About this file

This document is a Justification for an Exception to Fair Opportunity for a Department of Veterans Affairs (VA) contract to acquire a Visage PACS Radiology Workflow Solution through the NASA Solutions for Enterprise-Wide Procurement (SEWP) V Government-Wide Acquisition Contract. The VA's VISN 23 Healthcare Technology Management requires expanding its existing Visage Radiology Picture Archiving and Communication System (PACS) Viewer to include a Radiology PACS Worklist and Workflow Engine, with migration to the VA Enterprise Cloud. The solution will enable radiologists to efficiently route medical imaging studies to specialized worklists by mining HL7 and DICOM data, eliminating the need for multiple applications and improving workflow efficiency.

The procurement will be a firm-fixed price delivery order with Blue Tech, Inc. as the sole authorized reseller, solicited as a Women-Owned Small Business (WOSB) Sole Source requirement. The total contract will include installation and implementation of the Radiology PACS Worklist and Workflow Engine within 120 calendar days of award, followed by 12 months of licensing and maintenance with two 12-month option years. Market research conducted between January 2024 and January 2025 determined that alternative products from Optum Radiology Imaging and Philips Enterprise Imaging cannot meet the VA's requirements without significant additional costs and implementation challenges.

Visage PACS Radiology Workflow Solution Control Number

JUSTIFICATION FOR AN EXCEPTION TO FAIR OPPORTUNITY

1. Contracting Activity: Department of Veterans Affairs (VA) Office of Procurement, Acquisition, and Logistics Technology Acquisition Center 23 Christopher Way Eatontown, NJ 07724

2. Description of Action: This proposed action is for a firm-fixed price delivery order issued under the National Aeronautics and Space Administration (NASA) Solutions for Enterprise-Wide Procurement (SEWP) V Government-Wide Acquisition Contract (GWAC) for a VISN 23 Visage Enterprise Radiologist Worklist and Workflow Engine (herein referred to as the radiology workflow solution).

3. Description of the Supplies or Services: The Department of Veterans Affairs (VA), VISN 23 Healthcare Technology Management (HTM) has a requirement to expand its existing Visage Radiology Picture Archiving and Communication System (PACS) Viewer to include a Radiology PACS Worklist and Workflow Engine, and to migrate the system to the VA Enterprise Cloud. VISN 23 needs to deploy Platform for Workflow Orchestration of the V7 Viewer for radiology medical imaging at VISN 23 locations.

Visage Workflow is the only workflow engine that can enhance efficiency by mining HL7 and DICOM data to discern the study purpose and subsequently auto-route studies to designated sub-specialty worklists. The Platform (including the Workflow component), together with other Visage products will provide the diagnostic imaging environment.

Upon completion of the installation, testing, and training, VISN 23 will have a VA Enterprise Cloud-based radiology workflow solution to function seamlessly with the existing Visage PACS viewer and Visage PACS Storage. The total cost for this acquisition is including all option years, and the period of performance will include an installation and implementation of the Radiology PACS Worklist and Workflow Engine 120 calendar days after award and will be followed by 12 months of licensing and maintenance with two 12-month option years from the date of award.

4. Statutory Authority: The statutory authority permitting an exception to fair opportunity is Section 41 U.S.C. 4106(c)(2) as implemented by the Federal Acquisition Regulation (FAR) 16.505(b)(2)(i)(B), entitled “Only one awardee is capable of providing the supplies or services required at the level of quality required because the supplies or services ordered are unique or highly specialized.”

5. Rationale Supporting Use of Authority Cited Above: VISN 23 is currently using a 3rd party worklist manager, causing the Radiologists to have to have another application open on their desktop, causing more RAM/CPU usage on the workstations, and necessitating the VA to have another set of servers to run the application. To ensure the successful implementation of the Electronic Health Record Modernization (EHRM) initiative and meet the rigorous scrutiny of interfaces with Oracle Health, it is imperative to streamline our PACS system. Currently, VISN 23’s PACS is comprised of disparate systems from various vendors, significantly reducing the likelihood of approval for all interfaces. Therefore, adopting a single-vendor PACS system is crucial to enhance

Visage PACS Radiology Workflow Solution CONTROL NUMBER compatibility, reliability, and integration with Oracle Health. Visage Workflow is a software that is native in the current Visage viewer that the Radiologists in VISN 23 currently use and doesn’t have to be launched from another application. Visage Workflow is the only product that could accomplish this all within one singular application. If any other vendor is selected, VISN 23 would have to also replace our viewing and archive portions, at a cost estimated to be , plus additional implementation lead time and effort, which have taken up to one year in the past to completely replace all components of the PACS system. This would also draw resources away from normal operations and preparations for Oracle Health transition.

The cost information for replacing the entire PACS system was based on previous acquisitions of similar size and scope.

Additionally, VISN 23 Radiology PACS Administrators currently configure imaging studies based on modality, anatomical region, and examination purpose. This structure enables Radiologists, who possess sub-specialties, to efficiently identify relevant studies for interpretation. However, recent VA policy mandates standardized naming conventions for all imaging studies, excluding the examination purpose. This reduction in specificity necessitates Radiologists to open each study manually to ascertain its relevance, significantly hindering workflow efficiency. Visage Workflow is able to determine the study purpose and create separate worklists, without this information being in the naming convention.

Visage Workflow is the only workflow engine that can enhance efficiency by mining HL7 and DICOM data to discern the study purpose and subsequently auto-route studies to designated sub-specialty worklists. This automated routing mechanism is crucial in reducing the amount of time radiologists have to spend searching for the studies they need to read.

6. Efforts to Obtain Competition: Market research was conducted, details of which are in the market research section of this document. This effort did not yield any additional sources that can meet the Government’s requirements. There is no competition anticipated for this acquisition. In accordance with FAR 5.301 and 16.505(b)(2)(ii)(D), the award notice for this action will be synopsized on the Contract Opportunities Page and the justification will be made publicly available within 14 days of award.

7. Actions to Increase Competition: The Government will continue to conduct market research to ascertain if there are changes in the marketplace that would enable future actions to be competed.

8. Market Research: The VISN 23 PACS Administrators conducted market research by reviewing other similar software. This market research was conducted between January 2024 and January 2025. The other similar software reviewed included Optum Radiology Imaging and Philips Enterprise Imaging. Based on reviews of these products, the VISN 23 PACS Administrators determined that none of these products can meet the Government’s requirements with VISN 23 current infrastructure without the significant duplication of time and costs detailed above. Additionally, none of the other systems can perform the HL7 and DICOM data mining and auto-routing to sub-specialty worklists.

Market research was performed by the Contract Specialist (CS) in May 2025 through July 2025 utilizing the Provider Lookup Tools on the NASA SEWP V GWAC website, which identified one HUBZone and Woman-Owned Small Business (WOSB) Value Added Reseller GWAC holder that can fulfill the requirement, Blue Tech, Inc. Additional market research was completed and the CS contacted the Original Equipment Manufacturer (OEM), Visage Imaging, asking if there were any other authorized resellers for the Visage products and the OEM confirmed that Blue Tech, Inc. was the only authorized reseller. As a result, this requirement will be solicited as a WOSB Sole Source requirement via NASA SEWP V GWAC website.
