X X X X Title: Genomic Sequencing Testing Servicing

The purpose of this amendment is to revise an answer provided in Amendment 0002; revise Section B.5; revise Section E.1;

and revise Section E.3.

See Continuation Pages and Attachment 1 to this amendment for details.

All other terms and conditions remain unchanged.

CONTINUATION PAGES

The purpose of this amendment is to: 1) revise an answer provided in Amendment 0002 Attachment 1, “RFP Questions and Answers”; 2) revise Section B.5; 3) revise Section E.1; and 4) revise Section E.3. (Changes highlighted in yellow).

Accordingly,

1) The response provided to Amendment 0002 Attachment 1, “RFP Questions and Answers” question 11b. is hereby revised as follows:

b. If the offeror is not currently able to provide a service listed on the CLIN table, will the RFP be considered non-responsive?

As stated on p.55 in RFP provision E.1, “Addendum to FAR 52.212-1, Instructions to Offerors – Commercial Items, section B(2)(d)(i): “Importantly, although the Price/Cost Schedule reflects all of VA’s current needs for genomic sequencing test services, Offerors are advised that as new and emerging technology becomes available, VA intends to add additional genomic sequencing testing to this contract by modification, if mutually agreeable to the parties. The PWS should reflect an Offeror’s ability to expand the testing services available.”

Accordingly, offerors are not required to currently provide all the services requested in the CLIN table of the RFP to receive consideration, but those that demonstrate the ability to expand testing services to include all CLINs may receive a higher evaluation rating.

Yes, Offerors who do not provide all the services listed in B.5, Price/Cost Schedule, will be considered non-responsive to the RFP.

2) See Attachment 1, “Revised B.5”.

3) E.1, Addendum to FAR 52-212-1 Instructions to Offerors – Commercial Items, Section B(2)(d)(i)(a), Factor 1: Performance Work Statement (PWS) is hereby deleted in its entirety and substituted with the following:

Factor 1: Performance Work Statement (PWS)

Offerors shall provide a Performance Work Statement (PWS) in response to the Statement of Objectives and this RFP.

The PWS shall clearly illustrate the Offeror’s ability to provide Solid Tumor Next-Generation Sequencing (NGS), Hematologic Tumor (Liquid biopsy for Solid Tumor and Hematologic malignancy NGS panels), and Complete Panels (Whole Exome and Whole Transcriptome) mutational burden for genomic biomarker testing services to VA Clinicians and Hospitals nationwide.

Importantly, although the Price/Cost Schedule reflects all of VA’s current needs for genomic sequencing test services, Offerors are advised that as new and emerging technology becomes available, VA intends to add additional genomic sequencing testing to this contract by modification, including but not limited to Whole Exome and Whole Transcriptome testing, if mutually agreeable to the parties. The PWS should reflect an Offeror’s ability to expand the testing services available, and detail an Offeror’s strategy to anticipate and incorporate new technologies as they become available to the marketplace.

Also, the needs for individual tests are Veteran-specific. Participating Medical Centers can be found at Attachment A – Participating Medical Centers; there are currently approximately VHA 80 locations that provide oncology services that perform Genomic Sequencing tests. As additional Medical Centers become available for Genomic Sequencing they may be added without additional costs to the contract. Therefore, the PWS shall clearly specify which state(s) the Offeror services and demonstrate capacity for growth.

Additionally, the PWS should provide a detailed process for management of the overall contract, including working with the VA Program Manager, COR, and Medical Centers to collect, ship, prioritize, and report testing results.

The Offeror shall demonstrate in its PWS how the applications, databases, and other products it will produce will meet all requirements for compliance with Section 508 and VA’s IT Security Requirements.

4) E.3, Addendum to FAR 52.212-2 Evaluation – Commercial Items, section A, “Basis for Award” is hereby deleted in its entirety and substituted with the following:

E.3 ADDENDUM to FAR 52.212-2 EVALUATION – COMMERICAL ITEMS

A. BASIS FOR AWARD

The Department of Veterans Affairs (VA) Strategic Acquisition Center is issuing this Request for Proposal (RFP) to enter into a single-award Indefinite Delivery Indefinite Quantity (IDIQ) contract. The maximum aggregate dollar value for all task orders awarded via this five-year single-award IDIQ is $TBD and the minimum dollar guarantee for the awardee is $5,000.00. Offeror who fail to meet all the terms of the solicitation will be ineligible for award.

The award resulting from this solicitation will be made based on the best overall proposal that is determined to be the most beneficial to the Government (i.e., best value), price and non-price factors considered. FAR 2.101 defines best value as the “expected outcome of an acquisition that, in the Government’s estimation, provides the greatest overall benefit in response to the requirement.”

Offerors must provide a quote for every Contract Line Item Number (CLIN) listed in B.5, Price/Cost Schedule in order to be considered for award of the IDIQ.

Non-Price Factors (listed in descending order of importance)

1. Technical Approach

a. Sub-factor: Performance Work Statement (PWS)

b. Sub-factor: Quality Assurance Surveillance Plan (QASP)

2. Socio-economic Considerations; and

Price Factors

3. Price.

Non-price factors when combined are significantly more important than price. The following non-price factors are listed in descending order of importance: Technical Approach and Socio-economic Considerations. The Technical Approach sub-factors: Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP) are listed in descending order of importance. It should be noted that award might be made to other than the lowest priced offeror if the Government determines that a price premium is warranted due to merits of one or more of the non-price factors.

Proposals will be evaluated using a phased process, as follows:

Phase I: The Technical Evaluation Team (TET) will review ONLY the Technical Evaluation Matrix provided by offerors to determine whether each offeror meets the minimum technical requirements as specified in the matrix. After review and concurrence by the Source Selection Authority (SSA)/Contracting Officer (CO), those Offerors who do not meet the minimum requirements will be notified of such and will not be evaluated further.

Phase II: Proposals will be evaluated in their entirety from those offerors who advanced from Phase I. After review of all factors, the SSA/CO will determine the apparent awardees or set a competitive range of the most highly rated proposals, which may be further reduced for efficiency in accordance with FAR 15.306(c)(2), if applicable.
