Please refer to SECTION E – SOLICITATION PROVISIONS, ADDENDUM to FAR 52.212-1 INSTRUCTION TO OFFERORS – COMMERCIAL ITEMS, VOLUME 1 – Price Quote – Submission Requirements, as follows:
The following paragraph is hereby REMOVED in its entirety: “Successful Quoters shall provide proof of Malpractice Liability Insurance for each Cardiologist Locum Tenens who will provide services under this contract prior to award of a contract”; and is REPLACED by the following paragraph: “Successful Quoters shall provide proof of Malpractice Liability Insurance for each Cardiologist Locum Tenens who will provide services under this contract at time of award of a contract.”
CHANGE 3
Please refer to SECTION E, SOLICITATION PROVISIONS, PARAGRAPH E.7, 52.212-2 EVALUATION – COMMERCIAL ITEMS (OCT 2014). PARAGRAPH E.7, 52.212-2 EVALUATION – COMMERCIAL ITEMS (OCT 2014) is hereby REMOVED in its entirety and REPLACED with Paragraph E.7, 52.212-2 EVALUATION – COMMERCIAL ITEMS (OCT 2014), as follows:
“E.7 52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014)
(a) The Government will award a contract resulting from this solicitation to the responsible Quoter whose Quote conforms to the solicitation and will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate Quotes: Price, Technical Expertise and Past Performance. (Because this is a FAR Part 13 procurement, solicitations are not required to state the relative importance assigned to each evaluation factor).
(b) Options. The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that a quote is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of a quote, mailed or otherwise furnished to the successful Quoter within the time for acceptance specified in the quote, shall result in a binding contract without further action by either party. Before the Quoter's specified expiration time, the Government may accept a quote (or part of a quote), whether or not there are clarifications after its receipt, unless a written notice of withdrawal is received before award.
(End of Provision)”
CHANGE 4
The following requirement is added to SECTION E – SOLICITATION PROVISIONS, ADDENDUM to FAR 52.212-1 INSTRUCTION TO OFFERORS – COMMERCIAL ITEMS, VOLUME 1 – Price Quote – Submission Requirements:
