STATEMENT OF WORK (SOW)
1. General A service contract is required for the certification of chemical fume hoods, laminar flow hoods, and biosafety cabinets that are used in the Research and Development Department (R&D) and Laboratory Services at the University Drive and H. J. Heinz campuses. The contract will be blanket purchase agreement (BPA) with fixed price task orders to provide the services as required. The BPA will establish unit pricing for a five-year period. Task orders will be created by the duly appointed warranted officer as necessary to obligate funding and schedule the certification services.
1. Background and Tasks VAPHS uses equipment that must be checked regularly to ensure that it is functioning properly including chemical fume hoods, laminar flow hoods, and biosafety cabinets and to meet OSHA, AIHA/ANSI, SEFA, ASHRAE and VA Safety standards. The certification will ensure that all of the parts of the equipment are working as designed and intended.
Laminar flow hoods create particle-free working environments by projecting air through a filtration system and exhausting it across a work surface in a laminar or directional air stream. They provide a clean air environment for a number of laboratory requirements. Testing and certification of laminar flow clean benches. Testing to be performed per the manufacturer’s specifications. Tests to include the following:
· HEPA filter leak test.
· Air velocity profile test.
· Non-Viable Particle Counts
· Smoke pattern testing
· Electrical leakage test.
· Ground circuit continuity test Biosafety cabinets are designed to provide both a clean work environment and protection for employees who work with biological hazards. Testing and certification of Class II Biological Safety Cabinets. Testing to be performed per cabinet manufacturer’s field-testing specifications and or NSF Std. 49. Tests to include the following:
· HEPA filter leak test of supply and exhaust filters.
· Access opening velocity test.
· Downflow velocity test.
· Smoke pattern testing
· Electrical leakage test.
· Ground circuit continuity test.
A chemical fume hood is a ventilation device that is designed to limit exposure to hazardous or noxious fumes, vapors or dust. As per ASHRAE 110 and the manufacturer’s instructions, chemical fume hoods must be certified to guarantee that they are working properly. Test to include the following:
· Airflow direction verification
· Average inward air velocity measurements
· Smoke pattern visualization
· Sash movement containment
Contractor must provide dated certification sticker for each hood or cabinet displaying either the date of the successful testing or date the certification expires.
Customer Notification
Contractor shall notify the Research, or Safety contact and the Police service when they arrive on site.
Failure to comply, either in whole or in part, with either the notification requirements will be deemed significant non-compliance with the contract and may be justification for termination of the contract.

When the service(s) is/are completed, the Contractor shall document the services rendered and provide to the VA contact. The summary report shall include which units were serviced and identify any that failed along with the reason.
Contractor shall provide certification reports for all serviced equipment. The Contractor’s report template shall be provided to the COR for acceptance. Reports must include, but are not limited to:
1. Technician Name
1. Date of Service
1. Make

1. Performance Monitoring
Acceptance and monitoring of deliverables will be by the Biosafety Officer for R&D and by the Industrial Hygienist for Safety. Acceptance will be measured by on-time delivery of services. The Biosafety Officer and Industrial Hygienist will arrange the site visit and accompany the contractor to the prospective areas where all of the equipment is located and observe that services outlined in the contract are completed in each department.
Invoices must contain the date of service, invoice number, PO number, amount due, and a description of the service and/or a description of the product, and the amount being charged for the service/product.

1. Other Pertinent Information or Special Considerations.
It is imperative for the vendor to have knowledge and prior experience in certifying chemical fume hoods, laminar flow hoods and biosafety cabinets. Knowledge of research laboratories and the equipment used in research laboratories is preferred.
1. Inspection and Acceptance Criteria.
Acceptance and monitoring of deliverables will be by the Biosafety Officer for R&D or by the Industrial Hygienist for Safety. Acceptance will be measured by on-time delivery of services. The Contractor will coordinate visits with the Biosafety Officer or Industrial Hygienist who will arrange the site visit and accompany the contractor to the areas where all of the equipment is located and observe that services outlined in the contract are completed.

VAPHS anticipates that this need for a service agreement for the biosafety cabinets, laminar flow hoods and chemical fume hoods will be ongoing. The contract shall be a 5-year BPA.
1. Workload
While the majority of the workload will fall within the scheduled month-long periods, the Contractor shall be available for single unit unscheduled certifications. Unit pricing will apply to these visits without any additional reimbursements.
