About this file

This is an amendment to a request for quotes (RFQ) for an endoscope equipment lease and service agreement issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2. The RFQ seeks quotes for a firm-fixed price contract to provide endoscope equipment on a lease basis along with repair services at the Syracuse VA Medical Center. Quotes are due by November 10, 2020 and will be evaluated based on technical capability, past experience, and price, with award made to the lowest priced technically acceptable offer. Technical capability requirements include providing and repairing the equipment listed in the statement of work, loaning replacement equipment during repairs, and ensuring leased equipment integrates with existing documentation software. Offerors must also describe reprocessing leased equipment using the medical center's current capabilities. References and a subcontracting plan are required to demonstrate past experience.

Michelle A. Harsch Contracting Officer

52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

A firm-fixed price contract will be awarded to the responsible offeror that offers the lowest price, technically acceptable offer in response to this Request for Quotes. Each quote must contain all the information required by the RFQ. A responsibility determination will be made in accordance with FAR 9.1, Responsible Prospective Contractors.

Award will be made based on the lowest evaluated price of quotes meeting or exceeding the technical acceptability standards for non-cost factors (1 and 2).

The offeror’s technical quote shall be credible and meet the capability requirements as follows:

FACTOR 1 – CAPABILITY

Technical Capabilities:

a. Explain ability to provide all equipment listed in the “equipment requirements” of the Statement of Work

b. Describe ability to repair all equipment listed in the “equipment requirements” of the Statement of Work (with the exception of video processors and light sources)

c. Confirm the ability to provide loaner equipment while any item covered by the service agreement is out for repair

d. Describe the ability of leased equipment to integrate with existing Endosoft documentation and reporting software

e. Describe the requirements for reprocessing/cleaning all leased equipment using equipment currently available at the Syracuse VAMC and in accordance with VA Directive 1116(2)- STERILE PROCESSING SERVICES (SPS)2016-03-23

f. Submit a copy of offerors subcontracting plan.

FACTOR 2 – PAST EXPERIENCE

Submit at least three (3) references that reflect satisfactory experience of providing endoscope equipment lease and repair services for contracts of similar scope, size and complexity that are either ongoing or were completed within the past two years. Include the following information:

Company name and address
Name, telephone number and e-mail address of Company POC
Dates of contract performance and award amount

The government may seek relevant performance information on all responsive and otherwise Technically Acceptable offerors through sources such as DCMA offices, CPARS, PPRS or other government data bases and other sources that are available. The government shall not be limited in its search for performance data. The offeror shall not submit performance data for this factor. When performance is evaluated, the outcome is either satisfactory or unsatisfactory. Unsatisfactory performance will disqualify an offeror from award; however, no past performance will be rated neutral.

FACTOR 3 - PRICE-Complete the B.3 Price Schedule

1. Price will be evaluated fair and reasonable based on the total aggregate cost of a Base plus two (2) Option periods.

**Quotes are to be submitted electronically in two (2) parts to Michelle.Harsch@va.gov. Part 1 shall be Technical Capability and Past Experience and Part 2 shall be the Price Quote (B.3 Price Schedule)

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
