PRE-PREPOSAL CONFERENCE
Pedestrian Sky Bridge Slope Modification, VAMC, San Juan, PR

REVIEW OF BID ITEMS:
SCOPE OF WORK: The contractor shall provide all labor, materials, equipment, transportation, supervision, supplies, and tools necessary to completely prepare site for building operations, including demolition and removal of existing finishes, and furnish labor and materials and perform work for changing the ramp slope of the pedestrian bridge connecting the parking garage to the hospital structure as required by drawings and specifications.
A. CLIN #001 - BASE BID GENERAL CONSTRUCTION: Work includes general construction including removing the existing flooring, preparing the concrete for the cementitious underlayment fill, installation of new handrails and application of new flooring and finish materials.
INSURANCE: Prior to performance - contractor must provide evidence of general liability insurance.
DAVIS BACON ACT APPLIES. Not applicable
BID BOND: In the amount not less than 20 percent of the bid price.
COST RANGE: $100,000-$250,000
COMPLETION TIME: 90 calendar days from date of notice to proceed.
AMENDMENT: If questions warrant a clarification or change, an amendment will be issued. Amendments to the solicitation will be posted at http:www.fedbizopps.gov/. Paper copies of the amendments will NOT be individually mailed.
SET ASIDE: Service Disabled Veteran-Owned Business Set-Aside.
BIDDING MATERIALS: Bidding materials may be obtained only upon written application to the issuing office. Bidders should allow 5 working days after receipt of their request by the issuing office for reproduction, in addition to mail delivery time when requesting bidding material. All requests shall be on company letterhead to include point of contact, physical mailing address, phone number, fax number, e-mail address.
No telephone requests will be accepted. Bidding materials are not available at FedBizOpps website.
BID SUBMISSION:
SUBMISSION OF OFFER
General
The offeror’s proposal shall be submitted as set forth below:
The Offeror’s proposal shall consist of (2) separate volumes, Volume I – Factors l through 5 and Volume II – Price and certifications. The Offeror’s proposal shall be submitted and received by the Contracting Officer by the closing date and time as specified in section B2 of this solicitation. The Submission address is as follows:

CAUTION STATEMENT: Hand carried offers are not permissible for the requirement. Offerors are advised that commercial carrier deliveries will be delivered between 7:30 a.m. and 4:30 p.m., Monday through Friday, except for legal federal holidays as set forth in 5 USC 6103. Offerors must ensure that their commercial carrier service provide delivery prior to the scheduled opening/closing time. Packages must be plainly marked ON THE OUTSIDE OF THE COMMERCIAL CARRIER’S ENVELOPE with the solicitation number, date, and time set forth for receipt of offers.
Submission Requirements for Volume 1
Volume 1 – Factors 1 and 5 – The Offeror shall submit one (1) sealed original offer and five (5) copies with a cover sheet displaying the RFP number, contractor name, and an index of the following five (5) factors contained within the volume.

Submission Requirements for Volume 2
Volume 2 – Factor 6: Price –Shall consist of a cover sheet displaying the RFP number, contractor name, and an index of the following sections in the volume.
Tab 1: The completed and signed SF-1442, All blocks under section “OFFER” on page 2 of SF1442 (Item 14 thru 20C) must be completed. Offerors must also complete the Price Schedule on page 5.

BID DUE DATE: The Due date for submission of offers is April 18, 2019 by 2:00 P.M, EST. Late proposals shall not be considered. It is the responsibility of the Offeror to ensure their entire proposal is received, on time and in accordance with these instructions.
PROJECT OVERVIEW: Jeff Carrington, VA Project Manager.
SITE VISIT: Immediately following this meeting or upon request to Carlos Montenegro, VA Senior Resident Engineer.
QUESTIONS: No requests for clarifications or questions concerning this solicitation will be provided in response to telephone calls. Formal communications such as requests for information and/or information or questions concerning this solicitation should be submitted via email. All requests for information and/or information or questions should be submitted no later than 2:00 P.M. EST, April 5, 2019 to: derek.underwood2@va.gov.
