About this file
This is a project example template (Exhibit A) that offerors must complete to demonstrate their relevant experience for the flagpole replacement project at VA Lebanon Medical Center. The template requires detailed information about 1-2 similar projects completed within the last 5 years, including contract values, completion dates, scope descriptions, and points of contact. Offerors must provide specific details about work performed by their firm and any subcontractors, particularly noting if the same subcontractors will be used for the current solicitation.
The template relates to a pre-solicitation notice for a design-build contract to replace a flagpole at Building 1 of the Lebanon VA Medical Center. This will be a two-phase best value procurement set aside 100% for SDVOSBs under NAICS 236220 with a $45M size standard. The project has an estimated value of $500,000-$1,000,000 with a 220-day period of performance. The solicitation is expected to be released around December 30, 2024. Contractors must partner with an architecture/engineering firm and be registered in SAM and verified in VetCert at time of Phase I submittal.
