About this file

This document contains two sections related to a federal contract opportunity for mobile lithotripsy services. The first section is a conflict of interest certification statement to be completed by offerors indicating whether any conflicts exist relating to the key personnel proposed. It requires identification of any past, present or planned interests that could pose a conflict. The second section outlines certification requirements for the contractor regarding compliance with immigration and nationality laws as they relate to non-immigrant foreign national employees working on the contract.

The associated federal contract opportunity notice seeks quotes for extracorporeal shock wave lithotripsy services to be provided at the Lebanon VA Medical Center in Pennsylvania. Interested contractors are asked to submit company information by April 3rd to assist with market research and planning for the forthcoming request for quote. The work includes medical services under NAICS code 621512 with a size standard of $16.5 million. Questions should be directed to the specified contract specialist by email.
