DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C244-19-AP-0494

1. Contracting Activity:

Department of Veterans Affairs, VISN 4, Lebanon VA Medical Center

Behavioral Health Service

1700 South Lincoln Avenue

Lebanon, PA 17042

Purchase Request ID: 595-19-1-096-0016

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Lebanon VAMC has a BRAND NAME requirement for 1,100 NeuroStar Treatment Sessions (CSM 500). The Treatment Sessions are used with the NeuroStar Advanced TMS System to provide treatment to the medical center’s Behavioral Health patients. The estimated cost of the required treatment sessions is $107,800.00.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The Lebanon VAMC owns the NeuroStar Advanced TMS System. Neuronetics is the sole manufacturer of NeuroStar. To maintain continuity in patient care and avoid voiding the equipment warranties and maintenance services the Lebanon VAMC is required to procure NeuroStar brand name treatment sessions.

4. Description of market research conducted and results or statement why it was not conducted:

Market research activities consisted of contacting the manufacturer for information on authorized distributors, reviewing FPDS records for historical information on awards for the same items, and searches of the Vendor Information Page (VIP) network for Service Disabled Veteran Owned Small Businesses (SDVOSB) and Veteran Owned Small Businesses (VOSB). Market research indicates that the manufacturer, a small business, and an SDVOSB authorized distributor are the only two vendors able to provide the required treatment sessions on the open market.
