Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Items,” as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.

This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-07 effective Aug 10, 2022.

This solicitation is set-aside for SDVOSB.

The associated North American Industrial Classification System (NAICS) code for this procurement is 621511, with a small business size standard of $16.5 Million.

The FSC/PSC is H999.

The following solicitation provisions apply to this acquisition:

Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, “Offerors Representations and Certifications–Commercial Items.” If paragraph (j) of the provision is applicable, a written submission is required.

The following contract clauses apply to this acquisition:

52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES

(a) Basis for Award. The Government will issue a purchase order to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government, price and other factors considered.

(b) The following factors shall be used to evaluate quotations:

(1) Technical Capability

(2) Past Performance.

(3) Price

(c) Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations. The evaluation will consider the following:

(1) Technical or Quality: The quotation will be evaluated to the extent to which it can meet and/or exceed the Government’s requirements as outlined in the solicitation and based on the information requested in the instructions to quoters section of the solicitation.

(2) Past Performance: The past performance evaluation will assess the relative risks associated with a quoter’s likelihood of success in fulfilling the solicitation’s requirements as indicated by the quoter’s record of past performance. The past performance evaluation may be based on the contracting officer’s knowledge of and previous experience with the supply or service being acquired; Customer surveys, and past performance questionnaire replies; Contractor Performance Assessment Reporting System (CPARS) at http://www.cpars.gov/; or any other reasonable basis.

(3) Price: The Government will evaluate the price by adding the total of all line item prices, including all options. The Total Evaluated Price will be that sum.

(d) Options. The Government will evaluate quotations for award purposes by adding the total price for all options to the total price for the basic requirement.

To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:

"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."

"The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"

Quoters shall list exception(s) and rationale for the exception(s), if any.

Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).

II. Project Scope

1. The White River Junction VA Healthcare System requires the services of a qualified firm to provide services allowing for water quality sampling and testing.

2. The Contractor shall also be responsible for all materials delivered and work performed until completion and acceptance of the entire work, except for any completed unit of work which may have been accepted under the contract.

3. Existing Site Conditions. Construction will occur within the Sterile Processing Department, all activity within the Sterile Processing Department (SPD) must follow the proper PPE procedures outlined by WRJ SPD.

4. Work shall be completed in conformance with the time schedule requested by the VA Contracting Officer’s Representative (COR) or Resident Engineer (RE). The Contractor is solely responsible for preparing all necessary regulatory notifications to meet the time schedule.

III. Tasks and Requirements A. General Contractor Requirements Contractor shall furnish all labor, supervision, materials, supplies, and equipment necessary to accomplish each listed task in this statement of work to accomplish the requirements of the VAMC.

2.1 Water Sample Collection

The Contractor will collect water samples from fourteen (14) designated sampling points within the facility Sterile Processing Service (SPS). Sample parameters and frequency will be in accordance with Table 1 in this document. Refer to AAMI TIR34 for more details on testing requirements.

Sample points will be for SPS critical water as defined by AAMI TIR34. This includes utility water entry point to the building, critical water feed port to SPS, 4 sinks, and 8 Automatic Endoscope Reprocessor (AER) ports. Samples can be collected from the existing ports for the AERs and any faucet on the sink. Sample point quantities and locations are subject to change and will be issued via modification.

Two sample ports, building entry port and critical water feed port, are located in a mechanical room and will require no specialized PPE. All existing AERs and sink sample points are in the decontamination side of SPS which requires specialized personal protective equipment (PPE) to enter. The contractor will be required to don and doff, per WRJ VAMC protocol, all appropriate PPE which will be provided by the VAMC upon entry.

Samples are to be collected and received/accepted by the testing laboratory within 24 hours. Contractor to ensure samples arrive during the laboratory’s normal operating hours.

The contractor will be required to schedule water sample collection with the COR.

Contractor staff collecting samples will be trained and follow SM 9060 for potable water sample collection.

2.2 Water Sample Analysis

Sample analysis will be conducted by a laboratory that holds current accreditation for each analyte and method. Accreditation and qualifications of testing laboratory must be provided to the VA during solicitation.

2.3 Results Reporting

The VAMC will be provided with a copy of the sample chain of custody and laboratory results reports. Turnaround times (TAT) for results reporting are listed Table 1. Results may be provided to designated VAMC points of contact via email.

Additionally, the contractor will provide a copy of the standardized testing process for each parameter for government review prior to work start. This process must be followed each time the samples are taken a report furnished to the VA stating that the testing was done in accordance with the approved procedures.

Results reports will include the name of the laboratory that performed the analysis, test method, sample source name and location, the date and time the sample was collected, received and analyzed.

The Contactor will notify the VAMC of any samples that exceed hold time or temperature, whereupon the VAMC will determine whether to proceed with analysis or have the Contractor resample at no additional cost to the VAMC.

2.4 Resample/Additional Samples

If any samples results are out of specifications with AAMI TIR34, the VAMC may request additional samples be collected outside the regular sample frequency. The Contractor will have 7 days upon notice by the VAMC to resample or collect additional samples.

2.5 Responses

The Contractor will respond to basic inquiries and clarifications of sample results within 6 hours of inquiry from the VAMC. The Contract will notify the VAMC 14 days in advance to schedule regular sample collection. The Contractor will coordinate timing with the COR so SPS operations are not impacted. This may mean testing has to occur off-tour.

3.1 Materials

The Contractor is responsible for supplying all materials and supplies needed for sampling under this contract.

4.1 Performance Requirements

The Contractor has an ongoing obligation throughout the Period of Performance to notify the VA Contracting Officer immediately and in writing, upon its loss (or any of its subcontractors) of any required certification, accreditation, or licensure.

Contractor shall follow VA requirements for physical and information security. Contractor’s assigned employees will be subject to background investigation and shall obtain temporary contractor badging for all employees servicing the contract prior to performing site work.

All work will be performed between the hours of 7:00am to 4:30 pm Monday through Friday, excluding federal holidays, except where off hours are specified. Off hours and weekend work may be requested of the VAMC Contract Officer’s Representative (COR) with a 48-hour request notice.

The provisions of all applicable OSHA, EPA, and all Federal, State, and local regulations shall be followed throughout the course of this project.

Smoking/Tobacco/Vaping is not permitted on campus.

5.1 Safety Requirements

The Contractor will comply with all OSHA, NEC, as well as all national, federal, and state requirements as needed. Contractor will wear appropriate personal protective equipment (PPE) throughout work. Contractor will comply with Infection Control requirements to include, but not limited to wearing a surgical mask and/or shield as required.

IV. Bidding Schedule/Period of Performance The Contractor will provide a cost for each individual test.

The Contractor will provide a mobilization fee for each service trip to the VAMC.

The Contractor will provide a material mark-up% over wholesale cost the vendor will charge for materials used in sampling not covered in the sample/test pricing.

The Contractor shall provide a cost breakdown of any other costs associated with testing and sampling.

This contract is a base plus four option year contract.

The Contractor shall provide a base price for a years’ worth testing of services as outlined in Table 1 Water Quality Monitoring. For any additional testing/sampling outside the frequency outlined in Table 1, the Contractor shall submit an estimate of those services to the VA upon request using the pricing submitted in the proposal for this contract.

V. Work Schedule

1. Normal working hours are from 7:30 AM to 4:30 PM.

VI. Privacy Statement Information Security: The C&A requirements do not apply, a Security Accreditation Package is not required. Incidental exposure to protected health information may occur from accessing the work site. Staff will follow all Privacy & Security policies and procedures.
