STATEMENT OF WORK FOR
VA MANHATTAN CAMPUS PATIENTPOINT PATIENT HEALTH EDUCATION CONTENT AND TECHNOLOGY DELIVERY SYSTEM
Introduction/Background
The VA New York Harbor Healthcare System is dedicated to providing quality health care to Veterans using the abilities of all employees supported by our commitment to education and research.
We are recognized locally, regionally, and nationally as a leader in quality patient care, positive customer service, medical/allied health education, health-related research and employment opportunities. We seek to be the provider of choice of Veterans and the community by offering an efficient, integrated quality health care system capable of providing a full range of primary, specialty and chronic health care services in a system that is readily accessible and responsive to change.
Our key business drivers are quality, financial integrity, patient satisfaction and employee satisfaction.
VA Manhattan Library Service
The Learning Resources Center (“library service”) at the NY Harbor Healthcare System’s Manhattan campus is committed to advancing Veterans’ health education. The library service would like to procure PatientPoint digital patient health education which would improve compliance and success with national initiatives, such as Whole Health and Patient Centered Care. The utmost important benefit is improved health literacy and overcoming barriers to successful patient engagement. Digital patient health education resources that describe and illustrate the management of diseases and disorders have the ability to change communications into actions and promote the practices of preventive medicine and patient centered care. Patients educated about their conditions, treatment plans, and progresses are more likely to become actively involved in their care. As patients assume responsibility for their own care, and change and improve health behaviors, they become partners in care with their PACT Teams. This relationship empowers patients, ensures same day access/continuity of care, increasing patient satisfaction. Further, adherence to medication and treatment improves patient safety, which, in turn, reduces the clinicians’ risk of liability and leads to more efficient and cost-effective health care delivery. PatientPoint’s digital patient health education materials also ensure standardization of processes so that all staff approaches patient education the same way, and all patients receive quality education resources based on best practices. PatientPoint advertises that each piece of the content is reviewed by a practicing physician in the appropriate specialty to ensure information is accurate and up-to-date.
Objective
VA New York Harbor Healthcare System (VHANYH) requires digital patient health education that can be delivered at the point of care within outpatient settings. PatientPoint can provide Waiting Room and Exam Room Digital Screens that are separate from the VA network and can educate patients on the conditions most often treated by VA PACT and MH Teams. Also available from PatientPoint are Interactive Exam Room Tablets for the physicians to enrich the exam room experience with multimedia education (articles, videos, 3D anatomical visuals) to aid patient comprehension and reinforce key steps for treatment compliance. VHANYH staff may also utilize PatientPoint’s free mobile engagement platform with secure, personalized communication, education and care coordination. All content is kept up to date with regular refreshes. Content is reviewed by subject matter experts in each specialty to ensure information is accurate and up to date. Unlike other vendors that require a connection to the hospital network, PatientPoint prefers to operate on its own WiFi network to reduce opportunities for information security breaches.
