1.0 Project Summary: At this time the James J. Peters VA Medical Center is in need of replacing the existing InMotion wrist/forearm and shoulder/elbow robots with newer updated equipment. These robots are supported by over 25 years of clinical research and allow patients with neurological disorders to perform intensive repetitive therapy needed for brain plasticity.

2.0 PPS Workflow: The required robotic systems are designed to target the sensory-motor systems needed for motor learning. The software is designed to interact with the patient with use of task-specific activities that are user friendly. The robotic system delivers reports about the patient’s movement patterns which provides feedback for the clinician’s assessment. The system shall be equipped with a highly-configurable program that can be changed to our facilities’ needs, including:

· Clinical degrees of freedom for wrist and shoulder

· Patient ergonomics

· Adjustable height table to accommodate wheelchair users

· Checking

· Fast and efficient setup

· No active patient movement required

· Task specific training

· Safe and effective treatment

· User friendly for multiple staff use

· Ability to service a wide range of patients, such as patients at the acute, subacute and chronic phases of recovery with mild to severe impairments will benefit from robotic therapy.

General Process Flow:

1. Occupational therapist (OT) performs an initial evaluation and deems no contraindications for use of robot, such as, significant joint pain, fixed contracture, advanced dementia.

2. OT prepares the workstation for patient access and sets up robot with the appropriate protocol.

3. Patient accesses robot workstation with wheelchair or standard clinic chair if ambulatory.

4. OT positions and straps the patient’s affected arm on the trough.

5. OT educates and provides instructions of the robotic activity.

6. Once patient completes a specific number of repetitions of the activity, OT reviews the movement patterns with patient, and another set of repetitions are performed.

7. Every session patient completes at least 400 repetitions of movement.

Detailed Process Flow

Intake Process Occupational therapy receives a doctor’s referral for patient with neurological disorder and an initial evaluation is scheduled to assess sensation and motor function. Once all contraindications have been ruled out, patient is scheduled for occupational therapy treatment sessions for robotic therapy. Patient attends therapy sessions and completes at least 400 repetitions of arm movement to promote neuroplasticity.

Prioritization Research supports robotic therapy is highly beneficial throughout the stages of stroke recovery. Patients with newer stroke diagnosis will be prioritized to optimize recovery during this prime window of opportunity.

3.0 Standard Features of robotic systems:

· A range of protocols, reports, and tools to choose from

· Clock activity - After each set of 80 motions, a performance metrics display will provide the patient a quantitative score. After 320 motions, it will show a bar graph of 4 sets of these scores

· Other games - Cretan Square Maze Game; Pick game; Pong game; Race game; Squeegee game

5.0 User Training:

The occupational therapists received training in 2017 and vendor will provide additional training with the robots once delivered.

On the job site training will consist of:

1. Robotic components and adjustability

2. Target population and contraindications

3. Preparing patients for therapy sessions

4. Running the clock therapy game tasks

5. Clock game therapy protocols

6. Performance metrics in adaptive clock game

7. Performance and utilization progress reports

8. Clock games menu console function

9. Printing reports

10. Powering robot off

6.0 Customer Responsibilities:

· Customer to provide spacing requirements

· Customer to provide all power requirements including electrical outlet

· Customer to ensure patients are appropriate for robotic therapy

7.0 Other Project Assumptions:

· All installation work will be conducted during normal business hours. Normal business hours are defined as 8AM to 5PM Monday thru Friday, excluding national holidays.

· All parts and services will be supplied by Bionik Laboratories.

· Any requests for new/modified functionality, additional components or additional services not covered in this SOW will be considered a scope change. Parts and/or labor associated with any scope change will be covered by a Vendor project change order.

10.0 Schedule
