Answer: The 4 represents Quarter.
6. On page 5, it states that this box will be serviced every other week in the quarter, so wouldn’t the 4 be a 6.50 +/- and then the unit price would be the price for the transportation and disposal of the waste per haul?
Answer: Price schedule has been revised to reflect the approximate pulls per quarter.
7. Can the hauler provide a fee for the haul and a fee per ton on the disposal cost, or do these cost need to be combined?
Answer: As requested, one combined charge.
8. For the 35 yard compactor for trash what does the quantity of 4 represent?
Answer: The 4 represents Quarter.
9. On page 5, it states that this box will be serviced +/- 9 times in the quarter, so wouldn’t the 4 be a 9 and then the unit price would be the price for the transportation and disposal of the waste per haul?

14. Why would no extra charge be applied if there were more pulls completed within any quarter? The hauler has no control over the waste being produced at the hospital?
Answer: To save money.
15. On page 6, the first item 5, what if there is no rebates to be paid? The ZS is combined together and there is no way to determine what the amount of tonnage is found in a load by commodity. Do we just state that within the technical submittal?
Answer: Performance Work Statement (PWS) has been revised; please see below.
16. On the first page: I see that blocks 12,17,23,24, and 30 need to be completed. Where can I find the information needed in 17A, and what is meant by 17B?
Answer: This information for 17A is the bidder’s information. It is meant by 17B to put an address if your billing address is different.
17. How should the information be placed in lines 23 and 24? Do we just place the annual amount per year by adding up the 4 services requested for each individual year?
Answer: Please complete B.4, Price/Cost Schedule and not 23 and 24.
18. What do we do with the survey on pages 26 and 27, plus pages 32 and 33? Does the VA complete these forms, or does the VA send these surveys out to various references?
Answer: Please read page 43 which provides instructions. The survey on pages 26 and 27 can be sent to your references for completion or I may send this survey to your references provided for completion. Paage 43, page 32 and 33 is the Quality Assurance Surveillance Plan which is how the awardees services will be evaluated.
