STATEMENT OF WORK

1. GENERAL INFORMATION

1. Title of Project: Veteran Transitional Healthcare Service for Geri-Psychiatric cohort at James J. Peters (JJP) VA Medical Center, Bronx, NY

2. Background: Throughout the United States, the issue of preventable readmissions is a major concern costing healthcare systems billions of dollars. One estimate identifies “wasteful spending” directly related to unnecessary readmissions and inadequate care coordination at between $25 – $45 billion annually, and places responsibility for this outcome on inadequate care coordination by hospitals. This is not only an issue of “waste” of resources in the private hospital sector, but unfortunately, also for the VA healthcare system.

Unnecessary emergency department (ED) visits and unnecessary readmissions occur as a result of patients not following or sufficiently comprehending discharge instructions, nor understanding their symptoms and treatment protocols after leaving the hospital.

3, Objective: The goal of the proposed transitional care model is to help prevent suicides among high risk geriatric psychiatric patients (identified as veterans 65 years or older with a documented suicide prevention safety plan in their electronic health record) and mitigate COVID-related healthcare and communication challenges in the JJP VAMC geri-psych cohort.

4. Scope of Work: The Department of Veterans Affairs, James J. Peters VA Medical Center, Bronx, NY, requires a contractor to develop the proposed transitional care model. The contractor provide the geriatric psych cohort with clinical outreach, follow-up, and clinical connection to ensure they are not lost to follow-up and continuity of care is maintained. These Veteran Health Extenders shall be hired by the contractor; these staffers will consist largely of Licensed practical Nurses to perform home visits with patients recently discharged from our inpatient unit. These home visits are essential for geri-psychiatric patients who refuse to come onto the campus for safety concerns (fear of COVID-19), transportation barriers (unable to drive or take public transportation) or health barriers (unable to walk to recent surgeries, arthritis, or other physical ailments that come with older age). The health extenders can provide in person care, health assessments, and facilitate Virtual health care set-up at the patients home to encourage future telehealth visits.

5. Performance Period: Date of award through one year.
