CONTINUATION PAGE
Are the travel funds for the first year (and three option years) included within the $4.5M ceiling for the IDIQ? Yes Where are the funds for the maintenance, cleaning, storage and shipping of equipment? Cleaning of the simulation equipment entails the Contractor wiping task trainers with government-supplied wipes. Equipment maintenance, storage and shipping costs are covered outside of the contract.
Where are the funds for training the alternate instructors that are required and necessary to ensure single-point-of-failure issues? It is included in the overall cost of the contract. Train-the-trainer workshop is offered twice annually with maximum participants listed in PWS to allow for a pool of each: contract instructors, equipment support individuals and GTAs.
How the contractor to validate that is the Equipment Support Individual can drive a conversion van or lift 100 pounds? Your agency would need to determine what information is needed to meet these standards. However the minimum needed would be:
· Valid driver’s license and form stating they can drive a conversion van
· A signed form for the staff member stating he/she is able to lift 100 pounds Is the GTA positions part of another contract? The PWS says: A Gynecologic Teaching Associate (GTA) is also required for this training and will be procured through a separate contract. See the portion of the PWS that addresses the GTA requirement in section B.2 Page 17 of the solicitation attachment. Physician Instructors, Nurse Instructors, Equipment Support Individual and GTA are included in this requirement.
Is it the government’s intent to have the contractor for this Women’s Healthcare Services Clinical Skills Training procurement continually monitor the education, training, skills and performance of the GTA positions? Yes Is the PM responsible for this substantial monitoring of GTAs, and where are those funds/hours within the Price/Cost Schedule beginning on page 26? The contractor will determine who monitors the GTA; these funds are part of the overall cost of the contract.
Is the lead Instructor responsible for this substantial monitoring of GTAs, and where are those funds/hours within the Price/Cost Schedule beginning on page 26? The contractor will determine who monitors the GTA, these funds are part of the overall cost of the contract.
How many past performance examples are required for this proposal? As many as the contractor feels addresses the evaluation factor in section E.12 in relation to recency and relevancy.
Is the 50-page proposal limit (noted on Page 54) applicable to a specific volume, or all volumes combined? Does this limit include resumes? The 50 Page limit applies to everything but the resumes. Provide resumes to personnel in a 4th volume.
Section E.2 states that Volume 3 should include the “completed attachments in section D.” Can you please clarify which portions of Section D, “attached document: QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)” should be included in Volume 3? All attachments in section D need acknowledged at submittal of quote.
On page 54, are the Instructions for Volume 3 intended to reference Factor 2, such that Factor 1 (referenced in Volume 2 instructions) refers to personnel experience and Factor 2 refers to company experience?
Submission Documentation When submitting proposal, provide in separate volumes.
Volume 1: RFQ Fill-In Requirements – Included with this Volume will be the Price Schedule Volume 2: Capability Statement outlining the companies plan for providing services per the PWS. See Technical Factors 1 in section E.12 Volume 3: Past and/or present company contract experience (with-in 5 years), and completed attachments in section D. See Past Performance Factor 3 in section E12.
Volume 4: Employee’s Resumes and References For purposes of evaluations, vendors submitting quotes for this requirement shall limit proposals to 50 pages for volumes 1-3.
