About this file
This justification and approval document authorizes a sole-source task order against an existing IDIQ contract to provide multi-specialty nursing and medical staffing services at the VA Pittsburgh Healthcare System. The one-year firm fixed price task order, valued at approximately $746,000, will be issued to Sanford Federal Inc for certified nursing assistant, constant observation technician, and other medical staffing positions. The justification cites FAR 16.505(b)(2)(i)(C) regarding logical follow-on orders, indicating Sanford Federal received the original order under IDIQ 36C24421D0083 and its staff are familiar with facility processes and onboarding requirements. Issuing the new task order is deemed in the interest of economy and efficiency to maintain continuity of existing staff rather than incur onboarding and credentialing costs associated with new service providers.

Exception to Fair Opportunity Memo
Acquisition Plan Action ID: 36C244-24-AP-0149
Contracting Activity: Department of Veterans Affairs, VISN 04, VA Pittsburgh Healthcare System (VAPHS), Network Contracting Office 4 (NCO4), 2237 #646-24-1-716-0002.
Description of Action:
The proposed task order to Sanford Federal Inc against IDIQ 36C24421D0083 is for a one-year firm fixed price (FFP) effort for multi-specialty nursing and medical staffing for VA Pittsburgh
Healthcare System.
Description of Supplies or Services:
The proposed action is to provide multi-specialty nursing and medical staffing support for VA Pittsburgh Healthcare System. Specifically, the task order shall provide staffing for the following positions: Certified Nursing Assistant and Constant Observation Technician (Patient Sitter). The total estimated value of the proposed order is
Statutory Authority: The statutory authority permitting an exception to fair opportunity is Section 41 U.S.C. 4106(c) as implemented by the Federal Acquisition Regulation (FAR) 16.505(b)(2)(i):
FAR 16.505(b)(2)(i)(C): The order must be issued on a sole-source basis in the interest of economy and efficiency because it is a logical follow-on to an order already issued under the contract, provided that all awardees were given a fair opportunity to be considered for the original order.
Rationale Supporting Use of Authority Cited Above:
Sanford Federal Inc received an original order for these services in 2022 against its IDIQ resulting from competitive RFP 36C24420R0048. The contractor has staff in place that are familiar with facility processes and procedures and have completed facility on-boarding and credentialing and privileging as needed. It is in the interest of economy and efficiency to maintain existing staff through the several providers fulfilling orders resulting from that RFP. Turnover of existing staff would incur risk of service disruption associated with onboarding and credentialing and privileging and entail a learning curve for new service providers.
Efforts to Obtain Competition:
The Government awarded multiple IDIQ contracts resulting from competitive RFP 36C24420R0048 to contractors determined technically capable of meeting the requirement for multi-specialty nursing
