REQUEST FOR QUOTE FOR DEMOLITION AND INSTALLATION OF CONCRETE PATIO/SIDEWALK AREAS, BOISE VA REGIONAL OFFICE

1. General Scope of Work Provide all labor, materials, tools, equipment, engineering, supervision, and services to remove and replace the patio pad and adjacent sidewalks at the VA Regional Office located at 444 W Fort St, Boise, ID 83702.

1.1 Background

This requirement is to demolish remove and reinstall a concrete patio and sidewalks at the Boise VA Regional Office.

1.2 Objectives

a. Demolition of approximately 1,280 sq. ft. of concrete patio and sidewalks and aggregate base.

b. Installation of approximately 1,280 sq. ft. of concrete patio and sidewalks and aggregate base.

c. Remove and re-install six patio tables and one patio bench.

1.3 Schedule:

The following is the Government’s anticipated project schedule, subject to approval of the Contractor’s final project schedule:

· Notice to Proceed: Day 1

· Safety/Environmental/Schedule Submittals: 5 Calendar Days After Notice to Proceed

· Material Submittals:

10 Calendar Days After Notice to Proceed

· Project Completion Prior To Final Inspection: 30 Calendar Days After Notice to Proceed

· Overall Project Period: 35 Calendar Days After Notice to Proceed

1.3 Personnel

The Government shall neither supervise Contractor employees nor control the method by which the Contractor performs the required tasks. Under no circumstances shall the Government assign tasks to, or prepare work schedules for, individual Contractor employees. It shall be the responsibility of the Contractor to manage its employees and to guard against any actions that are of the nature of personal services or give the perception of personal services. If the Contractor believes that any actions constitute, or are perceived to constitute personal services, it shall be the Contractor's responsibility to notify the Contracting Officer (CO) immediately. The Contractor will provide delivery and installation of the concrete and associated installation requirements. The selected Contractor will provide all necessary material handling equipment to complete the installation.

2. Performance Requirements

The following section specifies the Performance Objectives and Performance Elements for the contract.

2.1 Phase I – Pre-Construction Phase

Contractor will perform all required training. See below for specific training requirements.

2.1.1 Completion of 30-hour OSHA Safety Course

Site foreman is required to have 30hr OSHA training course and provide COR copy of certificate.

Performance Standards

Standard: Compliant with federal, state, and local laws and regulations.

Acceptable Quality Limit (AQL):

Onsite foreman possesses valid OSHA 30 training.

2.1.2 Completion of 10-hour OSHA Safety Course

All employees of general Contractor or subcontractors shall have the 10-hour OSHA certified Construction Safety course and/or other relevant competency training.

Performance Standards

Standard: Compliant with federal, state, and local laws and regulations.

Acceptable Quality Limit (AQL):

Employees have completed OSHA training.

2.2 Phase II - Construction Phase

Contractor will perform all construction phase activities. See below for activities and requirements.

Prior to commencing work, general Contractor shall provide proof that an OSHA certified “competent person” (CP) (29 CFR 1926.20(b)(2) will maintain a presence at the work site whenever the general or subcontractors are present.

2.2.1 Material Removal and Disposal

The Contractor shall remove the six patio tables and bench. The Contractor shall remove the existing concrete and dispose of it in accordance with industry standards. All demolition actions that generate excessive noise (jackhammering, etc.) must be completed between 6:00pm and 6:00am. All materials that have been removed must be hauled from the worksite the same day as it is removed. The Contractor shall recycle or divert wastes from landfill disposal to the maximum extent practicable. The Contractor shall, before commencement of any work, carefully survey the existing site to determine the extent of the work. The Contractor shall be responsible for the proper disposal of all materials meeting all rules pertaining to disposal. Rubbish and debris shall be removed from Government property at the completion of the project. At the end of each work shift the area must be clean and free of any tripping hazards.

Performance Standards

Standard: Compliant with federal, state, and local laws and regulations.

Acceptable Quality Limit (AQL):

Waste materials are removed from worksite and disposed of the same day it is demolished.

Deliverables

100% compliance as verified by COR.

2.2.2 Environmental Controls

The amount of dust resulting from the project shall be controlled to prevent the spread of dust to occupied portions of the building and to avoid creation of a nuisance in the surrounding area. The Contractor shall repair any damage to facilities, landscaping, curbs, sidewalks, etc., including those that are the property of a third party, resulting from failure to comply with the requirements of this Contract or failure to exercise reasonable care in performing the work. At Contractor's own expense, Contractor shall immediately restore to service and repair any damage caused by Contractor's workmen to piping and conduits, wires, cables, etc., of utility services or of fire protection systems and communications systems (including telephone) which are not scheduled for discontinuance or abandonment. Repairs must be accepted by the Government. If the Contractor fails or refuses to repair the damage promptly, the Contracting Officer may have the necessary work performed and charge the cost to the Contractor. The Contractor shall fully restore the contract work area site to its original condition. The restoration shall include cleaning and maintenance as required to return site to its original condition as determined by the COR and CO.

Performance Standards

Isolate area of installation from rest of building to the maximum extent practicable.

Acceptable Quality Limit (AQL):

Excessive dust does not enter facility.

Deliverables

OSHA standard verified by COR.

2.2.3 Concrete Installation

Concrete shall have a compressive strength of at least 4,000 psi. Concrete must contain non-reactive aggregate and conform to ASTM C33, specifically ASTM C1778. The concrete work shall match or blend with the surrounding concrete. Delivery and installation shall occur within 30 days of receipt of Notice to Proceed (NTP) Letter. Work area must be protected from unauthorized entry (when wet) with appropriate barriers and signage. The Contractor shall provide all equipment, personnel, and supervision necessary to remove and install the concrete patio and sidewalks. Contractor shall provide adequate facilities (portable toilets, water, etc.) for employees as building access is restricted. The Contractor shall reinstall the patio tables and bench.

Performance Standards

Complete concrete work as necessary.

Acceptable Quality Limits

100% of concrete must be prepared properly and in accordance with manufacturer instructions.

Deliverables A003 Provide copy of concreter requirements to the COR prior to installation.

2.3 Phase III - Post Construction Phase

Contractor will perform all required post-construction activities. See below for requirements.

2.3.1 Completion of Work

Contractor shall remove all construction material (boxes, banding scrap, plastic, paper, etc.) and debris (scrap pieces of concrete, etc.) from the work area. This will be completed daily. The Contractor shall make any necessary arrangements with private parties and with county officials pertinent to locations and regulations of such disposal. Any fees or charges to be paid for disposal of materials shall be paid by the Contractor.

Performance Standards All construction material and debris removed from the work area.

Acceptable Quality Limits

100% of area clean as verified by COR.

2.4 Phase IV - Final Inspection

Prior to the final inspection for a given Project or phase, the Contractor is required to have all work required by the Contract for the Project or phase completed. During the final inspection, a punch list of correction items will be generated. It typically requires 2-3 days following the final inspection before a punch list is available. Final payment will not be made until all punch list items are properly corrected.

3. Special Requirements

This section describes the special requirements for this effort. The following sub-sections provide details of various considerations on this effort.

3.1 Security

The General Contractor is responsible for assuring that all subcontractors working on the project and their employees also comply with the standards outlined here. All general Contractor’s employees, subcontractors, and material suppliers entering the VBA property consent to search by VA Police at any time. The areas of search shall be all encompassing including but not limited to: their person; their personal effects; personal vehicles, as well as all Contractor vehicles, equipment, trailers, offices, storage shed, tool boxes and all containers. Weapons are not permitted on VA property. If work requires the Contractor or its employees to have entry into the Regional Office, they must be escorted at all times by the COR or a designated Government representative.

No photography of VA premises is allowed without written permission of the Contracting Officer.

3.2 Safety

Equipment - The Contractor shall provide all safety equipment/devices, SDS, personal protective equipment and clothing as required for its employees. Copies of all SDS shall be provided to the COR.

3.2.1 The Contractor as well as the on-site supervisor will have completed no less than 30 hours of OSHA approved safety training.

Safety Plan Contractor will provide a site-specific safety plan as well as an Activity Hazards Analysis. An example of a VA approved AHA can be found at http://www.usace.army.mil/Portals/2/docs/AHA2.pdf

3.2.2 Compliance - The Contractor shall comply with all applicable Federal, State and local legal requirements regarding workers health and safety. The requirements include but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR) Parts 1910 and 1926. Contractor is solely responsible for determining the legal requirements that apply to activities, and shall ensure safe and healthful working conditions for its employees.

3.3 Mishaps

Mishap Notification and Investigation: The Contractor and its subcontractors (if applicable) shall promptly report pertinent facts regarding mishaps involving Government property damage or injury to Government personnel and to cooperate in any resulting safety investigation. The Contractor shall notify (via email) the Contracting Officer, the Contracting Officer’s Representative, and/or other applicable members within four (4) hours of all mishaps or incidents. The Government person notified by the Contractor will in turn notify the Government safety officer. Contractor notifications made after duty hours shall be reported to the COR. If requested by the contracting officer, the contracting officer’s representative, and/or the program manager, the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records until released by the investigating safety office. If the Government investigates the mishap, the Contractor and the subcontractors shall cooperate fully and assist the Government personnel until the investigation is completed.

3.4 Government Furnished Materials

The government shall provide, at no cost, parking space and access to the VARO loading docks.

3.5 Environmental Requirements

Non-hazardous waste disposal

Demolition - The Construction Contractor shall provide all demolition, removal and legal disposal of materials. The Construction Contractor shall ensure that facilities used for recycling, reuse and disposal shall be permitted for the intended use to the extent required by local, state, federal regulations.

3.5.1 Disposal vendors - The Whole Building Design Guide website http://www.cwm.wbdg.org provides a Construction Waste Management Database that contains information on companies that haul, collect, and process recyclable debris from construction projects.

3.6 Applicable Directives

The Contractor shall comply with all documents listed below as mandatory and referenced under paragraph 2.0, Performance Requirements. Compliance with documents listed as non-mandatory is the Contractor's option.

3.6.1 Mandatory compliance (list)

4. Quality

This section describes the Quality Control components for this effort. The following sub-sections provide details of various considerations on this effort.

4.1 Quality Control

The Contractor shall develop a Quality Control Plan (QCP) and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The Contractor’s QCP is the means by which the Contractor ensures their work complies with the requirement of the contract.

5 Deliverables

The Contractor shall provide deliverables as described below. Deliverables shall be specified by the government. Format and delivery schedule for deliverables are outlined in this PWS.
