Description This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services,” as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.
This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2025-04.
This solicitation is set-aside for VOSB
The associated North American Industrial Classification System (NAICS) code for this procurement is 541380, with a small business size standard of $19.0 Million.
The FSC/PSC is J045.
The Edward P. Boland VAMC at 421 N Main St, Leeds, MA 01053 is seeking a qualified contractor to perform comprehensive inspections and maintenance on all doors associated with their Fire Protection System.

PERFORMANCE WORK STATEMENT
Central Western Mass VAMC Backflow Prevention Device Testing
1. GENERAL:
The Edward P. Boland VAMC is required to test 52 backflow preventing devices semi-annually in accordance with 310 CMR and also performing annual testing on 23 eyewashes/showers in accordance with VHA directive 7704. Provide professional services to include all labor, transportation, materials, apparatus, tools, equipment and permits necessary to test and repair all backflow prevention devices at Central Western Massachusetts VAMC (421 N Main St, Leeds, MA 01053) and 6 Cherry St, Northampton, MA 01053. A contract shall be awarded for a base year (12 months) with (4) potential annual option years to be exercised at the discretion of the Contracting Officer. Testing must be performed by a Certified Backflow Preventer Device Tester in accordance with 310 CMR.
2. PROJECT SCOPE OF WORK
a. All Backflow tests will be performed by a certified backflow tester. With each device that is tested the tester will prepare a paper copy of the test report to be left with the site point of contact. Example test form in Attachment A. Each report shall include the device location, serial number, tester’s name, signature, test date, and test results. After backflow devices have been certified a written report shall be submitted to the point of contact at the Edward P. Boland VAMC. This report shall include all inspection sheets for each backflow device tested.
b. The contractor shall provide detailed pricing per backflow device/unit.
c. Two of the backflow devices require non-permit confined space entry.
d. The contractor shall provide a repair line for five thousand dollars this line item can executed by the VA if necessary for devices that may fail inspection.
e. Any operational issues that cannot be immediately repaired and retested, shall be communicated to the COR, a repair and retesting plan shall be communicated within 24 hours, including a quote for the repairs. Regulations require the repair and re-test of defective devices be completed within seven calendar days of the failed test.
f. In the event of a backflow failure, the VA will reach out for a test and repair plan. The contractor will respond within 24 hours to provide a service date. If repairs are required, the contractor will provide a repair quote within 24 hours. Following a repair the Contractor will provide a test report (e.g., attachment A).
g. All eyewash tests will be done by someone who is ANSI certified.
h. Work will be in accordance with applicable VHA, OSHA, EPA, and Mass DEP standards and regulations.
i. All activities shall be coordinated with the Facility Management Service (FMS), which shall ensure access to all areas of work.
j. See the list of backflow prevention devices to be tested below and its respective location, size, model, and serial number. The VA reserves the right to add additional backflow test locations throughout the life of this requirement at an additional cost.

3. SUBMITTALS
a. At the time of bidding, the Contractor will submit to VACWM of all materials to be used, including MSDS sheets. Credentials of all technicians performing the testing and repair will be provided to VACWM COR two weeks prior to initiation of testing and repair.
4. CODE REQUIREMENTS AND COMPLIANCE
a. All work and services shall be executed in accordance with National Fire Protection Association Standards for Healthcare Facilities, Veteran’s Administration (VA) protocol, standards, as well as manufacturer recommendations. This entire project shall conform 100% to all OSHA standards, and applicable building codes, as well as VA standards. There are backflow devices located in areas identified as confined space locations and will require correct safety measures taken.
b. The Contractor shall comply with the public laws and statutes including all changes and amendments of federal, state, and local environmental statutes and regulations in effect of date of issuance of this contract, including, the applicable portions of the documents cited in the basic contract and below.
c. In the event the VA COR or POC identify issues with safety, the work will be stopped until the issues are addressed.
5. RESOURCES:
a. The Government will not furnish any resources for the fulfillment of the services required under this contract. The contractor must provide all tools, equipment, materials, transportation, professional expertise, personnel, and supervision for all services required under this contract. The contractor must demonstrate satisfactory operation of the devices to the COR for approval.
6. WORK LOCATONS AND RESTRICTIONS:
a. Testing will be coordinated 7 calendar days in advance of the planned work. Any required shutdowns will be planned at least 72 hours in advance of the planned work.
b. All work will be done between the hours of 7:00AM to 3:30PM Monday through Friday, or weekends, holidays excepted, unless other times are arranged in advance and approved in writing by the COR and no additional charges are accrued .
c. When the Contractor’s work interferes with hospital functions, such as when work produces excessive noise, odors, dust, utility service interruptions, or other interferences with normal hospital operations that cannot be contained within the area of work, the Contractor will schedule said work at other than normal hours and as directed by the COR at no additional cost to the Government.
7. ARRIVAL (CHECK-IN/CHECK-OUT) REQUIRMENTS:
a. The certified tester shall report to the COR to check-in/check-out. This check-in/check-out is mandatory.
b. Work sites shall be left in the same manner of cleanliness and order in which they were found.
8. IDENTIFICATION, PARKING, SMOKING AND VA REGULATIONS:
a. The Contractor shall always wear approved Department of Veterans Affairs identification while on the premises of the VAMC.
b. It is the responsibility of the Contractor to park in the appropriate designated parking areas. The VAMC will not invalidate or make reimbursement for parking violations of the Contractor under any conditions.
c. The Department of Veterans Affairs is a smoke free environment and smoking is not permitted at the facility.
d. All Contractor personnel are subject to inspection of personal effects when entering or leaving the project site and shall have state ID and an appropriate form of commercial identification.
e. If a contractor will be working on site more than 5 working days, identification badges must be procured by contacting the COR or POC. All Contractor personnel are required comply with VAAR 852.273-74 (Available online at VA Technical Information Library) which covers the following requirements: background checks, and the wearing of identification (I. D.) badges issued by the VA. While on VA Facilities grounds, temporary or official I.D. badges shall be worn continuously. Failure of the background check may result in specific personnel being disallowed to work on VA premises and the Contractor will need to provide replacement personnel.
f. Possession of illicit drugs, alcohol, and weapons are prohibited. Enclosed containers, including tool kits, shall be subject to search.
g. Proper PPE shall be worn, as necessary to the task being performed and/or the area, where work is being conducted.

9. PERSONNEL ASSIGNMENT:
a. The Contractor shall assign the appropriate quantity and type of personnel to successfully complete all functions specified in this SOW. The Contractor shall maintain throughout the life of this contract, a current list of technicians and their credentials available for inspection by the Government at any time.
10. PERIOD OF PERFORMANCE
a. Period of Performance: Base + 4 Option Years (OY).
b. Each Backflow device is to be tested semiannually.
c. Each eyewash/shower is to be tested annually.
d. Work shall begin after Notice to Proceed is given to the contractor by the assigned point of contact.
e. All services shall be invoiced after receipt of Service.
The contract period of performance is 09/15/2025 to 09/14/2026.

The following solicitation provisions apply to this acquisition:
1. FAR 52.212-1, “Instructions to Offerors–Commercial Products and Commercial Services”
2. FAR 52.212-3, “Offerors Representations and Certifications–Commercial Products and Commercial Services”
Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://sam.gov/ in accordance with FAR 52.212-3, “Offerors Representations and Certifications–Commercial Products and Commercial Services.” If paragraph (j) of the provision is applicable, a written submission is required.
3. FAR 52.212-2 Evaluation-Commercial Products and Commercial Services [In accordance with FAR 12.301(c) when the use of evaluation factors is appropriate, the contracting officer may (1) Insert the provision at 52.212-2, Evaluation-Commercial Products and Commercial Services, in solicitations for commercial products or commercial services (see 12.602); or (2) Include a similar provision containing all evaluation factors required by 13.106, subpart 14.2 or subpart 15.3, as an addendum (see 12.302(d)).]
The following contract clauses apply to this acquisition:

1. Questions: Any questions must be submitted in writing no later than Wednesday 21 2025 at 3:00 PM EST. Answers to the questions will be posted by the following week.
2. Quote: Offeror shall provide a quote using the Line-item table above. Offerors must also provide all applicable labor rates for the solicited services so they can be compared to the applicable wage determination for compliance with the Service Contract Labor Standards.
All quotes shall be sent to the Contract Specialist, Shawn Lawrence at Shawn.Lawrence@va.gov.
Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. Options will be evaluated at time of award.
The following are the decision factors:
1. Price: Quotes will be compared by price.
1. Technical: The offeror’s quote shall be evaluated to determine if the organization has the experience and capabilities to provide the requested services IAW the Statement of Work in a timely efficient manner.
1. Contractor shall provide specification sheets for proposed equipment and include within the solicitation response.
The award will be made to the response most advantageous to the Government.
Responses should contain your best terms, conditions.
To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:
"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."
OR
"The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"
Quoters shall list exception(s) and rationale for the exception(s), if any.
Submission of your response shall be received not later than 27 August 2025 at 17:00 EST. All quotes shall be emailed to Shawn Lawrence at Shawn.Lawrence@va.gov.
Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
