Statement of Work for Carestream CS 9600 at VA Pittsburgh Healthcare System-University Drive University Drive C Pittsburgh, PA 15240-1003
1. Background
1.1. The VA Pittsburgh Healthcare System (VAPHS) is a tertiary referral center for the western half of VA Healthcare—VISN 4, treating Veterans from Altoona, Erie, and Butler, in Pennsylvania and Clarksburg, West Virginia. VAPHS proudly serves the Veteran population throughout the tri-state area of Pennsylvania, Ohio, and West Virginia. VAPHS consists of two clinical care facilities in Pittsburgh (HJH and University Drive locations) as well as five community-based outpatient clinics. VAPHS maintains academic affiliations with over 170 associated health educational programs, to include the University of Pittsburgh Schools of Medicine and Dental Medicine.
1.2. VA Pittsburgh Healthcare System requests the emergency replacement of existing Planmeca Inc. dental imaging equipment at both the University Drive campus and the HJH campus, locations of the VA Pittsburgh Healthcare System. The current technology/medical equipment has an acquisition date of 01/19/2011, is failing, and needs replacement to avoid any unnecessary delays or cancellations in Veterans needing dental services. This imaging equipment provides Dental Services, professionals, and specialists with the needed precision to capture and store dental images and plan for treatment options for Veteran’s dental needs. Any delay in the ordering process will disrupt patient care if an award is not made in a timely manner.
1.3. Prior to submitting any proposals, each vendor must complete a physical, on-site walkthrough of the dental suite both at HJ Heinz and University Drive. Each vendor must comply with local policy LD-073, which requires registration in VA Pittsburgh’s vendor credentialing system. Within this system, each vendor must supply proof of: flu vaccination, Tb test, Covid-19 vaccine, and HIPPA certification at a minimum. The purpose of this walkthrough is to provide bids for a turnkey solution. Each proposal should include purchase of the modality itself, but also all ancillary materials like cables for cable pulls, mounting hardware, switches in data closets, labor, etc. which would be included in a turkey installation. The cable must be cat6 from end point to end point.VA Pittsburgh Healthcare System (VAPHS) Dental Dept. requires the replacement of existing Planmeca Inc. dental imaging equipment at both the University Drive campus and the HJ Heinz campus, locations of the VA Pittsburgh Healthcare System.
