CONTINUATION PAGE

1. Total Estimated Patients enrolled/assigned to site: 1865 as of January 2021.” Please provide the most recent 3-month billing figures/members/enrollees for primary care services

ANSWER: April: 1892, May: 1888, June: 1886

2. Does the PACT team total include Women’s Health exam rooms? If so, does each PACT require WH exam room?

ANSWER: Refer to 4.9.1.1.2. Women’s health exam room is not included in the PACT team total. Each PACT Team does not require a Women’s Health Exam room.

3. Question: Would the government be willing to accept email submissions of proposal rather than requiring printed copies?

ANSWER: Not at this time. Printed copies are required in the numbers listed in the RFP.

4. In section B.2 Schedule of Services on page 9 of the Solicitation states in Line Item Number 0001 the base year estimated enrollment is 1935. As the incumbent contractor, the latest billed enrollment quantity for Tompkins CBOC was 1888 which is 47 enrollees lower than the estimated enrollment stated. Secondly this shows a slight increase through Option Year 3 which in fact, the enrollment has been steadily decreasing 12% over the last 6 years.

Question: Would the VA consider reducing the estimated enrollment in the base and outlying years that more closely aligns with historical enrollment?

ANSWER: VA believes the estimate of 1935 is reasonable given historical enrollment at this clinic and also believes that there is an opportunity for outreach in the geographic region to increase enrollment.

5. On page 17 Paragraph 2.1 of the solicitation states, “The clinical pharmacy specialist (CPS) shall be provided the same support staffing given to other providers on the team when they are working in the capacity of a mid-level provider” In addition, on page 49 paragraph 4.6.3.3.1 of the solicitation states, “Contractor to provide CPS support staffing, and ancillary support”.

Question: Is it acceptable to utilize a PACT team member for CPS support or will a dedicated MSA be required?

ANSWER: PACT team ember is acceptable for CPS support.

6. On page 17paragraph 2.1 of the solicitation states, “Clinical pharmacist will be pharmacist will be provided by VA onsite or via telehealth.” On page 49 paragraph 4.6.3.2.7 of the solicitation states “the Contractor shall meet all requirements for anticoagulation management outline in VHA Directive 1033 Anticoagulation Therapy Management (or subsequent revisions thereto) as well as local policy related to the management of patients on anticoagulation. Local policy is included in the attachments section. On page 54, paragraph 4.6.4.6.3 of the solicitation states “anticoagulation services will be provided by the Syracuse VAMC”.

Question: Is the VA or the Contractor providing anticoagulation services?”

ANSWER: The responsibility for anticoagulation management is a shared responsibility between the contractor and VA. The VA provides monitoring and management services for veterans on warfarin and injectable anticoagulants, while the contractor provides those services for stable veterans on non-warfarin oral anticoagulants (so-called Novel Oral Anticoagulants).

Contractor is responsible for the following:

PCP enters electronic consult for the “Anticoagulation Clinic”.

Lab Draws as scheduled by the Anticoagulation clinic.

PCP communications with Anticoagulation Clinic regarding medicine change, notification for surgical procedure.

Documents in CPRS any calls or appointments related to Anticoagulation issues.

7. On page 26 paragraph 2.8 of the Solicitation states Telehealth Clinical Technician (TCT) FTE Raito performance standard 1.0 FTE Telehealth Clinical Technician (TCT) per estimated 5,000 patients at clinic, with a trained back-up (e.g., another TCT or a member of the PACT Teamlet (LIP, RNCM, or LPN) depending on size of clinic/clinic workload/clinic telehealth services)”

Question: Is it acceptable to utilize an MA for the TCT position? In addition, with an enrollment of approximately 1935 in the first year, is it acceptable to hire a 0.4 FTE TCT or is the Contractor required to have a fulltime TCT available Monday through Friday, 8:00 AM to 4:30 PM? As the incumbent contractor, currently the Oswego CBOC has a full time TCT in place.

ANSWER: Primary is an LPN. Backup preferred to be an LPN but Health Tech is acceptable. Contractor is required to have a full time TCT.

8. On page 34 paragraph 3.2 of the solicitation states, “The clinic shall be accessible to VA Behavioral Health staff from 7:00 AM to 7:00 PM Monday through Friday” and on page 61 paragraph 4.6.5.3 states, “The Contractor shall provide a dedicated MSA who provides the following clerical support services for mental health providers at the CBOC”

Question: Given the Behavorial Health staff being there from 7 am to 7 pm and the requirement for a dedicated MSA, is the VA requiring more than one MSA to cover the 12 hours per day Monday through Friday?

ANSWER: The VA is not requiring the CBOC to be open for extended hours to see Veterans face to face. However, staff do need access to the CBOC for those hours as some may be on a compressed tour of duty (i.e. 8:00 AM – 6:30 PM) during which time they may be seeing Veterans via VVC. MSA support is not required for extended hours.

9. On page 35, paragraph 4.1.1 of the solicitation states, “NOTE: Patients referred for care outside the Contractor’s clinic (e.g. to VA community Care, VA facility, or similar) for care that the Contractor is required to provide under the terms of this contract will be removed from the applicable billing roster the month after the patient is referred for care.”

Question: Does this pertain to patients for example that may call the clinic at 4:15 pm (telephone triage) and the triage nurse (after discussing the issue) advises the patient that if they cannot wait until the next business day, the patient has the option to present to WellNow Urgent Care and use their VA benefits?

ANSWER: This refers to patients that are transferred permanently to Community Primary Care or transferred permanently to another VA facility. Nurse advise calls are episodic care, this section is referring to permanent transfer of primary care services.

10. On Page 45 paragraph 4.6.3.1 of the solicitation states, “The Contractor will provide all medications, including any necessary vaccines that are to be administered to patients in the clinic”.

Question: Will the VA please confirm that only the following vaccines are the responsibility of the Contractor?

a. TDAPg. Twinrix (Hep A&B)m. MMR Vaccine
b. Pneumovax 23h. Hep An. Tuberculin PPD 5 unit/0.1 ml injection
c. Prevnar 13i. Hep Bo. regular flu for younger Vets
d. TDj. HPV
e. Flu, High Dose Fluk. Depo
f. Shingrixl. Meningococcal Polysaccharide Vaccine

ANSWER: All of the above and Covid vaccine if it becomes a common vaccine.

11. On page 46 paragraph 4.6.3.1 of the solicitation states “Urgent/Emergent medications needed will be filled via VA contracted local pharmacy for up to 10 days.”

Question: Are the urgent/emergent medications provided by the VA contracted local pharmacy provided at the VA’s expense?

ANSWER: Yes, pharmacy will be providing an emergency first fill contract at the VA expense, to allow Veterans to get up to a 10-day supply of a new start medication at a local pharmacy.

12. On page 50, paragraph 4.6.3.3 of the solicitation states, “contractor is responsible for: Storage, Security, Administration and Monitoring Requirements of COVID-19 vaccine:” In addition, it states “Security”

· Ensure a plan is in place to ensure the security of the COVID-19 vaccine….

· Ensure a plan is in place for the clinic manager to reconcile the COVID-19 vaccines administered compared to on-hand inventory to monitor for inventory shrinkage. COVID-19 vaccine inventory shrinkage shall be reported to the VA Contracting Officer that at a minimum includes the number of COVID-19 vaccine does that cannot be reconciled, a brief summary of the review and corrective actions.”

Question: How often does the reconciliation need to be done, daily/weekly/monthly? If the clinic manager is not available to complete the reconciliation, is it acceptable to have the nurses do the reconciliation?

ANSWER: At this time, we will not be having COVID-19 vaccine stored outside of the main facility. At the point that we move to storing vaccine at off-site locations additional guidance will be provided. Currently we are required to monitor supplies daily (business days).

13. On page 51 paragraph 4.6.3.3 under Administration and Monitoring in the Solicitation states,

· Ensure COVID-19 vaccine is administered to the following groups in accordance with the SAVE LIVES Act (SLA):

· Veterans who are not eligible to enroll in the VA Health Care System:

· Specified Veterans who are eligible for hospital care, medical services and nursing home care abroad;

· Beneficiaries who are eligible for care due to a specified disability or death of a Veteran;

· Family caregivers of Veterans who are participating in the VA’s Program of Comprehensive Assistance for Family Caregivers;

· Caregivers of Veterans participating in the VA’s Medical Foster Home Program, Bowel and Bladder Program, Home Based Primary Care Program, or Veteran Directed Care Program; and

· The spouses of Veterans

· The Contractor will follow this procedure for administering COVID-19 vaccine to groups covered by the SLA: Follow procedure outline by the Veterans Service Center.

· The Contractor shall be reimbursed per the Medicare rate in effect at the time of vaccine administration for each dose of COVID-19 vaccine administered to individuals in the groups covered by the SLA. The contractor will not be reimbursed for administering COVID-19 vaccine to Veterans enrolled in care at the clinic or VA employees, or contract clinic employees that work onsite at the contract clinic location.

· The contractor shall submit a separate monthly billable roster for vaccinations administered to individuals covered by the SLA that include the following information:

· Patients name

· Date of service

· CPT code for each vaccine as delineated on the Centers for Medicare and Medicaid Services (CMS) website at cms.gov

· Other: Date of Birth

· Social security number

· Veterans Name

Question: The COVID-19 Vaccines administered to unassigned/non-enrolled patients in the clinic count towards the 40/month nurse-0nly visits? Will the VA CPS be available to draw up the COVID-19 vaccine doses for administration in the clinic? How would record keeping be accomplished for Administration of the COVID-19 Vaccine to spouses, caregivers of Veterans, etc. that are not in the VA’s healthcare system?

ANSWER: Yes, it counts towards the 40 visits per month. RNs will be able to draw up the COVID vaccine, the CPS may be virtual. At this time we are not storing vaccine outside of the main facility. If a COVID vaccine clinic is set up at a CBOC location, vaccine will be provided based on the number of Veterans scheduled. Pharmacy will either send a pharmacist to draw the doses or provide doses pre-drawn with the required paperwork to record administration of the vaccine.

14. On page 54 of the Solicitation under Ancillary Point of Care and Waived Testing Services, it states:

4.6.4.6.1 Mandated POC testing includes:

Pregnancy testing. Outpatient Site of Care must have point of care or stat pregnancy testing at the same site of care.

4.6.4.6.2 Non-Mandated POC Testing:

Glucose and fecal occult blood test.

Then on page 55 paragraph 4.6.4.6.7 of the solicitation states, “the VA will provide the test system/instruments and reagents for Contractor waived testing except for pregnancy testing, fecal occult blood testing cards and developer. The Contractor must contact the VA Ancillary Testing staff prior to purchasing test kits to ensure consistency of methodology/manufacturer.

Question: In 4.6.4.6.2 it states that Glucose and fecal occult blood tests are non-mandated test. 4.6.4.6.7 states that the VA will provide the test systems/instruments and reagents for contractor waived testing except for pregnancy testing, fecal occult blood testing cards and developer. Fecal occult blood test are non-mandated. The current process is that the Syracuse VA Medical Center sends the fecal occult blood cards to the clinics to be distributed to the patients. The patient then returns the cards back to the VA in a self-addressed stamped envelope for processing. Will this same process continue under the new contract?

ANSWER: The process will continue in regards to the FIT colorectal screening cards. The VA will send the FIT cards, envelopes, and prepaid postage stickers to the CBOC.

15. On page 60 paragraph 4.6.5.3.7 of the solicitation states, “Disaster preparedness: All contractor sites must have a designated Mental Health Disaster POC, who can serve as a member of the VA Parent facility’s Disaster Response Team”.

Question: Given that the VA provides all mental health staff, wouldn’t the Mental Health Disaster POC be a VA mental health staff member.?

ANSWER: Yes, this will be a VA staff member and is the person who sits on the Emergency management committee.

16. On page 64 paragraph 4.6.7.6 of the Solicitation states, “Contractor shall provide clerical support, including scheduling and ancillary support for VA telepharmacy services.

Question: Is it acceptable to utilize a PACT team member for support of VA telepharmacy or will a dedicated MSA be required?

ANSWER: PACT team member is acceptable. A dedicated MSA is not required.

17. VA Staffed Personnel

Question: How many VA staffed personnel will be providing services in the Tompkins CBOC and how many offices will be needed for VA provided staff?

ANSWER: Refer to space requirements for number of offices needed for VA staff. Exact number of staff assigned to the clinic is yet to be determined.

18. On page 74 paragraph 4.7.7 of the solicitation states, “Work Related Incident Treatment: When treating the Veteran for injuries sustained because of a work-related incident or an accident, the Contractor must complete the appropriate forms to allow the VA to assert a Federal Medical Care Recovery Act (FMCRA) or a Workers Compensation Claim.”

Question: What types of forms will the Contractor be required to fill out for Workers Compensation Claims?

ANSWER: NYS Workman’s Comp provider form (C4)

19. On page 77 paragraph 4.9.10 of the Solicitation states, “At the Contractors expense, the VA reserves the right to require documentation of proper operations, inspection, testing, and maintenance of fire protection systems, such as, but not limited to, fire alarm, fire sprinkler, standpipes, fire pump, emergency lighting, illuminated exit signs, emergency generator, prior to occupancy to ensure proper operation.”

Question: Is the VA looking for an emergency generator for the entire CBOC facility or for medication refrigerators?

ANSWER: The medication refrigerators need to be on a generator.

20. On page 85 paragraph 4.9.19 of the Solicitation states “Closed Circuit Television (CCTV) shall be provided by the contractor to monitor building entrances, restricted areas, mission critical asset areas, and alarm conditions. CCTV system shall be used for surveillance and observations of defined exterior areas, such as site and roadway access points, parking lots, building perimeter, and interior areas such as hallways, common areas and waiting areas, CCTV System will be viewed from a VA location within the clinic determined by the VA physical security officer. The design, installation and use of CCTV cameras shall support the visual identification and surveillance of persons, vehicles, assets, incidents and defined locations. The Contractor shall contact the VA Physical Security Officer at 315-425-4336 prior to installation of CCTV system to ensure proper placement.” The requirement for the VA to be able to monitor the CCTV activities remotely appears to be intended for a VA leased and operational facility as opposed to a contractor operated and leased facility. Our current contracted CBOC facilities across the U.S. utilize high resolution CCTV security cameras for both interior and exterior monitoring and routinely have recording capability of up to 30 days. This is the only specification that request the VA to monitor these cameras which require cellular based services and special equipment to accomplish.

Question: Was it the VA’s intent to utilize this level of monitoring by the VA, as opposed to the contractor responsibility, to monitor the camera activities at the privately leased contract facility? Or is the contractor required to record activity at the CBOC location that is made available for playback by the CBOC in the event of any criminal activity?

ANSWER: The VA does not need to monitor the CCTV activities remotely. The system is only required to record activity at the CBOC facility that is available for playback at the CBOC in the event of any criminal activity.

21. On page 87 paragraph 4.11.2 of the Solicitation states “Each IT and Telcom closet is required to have a card reader and motion alarm per VA Handbook 0730, Appendix B.” Then on page 87 paragraph 4.11.2.1.4 states “Primary lock, and a deadbolt (this can be one combined unit). If separate the locks will be keyed the same”.

Question: Is primary lock and deadbolt sufficient, or is there a card reader requirement? If a card reader is required, is that something the VA supplies or the Contractor? If the contractor supplies, what are the specs and do they need to connect to the VA somehow? Could not find the card reader requirements in the Handbook 730 appendix B.

ANSWER: Card reader is required and is supplied by the contractor.

SPECS:

ANSWER: VA provides the telephone lines and network access and cabling from the switch to the patch panels. The contractor provides the data lines running from the patch panels to the wall jack.

24. On page 101 paragraph 6.1.2 of the solicitation states, “The Contractor shall have 120 days from contract award to commencement of the provision of medical care to local Veterans.”

Question: Based on the size scope and logistics of drawings, permits and realistic construction timeframes, would the VA consider a 180 day start-up period for the cost effectiveness and efficiency for all systems start-up components.?

ANSWER: The contractor is required to begin providing services to Veterans 120 days after contract award.

25. Page 9, The CLINS reflects base year of 1935 primary care patients; the final sentence of section 2.1 states the Total Estimated patients enrolled are 1865 as of January 2021.
