B.2 STATEMENT OF WORK
Department of Veteran Affairs VA Medical Center
Statement of Work Sterile Processing Service & Dental Services
Water Testing Requirement
1.0 Introduction
This scope of work is to request a non-personal services contract to provide water testing in the Sterile Processing Services areas (SPS) and Dental Services at the James E. Van Zandt VA Medical Center to be compliant with American National Standards Institute (ANSI) and Association for the Advancement of Medical Instrumentation (AAMI), ST108 Standards. The tests will be performed in accordance with these standards.
The Contractor shall provide all personnel, equipment, materials, facilities, transportation, tools, and supervision to perform these services for the "SPS Water and Dental Testing” Service Contract.
2.0 Objective
The work performed under this contract will provide the James E. Van Zandt VA Medical Center test results to verify the critical water used for SPS and Dental is meeting the standards. The water testing should be done in accordance with the following:
1. Association for the Advancement of Medical Instrumentation (AAMI ST108).
2. Heterotrophic Plate Count 9215
3. Memorandum 2020-08-28 Required critical water systems for testing and report for Sterile Processing Services (SPS)
4. American Dental Association (ADA)
5. American National Standard Institute ANSI/AAMI ST108:2023
6. Clinical & Laboratory Standards Institute (CLSI) guidelines
7. Centers for Disease Control
8. Environmental Protection Agency (EPA) regulatory standards or drinking water The water quality needed for the various stages of medical device processing is determined by the type of medical device and by the disinfection or sterilization process used. Water quality measurement values per ANSI/AAMI ST108 are outlined in Schedule I. The contractor shall provide testing for all the locations and frequencies listed in Schedule II.
Utility Water: Water as it comes from the tap. This water is mainly used for flushing, washing, and intermediate rinsing (e.g., rinsing between cleaning and disinfection).
Critical Water: The water generally requires extensive treatment by a multi-step process that can include a pretreatment, does include primary treatment (e.g., RO and/or DI), storage, distribution, and can include final treatment to provide a level of assurance that microorganisms and inorganic and organic material are removed from the water.
Dental Water: The water is used in dental delivery systems for patient care procedures. This water requires treatment and monitoring to ensure compliance with microbial and water quality standards, minimizing the risks of contamination and biofilm development in dental unit waterlines.
2.1 Six general characteristics need to be considered:
1. Physical appearance of the water (color, clarity and absence of particulates/sediment)
2. The microbial level in water
3. Inorganic and organic contaminants of water
4. The pH of the water
5. Conductivity of the water
6. Water temperature
Water used for medical device processing must be within the appropriate standards for utility and critical water as listed in AAMI/ANSI ST108 to the point-of-use.
3.0 Scope
3.1 Testing Requirements:
The contractor shall provide AAMI/ANSI ST108 compliant water testing services. The requirements listed below include some of the critical ST108 requirements, however it is the contractor’s responsibility to identify and adhere to all ST108 standards.
3.2 Sampling Protocol
1. Vendor performing sampling shall use Personal Protective equipment.
2. Sample taps should be opened, and the water allowed to run for at least 60 seconds before a sample is aseptically collected in a sterile, endotoxin-free container.
3. The collection container should not be made of polypropylene. The container should be of the required quality not to introduce contamination into the test sample.
4. Sample taps should be treated with a general disinfectant (e.g., alcohol) prior to sampling and be completely dried to prevent contamination of the collected sample disinfectant residual from the sample tap. Bleach or other disinfectant solutions should never be used.
5. Hold the sample on ice, or at 2 and 6 °C, until it is delivered to the laboratory for testing.
6. Sampling containers: The appropriate sampling container should be used to collect the water sample.
7. Prior to sampling, the sampling surfaces (e.g., valves, hoses) should be disinfected both externally and internally to reduce microbial contamination.
Aseptic technique should be used for water collection.
8. Vendor shall bring all required supplies to perform services for proper sample collection up to 300ml per sampling. Vendor will bring a calibrated pH meter as pH must be tested immediately (within 15 minutes) at the sample draw. Vendor will also be required to bring a cooler with ice packs to keep the bacterial samples chilled as required during transport. Samples shall be maintained by vendor in a cooler to maintain temperatures between 2 and 6 degrees Celsius.
Vendor shall include maintained temperatures in the sample collection result report.
9. Recommended maximum elapsed time between collection and analysis is 8 hours (6-hour maximum transit time and 2-hour maximum processing time
3.3 On Demand Retesting
1. Retesting is required:
a. After out-of-range values as defined in Attachment 1. The contractor must be available to draw samples and begin retesting within 48 hours of notification or on the next testing day.
b. If the retesting is on one of the Medivators, the samples will then be drawn from 2 bowls inside the machine.
c. Sterile Processing Services staff shall cycle Medivators per IFU for collection and collect samples from within bowls. Retesting will not be conducted on Saturdays, Sundays, or Holidays
d. After system modifications
e. After installation of new equipment
2. Retesting must be provided upon facility request. The following retesting shall be included:
a. 8 endotoxin tests (4 visits)
b. 20 HPC tests (8 visits)
c. Ionic testing to include alkalinity, hardness, total organic carbon, conductivity, pH (2 visits)
3.4 Chain of Custody (COC) Requirements
1. The form will be an electronic document and shall have actual wet signatures or digital if electronic for transfer.
2. The form shall have the date and time of all transfers.
3. The form shall include temperature of the samples requiring temperature control at the point of sampling and each transfer.
4. Report forms shall have column titles at the top of each column that identify standards for low to high limits.
5. All samples from the sample collection to plating must follow all standards in this performance work statement.
6. Send the electronic report to the VA point of contact via email.
3.5 Reporting Requirements
1. At the time of sampling, the contractor shall provide the following:
a. Name and signature of person collecting the sample
