COURIER SERVICE
1. GENERAL: Contractor shall provide all labor, material, equipment, transportation, and certification/licensure for transportation of clinical specimens to various labs for testing as needed. The trips will consist of pickup/delivery of lab specimens between VA New York Harbor Healthcare Systems (VA NYHHS) three main facilities (Manhattan Campus, Brooklyn Campus, & St. Alban’s Campus) and its two CBOC’s (Staten Island and Harlem CBOC’s). 3. PERFORMANCE PERIOD:
· June 7, 2018 to December 7, 2018
4. DESCRIPTION: A courier service is required to pick-up and to deliver clinical specimens to our Manhattan Campus. The majority of the trips will be for special blood bank procedures; therefore, courier service needs to be available anytime day or night during the contract period of performance. Deliveries from VA NYHH’s Brooklyn Campus to the Manhattan Campus would be approximately four (4) times a day; 9am, 11am, 1pm, and 3pm. VA New York Harbor Healthcare System requires the courier to be at the VA Medical Center for pick-up within 30 minutes from the time a service call is placed. C.4 52.216-21 REQUIREMENTS (OCT 1995)

(b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. Subject to any limitations in the Order Limitations clause or elsewhere in this contract, the Contractor shall furnish to the Government all supplies or services specified in the Schedule and called for by orders issued in accordance with the Ordering clause. D.1 QUALITY ASSURANCE SURVEILLANCE PLAN:
Purpose:
The purpose of this Quality Assurance Surveillance Plan (QASP) is to provide guidance on how the quality of services provided under this contract will be monitored.

Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance.
This QASP is a “living document” and the Government may review and revise it on a regular basis. However, the Government shall coordinate changes with the contractor. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities.
Scope:
The areas to be addressed under the QASP are as followed:
