B8 Mold & Asbestos Remediation & Drainage Repairs

PROJECT SCOPE

GENERAL PROJECT WORK SCOPE DESCRIPTION

Existing Conditions:

The Canandaigua VAMC is located at 400 Fort Hill Avenue, Canandaigua, NY 14424. The layout is a campus with more than 30 buildings located on over 150 acres. Normal working hours are Monday – Friday 8:00am – 4:30pm. Building 8 houses numerous functions including, but not limited to audiology, chaplain services, patient work spaces, and patient care areas on the first (A) and second (B) floor. This remediation effort will take place in portions of the audiology and patient computer lab on the ground (G) floor. Work will also take place on the exterior of the building to fix water drainage issues.

Scope of Work:

The scope of this project is to remediate all mold and asbestos in rooms 9A (Audiology) and 10 (Patient Computer Lab) and to install drainage tile and waterproofing to prevent further water infiltration.

It is highly recommended that the contractor attend the pre-bid site visit outlined in the solicitation. The facility is occupied by personnel including patients on 8B (second floor). Special precautions are to be taken to ensure that air and waste materials are handled and disposed of properly and in accordance with regulations. If debris is removed through the tunnel, the contractor shall ensure the tunnel is cleaned of tracked, dust, etc. All items remaining in the room when it is turned over to the contractor are to be removed and disposed of in accordance with regulations. Special care will be taken to not damage the white fence surrounding the courtyard. Off-hour work may be required in order to minimize impact to services provided in the facility.

The contractor will provide all necessary labor, materials, and supervision to execute the work detailed in the construction documents. The contractor will be required to submit a proposed mold and asbestos remediation plan to the VA prior to work start. This plan will include copies of necessary certifications and an outline of procedures that will be followed including personal protective equipment that will be worn. Refer to specifications and submittal section of this project scope for additional submittal information. The work must be accomplished by a team that includes a certified mold remediation specialist and a certified asbestos remediation specialist and must be in accordance with the specifications and all regulations. Placement of the shower and decontamination unit will be in coordination with the COR. Once abatement is complete, 1 hour fire rated construction barriers must be installed for the project duration.

Negative air pressure is to remain in place until post remediation mold and asbestos results for the area are returned negative, the room is deemed clear and the contractor has gained approval from the COR. The contractor is to place a manometer in both rooms to ensure negative pressure is maintained throughout the project. The location of the manometer will be decided in coordination with the COR. The VA will procure a third party sampling and testing company to monitor both the mold and asbestos remediation.

The contractor is to clean, charge, and return to service all HVAC units, condensers, etc. found in the space and that service the space.

Lead based paint is not anticipated on the project; however the contractor is required to perform an inspection of any area that shall be disturbed for lead based paint. Asbestos floor tile, with mastic is present and is to be abated under this project. Any additional issues identified are to be brought to the CORs attention before proceeding.

Liquid membrane waterproofing will be applied to the subsurface exterior walls as shown on the drawings. A new drain line is to be installed from the drainage point shown on the drawings to a connection point at the main sanitary drain line.

Full depth excavation will occur to the bottom of the footer in designated areas. 4” perforated pipe drainage tile will be installed. The tile will be tied into the existing drainage system as shown in the drawings.

Work done under this project must comply with the provided infection control risk assessment (ICRA) including negative air pressure through the use of a HEPA filter and complete isolation of the area including sealing of the area with duct tape. All work area exits are to be HEPA vacuumed and damp mopped with a detergent solution at the end of each daily work shift. The work space is to remain locked and secured at all times.

Work includes, but is not limited to:

Demolition

· Remove all carpeting from rooms 9A and 10 (approximately 600 SF)

· Removal all asbestos tile and mastic from rooms 9A and 10 (approximately 600 SF)

· Remove all cove base from room 9A and 10 (approximately 130 LF)

· Remove sheetrock, paint, plaster, etc. in areas shown in the drawings. Remove from floor to ceiling (approximately 8’) and beyond if required

· Remove all ceiling tile (approximately 600 SF)

· Notify the COR if mold extends beyond the remediation areas shown in the drawings

· Remove existing countertop

Reconstruction

· Replace carpeting (approximately 600 SF) as shown in schedule of finishes

· Install new cove base (approximately 130 LF) as indicated in schedule of finishes

· Patch, paint and refinish walls, doorframes, and window frames to match existing

· Paint all walls, door, and other surfaces (including those not remediated/repaired)

· Material must be applied within manufacturer’s specifications

· Material must be guaranteed by the manufacturer in writing

· Material must be proven in subsurface applications to completely stop water intrusion in similar regions to this VA

Drainage Repairs

· A 12” trench will be excavated around the building as shown in the drawings (approximately 60 LF).

· Full depth excavation will occur to the bottom of the footing in these designated areas. The contractor is responsible for disposing of all materials removed during excavation. Disposal of these materials will meet all regulatory requirements.

· A 6’ tall chain link fence shall be erected around the trenched area prior to work start. OSHA requirements are to be strictly adhered to and construction signs will be posted while trenching operations are on-going. The area is to be secured from access at all times. Keys to the area will be provided to the COR in accordance with the specifications.

· The drainage tile will be piped into a new storm drain run that runs to the main as is shown on the drawings.

· Flat-style geotextile fabric will be installed at the bottom of the trenched area following compaction testing. Fabric sides will be overlapped at least 4” throughout. This geotextile will be sufficient in size to fully wrap around the backfilled material and pipe once it is placed.

· Drainage tile will be placed as is shown on the drawings.

· The top of the drainage tile will be >1’ below the bottom of the basement floor slab throughout.

· Sock-style geotextile fabric shall encase the drainage tile in accordance with the specifications.

· The drainage tile shall be sufficient in strength such that it does not decompress or become damaged when the area is backfilled.

· The excavated area will be backfilled with stone in two 6” layers to within 1’ of the surface with 1” washed gravel fill.

· The last foot from rock to surface will be topsoil.

· The contractor shall run a camera through the line at project completion to visually show the VA that the pipe is completely open. The contractor shall commission the line to show that it functions.

· The last foot from gravel to surface (above the flat-style geotextile fabric) will be filled with topsoil.

· The surface will be returned to the condition it was in prior to turn over for the project. This will include returning and/or replacing all plants, flowers, wood chippings, etc., in kind.
