. |
| b. | Who will be responsible for purchasing/providing the masking/QR codes for etching? (Censitrac may not sell the masking/QR codes to parties other than the healthcare facility) It will be the vendor responsibility. However if it something that the healthcare facility can only get, the VA can make the request and the cost can be deducted from the invoice. |
| c. | Will a separate line item be created to account for the cost of etching an instrument/device? The spreadsheet has been updated to add etching. |
| 4. | Page 9, bullet (3v) references that the vendor must provide to each facility an Insulscan Laparoscopic Insulation Continuity Tester. Does it have to be that exact model or can a substitute be used that has matching specifications and capability? We asking for exact model and some places have their own Insulscan equipment. |

2) The excel price file. Is pricing based on each visit? the spreadsheet is based on each instrument that could be possible touched. Will the price file be used to determine the price per visit for all facilities? The purpose of this spreadsheet is to have preprice services for instrument that could be touch during a visit. Actual instruments service will vary from site to site and visit to visit. What is the estimate for the number of visits total? Varys based on station needs.
