OVERVIEW. This is a master delivery schedule template. This template will be populated for all procurements. The template serves as a guide and may be modified by adding columns that correspond to the items being procured and delivered. Sample data has been provided for clarity, remove the sample data once actual data is available at each stage of the procurement.
VA PROCUREMENT SUMMARY WORKSHEET. The VA Program or Project Manager shall provide the initial summary of what has been ordered, creating one row per VA Facility and one column per Contract Line Item Number (CLIN). Vendors shall modify the columns to describe the major end items being procured/delivered. This worksheet will be based off the VA Purchase Order and will include base options in addition to any optional CLINs. The intent is to provide each VA Facility receiving equipment with the number of items for each CLIN that will be physically delivered.

Columns describing internal components of the equipment delivered are specifically not wanted - this adds unnecessary confusion and will delay the receiving/payment process. To better describe the boxes included in each shipment, Vendor's may elect to include columns for component parts shipped in separate boxes and any additional columns needed in order to provide a complete Bill of Materials (BOM).

VA ADDRESSES WORSHEET. This worksheet will be initially populated by the VA. All columns will be completed. Contractors shall coordinate deliveries with Site POCs before shipment of all shipments to ensure sites have adequate storage space. Prior to shipping, Contractor (prime) shall notify the Delivery Date Coordinator and the respective Site POCs, by phone followed by email, of all incoming deliveries including line-by-line details for review of requirements. VA Project Manager, Implementation Manager, and COR shall be copied on all email communication to sites. Contractor cannot make any changes to the delivery quantities or schedule at the request of Site POC other than the ability of the site to ask that their shipments be broken up into no more than three partial deliveries.

ORDER TRACKING WORKSHEET. This worksheet is for vendor use and will contain columns for all items being procured. Each cell shall be populated with the total number of items being delivered to each site. This worksheet will replicate the BOM and provide proof of delivery

SERIAL # WORKSHEET. The contractor will maintain an electronic list of all delivered equipment by serial number, noting delivery location, Purchase Order number and individual that signs for the equipment. Delivery location will include the Region number, VISN number, station number, street address, city and state. The electronic list will be provided in a file with one serial number per line in a standard format such as excel, or quote and comma delimited format. Contractor will provide the list via electronic mail after receipt of the equipment by the site. The list will be sent to the site POC and additional VA staff identified at the time the order is placed.
EPEAT. CONTRACTING FOR ENVIRONMENTALLY PREFERABLE PRODUCTS AND SERVICES. Requirements are the same for all vendors/suppliers to ensure products are in compliance with each contract method used for award. Scope, Definitions, Authorities, Policy, EPEAT, and Contract Clauses for compliance are identified in the FAR and Federal Register.
