About this file

This document is a notice of intent to sole source a contract from the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2. The VA intends to award a non-competitive contract to O.R. Elder Inc. to procure a Centurion Vision System with Active Sentry Enabled for the Buffalo VA Medical Center in Buffalo, New York. The justification for the sole source award is that O.R. Elder Inc. is the only responsible source that can meet the requirement. The NAICS code for this procurement is 339115 for Ophthalmic Goods Manufacturing and the Product Service Code is 6540 for Ophthalmic Instruments, Equipment, and Supplies. The point of contact for any questions is Jonathan Kilgore, Contract Specialist, at Jonathan.kilgore@va.gov.

VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT

Last Updated: 02/01/22 Page 1 of 2

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 34C242-24-AP-1319

1. Contracting Activity: Department of Veterans Affairs, VISN 02, Buffalo Medical Center. Purchase

2237: 528-24-2-4085-0034

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The

Ophthalmology Surgical Service at the Buffalo VA Medica Center has a requirement for one (1) Cataract Extraction Unit to be used during cataract surgery. The unit is designed to provide the surgeon with lower, more physiological intraocular pressure (IOP) and enhanced chamber stability even in complex patients.

Estimated dollar value is $75,766.80.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The customer currently uses a Centurion Vision System that is nearing its end of life and needs to be replaced. The customer requires a unit and associated handpieces that have certain features and capabilities, including:

• Handpiece: Active Sentry signals to the Centurion hardware and software that adjustments are needed to maintain consistent IOP.

• Sensor: Pressure sensor detects changes in anterior chamber stability as it occurs.

• Fluid Technology: Active Fluidics uses compression plates to adjust pressure on BSS irrigating solution bag, compensating for changes in the eye.

• Valve Technology: Releases fluid into aspiration line and minimizes occlusion break.

Market research determined that Alcon (the OEM) offers a newer version of the Centurion Vision System currently being used by the customer that has the previously mentioned required features and capabilities. The Ophthalmology Surgical staff are already trained and experienced with using their current Centurion Vision System. Purchasing a newer version of the same equipment will result in minimal additional training being required and will allow the equipment to be used by the entire facility, creating efficiency for surgical team workflow.

4. Description of market research conducted and results or statement why it was not conducted:

Market research was conducted in February of 2024. Techniques used include: searching VA’s VetCert, reviewing the Supply Chain Master Catalog (SCMC) to determine availability of requested equipment, and reviewing the OEM website and product literature.

The Ophthalmology Surgical team is needs to replace a Centurion Vision System cataract extraction unit manufactured by Alcon Vision that’s nearing end of life. The replacement unit is required to https://www.acquisition.gov/?q=/browse/far/13

Last Updated: 02/01/22 Page 2 of 2 have certain features and capabilities that will make it a suitable replacement. Market search showed that Alcon Vision sells a newer version of the Centurion Vision System owned by the customer and that it offers all of the capabilities needed by the customer. The Centurion Vision System is the only cataract extraction unit that meets the customer’s requirement. The OEM Alcon Vision provided a letter certifying that O.R. Elder Inc. is the designated Department of Veterans Affairs SDVOSB distributor for Alcon Vision surgical products.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
