Clauses that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.
The following clauses are incorporated into 52.212-4 as an addendum to this contract:
C.1 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.
(End of Clause)
C.2 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 15 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.
(End of Clause)

In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:
(a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.
(b) General Liability: $500,000.00 per occurrences.

ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS
Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.
The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:
GENERAL INSTRUCTIONS:
In addition to the information provided in FAR Clause 52.212-1 Instructions to Offerors –Commercial Items, the following instructions to vendors apply:
| (a) | All quotes shall be submitted in accordance with the following instructions, which establish the acceptable minimum requirements for the format and content of the quote. |
| (b) | Your quote must stipulate that it is predicated upon all the terms and conditions of this RFQ and signed by an official authorized to bind your organization. In addition, it must contain a statement to the effect that it is firm for a period of at least 60 days from the date of receipt by the Government. |
| (c) | Quote shall be submitted electronically via email to Anthony.Zibolski@va.gov and Wanda.Edwards@va.gov before the date and time that the quote is due. All quotes shall reference 36C10X18Q9130 in the email subject line. |
| (d) | All questions regarding this solicitation shall be submitted via email no later than the date/time specified to Anthony.Zibolski@va.gov and Wanda.Edwards@va.gov. Only written questions will be accepted. It is requested that all questions be combined into one Microsoft Word document. The email subject line shall reference 36C10X18Q9130. |
| (e) | Quotes not submitted directly to Anthony.Zibolski@va.gov and Wanda.Edwards@va.gov, or received after the due date/time stated on the cover page of this solicitation, may not be considered. The email system will not accept file sizes that exceed 5MB. It is the contractor’s responsibility to submit all parts of their quote no later than the date and time specified in the RFQ. |
WARNING: Please do not wait until the last minute to submit your quotes! Late quotes will not be accepted for evaluation. To avoid submission of late quotes, we recommend the transmission of your quote 24 hours prior to the required quote due date and time. Please be advised that timeliness is determined by the date and time an Offeror’s quote is received by the Government not when an Offeror attempted transmission. Offerors are encouraged to review and ensure that sufficient bandwidth is available on their end of the transmission.
(f) Quote Files: The submission shall be clearly indexed and logically assembled. Each volume shall be clearly identified and shall begin at the top of a page. All pages of each volume shall be appropriately numbered and identified by the complete company name, date and solicitation number in the header and/or footer.
All files shall be submitted as a Microsoft Excel (.xls/.xlsx), Microsoft Word (.doc/.docx), or Adobe Acrobat (.pdf) file or compatible as indicated in the table. Page size shall be no greater than 8 1/2" x 11". The top, bottom, left and right margins shall be a minimum of one inch each. Font size shall be 12-point. Times New Roman font is required. Characters shall be set at no less than normal spacing and 100% scale. Tables and illustrations may use a reduced font size not less than 8-point and may be landscape. Line spacing shall be set at no less than single space. Each paragraph shall be separated by at least one blank line. Page numbers, company logos, and headers and footers may be within the page margins ONLY, and are not bound by the 12-point font requirement. Footnotes to text shall not be used.
| (g) | File Packaging: Do not compress (.zip) quote files. VA Network Security Operations Center (NSOC) has temporarily blocked email attachments with the “.zip” extension as a mitigation measure against the ongoing world-wide ransomware event impacting many organizations. During this time, .zip file extensions will be permanently stripped from email traffic, and will not be recoverable. Due to VA email file size restrictions, vendors are encouraged to logically separate their quote into separate emails. If this is necessary, vendors should attempt to contain complete volume within single email transmissions (and not split volumes). Vendors are encouraged to review and ensure that sufficient bandwidth is available on their end of the transmission. |
| (h) | Content Requirements: All information shall be confined to the appropriate file. The offeror shall confine submissions to essential matters, sufficient to define the quote and provide an adequate basis for evaluation. Vendors are responsible for including sufficient details, in a concise manner, to permit a complete and accurate evaluation of each quote. |
| (i) | Quotes submitted in response of this solicitation shall contain the following in separate volumes: |
| Volume Number | |
| Evaluation Factor | |
| File Name | |
| Page Limitations |
| Volume I |
| Technical Capability |
| [Contractor Name] |
Tech.doc/pdf 20: there is no limit on résumés, qualifications or certifications
| Volume II |
| Price |
| [Contractor Name] |

| (j) | A Cover Page, Table of Contents and/or a glossary of abbreviations or acronyms will not be included in the page count of the technical Volume. However, be advised that any and all information contained within any Table of Contents and/or glossary of abbreviations or acronyms submitted with an Offeror’s quote may not be evaluated by the Government. |
| (k) | Each volume shall be clearly identified and shall begin at the top of a page. All pages of each volume shall be appropriately numbered and identified by the complete company name, date and solicitation number in the header and/or footer |
| (l) | Attachments are required to be in PDF (unencrypted) or MS Word formats only. |
| (m) | The use of hyperlinks in quotes is prohibited. Any quote that fails to comply with all solicitation instructions may be considered non-responsive and may not be accepted or considered for award. All information shall be confined to the appropriate volume. Vendors are responsible for including sufficient details, in a concise manner, to permit a complete and accurate evaluation of each quote. |
VOLUME I –TECHNICAL CAPABILITY
The technical volume shall demonstrate the ability to meet the requirements in the solicitation, to include the following:
| (a) | The vendor shall demonstrate successful, recent (within the past three years), and relevant (i.e. experience in providing services similar in size, scope, and complexity) as described in the Performance Work Statement (PWS).
