Canandaigua NY 14424 This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 621498, Size Standard $20.5 Million. The Network Contracting Office at Canandaigua (NCO 2) is seeking potential qualified contractor(s) to provide all of the services related to running and staffing a VA Patient Aligned Care Team (PACT) model Community Based Outpatient Clinic (CBOC) in the following locations in the State of New York:
· Kingston (Ulster County)
· Plattsburgh (Clinton County)
· Schenectady (Schenectady County)
· Troy (Rensselaer County)
Responses to this sources sought notice must include all of the following information:

11. Have you ever submitted an offer for a VA CBOC in New York State? If not, please explain your interest and/or capabilities in expanding services into New York State.
12. Has your company ever run a CBOC with 1,200 patients – 2,200 patients?
13. What locations would your company be interested in submitting a proposal for?

Responses to this market survey should be in writing only and emailed to Heather Schrader, Contract Specialist at heather.schrader@va.gov. No telephone responses will be accepted. Do not submit capability statements. Only responses to the above questions will be considered responsive to this market research request.
Responses must be received no later than Noon on September 10.
