Michael Spivack Contracting Officer The purpose of this amendment to the solicitation is to answer the following vendor questions:

Question #1: Will the government require a licensed medical technician, LPN, RN or any other “medical” person to sign the contractor forms signifying that the tests are completed?

Answer: Yes, for the medical evaluation. No, for the fit testing.

Question #2: In accordance with paragraph 3 Responsibilities to clarify the government (VAWNYHS) will provide the forms (documentation) for completion of valid tests?

Answer: Yes

Question #3: Will the government provide room/rooms for testing and training

Answer: Yes

Question #4: Is it possible to schedule a minimum of 2 consecutive 8 hour shifts per visit or are the shifts scheduled a day here a day there etc?

Answer: No, it has classically been done with consecutive days but not consecutive shifts beyond overlapping into nights and evenings.

Question #5: I am wondering if the 600 employees are ALL doing medical evaluations, as OSHA does not require annual medical evaluations only initially and if there are any changes?

Answer: Based on past experience the Government estimates that 75 - 80% of staff will require medical questionnaire only. 15 - 20% will require additional questionnaire and communication with a physician, and the remaining may need to see a physician.

Question #6: Paragraph v page 2 states contractor will provide pricing for N95 Respirator Fit Testing and Training. Does the government require a breakdown of pricing or providing the unit price and total price is all that is required? Does the government require a narrative of our pricing?

Answer: According to the attached price schedule the unit price and total price is all that is required. How costs are distributed is up to the vendor. However, in accordance with 52.212-1(e) the offeror can present alternative line times.

Question #7: In the event we have more questions is there a cut time for submitting questions?

Answer: The VA will do its best at answering all questions, but questions posed after 04/25/2018 are not guaranteed to be answered in a timely manner or before the closing date of this solicitation.

All other terms and conditions remain the same.
