THE FOLLOWING STATEMENT IS IN COMPLIANCE WITH THE UNITED NATIONS-IMPLEMENTED RESTRICTIONS ON WOOD

PACKAGING MATERIAL (WPM). THIS REQUIREMENT IS SPECIFICALLY CONCERNED WITH INVASIVE SPECIES SUCH AS THE

PINEWOOD NEMATODE. WPM IS DEFINED AS WOOD PALLETS, SKIDS, LOAD BOARDS, PALLET COLLARS, WOODEN BOXES, REELS, DUNNAGE, CRATES, FRAMES, AND CLEATS. THE STATEMENT BELOW CITES THE REQUIREMENTS IN DETAIL AND

COVERS LUMBER FOR BUILDING CONTAINERS AND PALLETS USED FOR DELIVERIES, RESULTING IN SHIPMENTS WORLDWIDE

PRESERVATION, PACKAGING & MARKING REQUIREMENTS

The contractor shall develop, implement and maintain the preservation, packaging and packing of all items to be delivered under the terms of this contract. Packaging procedures will be established in accordance with AFMCI 24-201, specifically Paragraph 3.7 and MIL-STD-2073-1, Standard Practice for Military Packaging, Packaging requirements are identified in the AFMC Form 158, Packaging Requirements submitted by the responsible packaging specialist. All “markings and labeling” of shipments shall conform to MIL-STD-129, DoD Standard Practice, Military Marking for Shipment and Storage. The contractor shall affix a completed DD Form 1574, Serviceable Tag-Materiel or DD Form 1574-1, Serviceable Label- Materiel to each unit pack or intermediate container and prime outer container(s). The tag or label will indicate the condition(s) of the materiel and to identify the individual article or contents, bundle, container to which they are securely attached.

INTERNATIONAL STANDARDS FOR PHYTOSANITARY MEASURES

(ISPM 15) REQUIREMENTS

The contractor shall use the wood packaging material regulation for international trade which describes Phytosanitary (including treatments) that have been approved for wood packaging material and provides for the approval of new or revised treatments. Use the latest revision of (ISPM 15), and the following statement complies with the United Nations-implemented restrictions on wood packaging material (WPM).

“This requirement is specifically concerned with invasive species such as pinewood nematode. WPM is defined as wood pallets, skids, load boards, pallet collars, wooden boxes, reels, dunnage, crates, frames, and cleats. The statement below cites the requirements in detail and covers lumber for building containers and pallets used for deliveries resulting in worldwide shipments (direct vendor deliveries, Air Logistics complex (ALC), Container Consolidation Points (CCP), Aerial Ports, Water Ports, or other DoD installations).

ELECTROSTATIC MATERIAL (When Applicable) The Contractor shall apply provisions of MIL-STD-2073-1, when identifying all solid-state devices and modules containing diodes, translators, integrated circuits and/or other microelectronic components sensitive to electrostatic forces. All such items shall be packaged IAW MIL-STD-2073-1 in conjunction with ANS/ESD S20.20, using electrostatic free barrier materials and shall clearly bear “Electrostatic Sensitive Device” labels IAW MIL-STD-129, Department of Defense Standard Practice Military Marking for Shipment and Storage.

PALLETIZED UNIT LOADS

The Contractor shall apply provisions and methods of MIL-STD-147, Palletized Unit Loads to contracts requiring the preparation and shipment of bonded palletized unit loads for DoD facilities when applicable.

SPECIALIZED SHIPPING CONTAINERS

The contractor shall apply provisions of MIL-STD-648, Department of Defense Design Criteria Standard, Specialized Shipping Containers. This standard establishes general design guidelines and associated tests for specialized shipping containers used by the Department of Defense. Definitive requirements for specific containers will be defined by the individual specification, acquisition, or task order. This standard is intended to be used as the basic reference document in all specifications and standards prescribing performance requirements to be applied to a specialized shipping container. Compliance with this intent is expected through normal application of the specification or standard preparation and revision processes.

HAZARDOUS MATERIALS (When Applicable) The contractor shall ensure Hazardous shipments comply with International Civil Aviation Organization (ICAO), U.S. Code of Federal Regulations (CFR) Title 49, International Air Transport Association (IATA) Dangerous Goods Regulation, International Maritime Dangerous Goods Code (IMDG, Annex 1), and AFMAN 24-604, Preparing Hazardous Materials for Military Air Shipments. Packaging shall meet “DOT specification or a United Nation (UN) specification ” manufactured in accordance with 49 CFR, Part 178 and the final packages assembled IAW 49 CFR, Parts 100-185 as applicable. A Competent Authority Approval (CAA) shall be acquired from the responsible Associate Administrator or other designated Department official to perform a function requiring prior consent under the Hazardous Material Regulation (HMR). Package(s) shall be marked IAW MIL-STD-129, DoD Standard Practice, Military Marking for Shipment and Storage.

The contractor shall affix a completed DD Form 1574, Serviceable Tag-Materiel or DD Form 1574-1, Serviceable Label- Materiel to each unit pack or intermediate container and prime outer container(s). The tag or label will indicate the condition(s) of the materiel and to identify the individual article or contents, bundle, container to which they are securely attached. If a symbol is used to represent the manufacturer or the approval agency certifying compliance with 49 CFR 178, subparts L and M, the symbol must be registered with the U.S. Department of Transportation. A copy of the Contractor’s Test Report and signed certification that the package configuration meets both CFR and UN requirements shall be made available to the procuring contract office, as required, prior to acceptance or shipment of any portion of the product IAW Contract Data Requirements List (CDRL DI-PACK-81059B) (Reference AFMC Form 158). Safety Data Sheets (SDS) are required for known hazardous items and applicable items shown in FED-STD-313, Tables I and II. The SDS shall have any applicable identification number, such as National Stock Number or Special Item Number. A copy of the SDS shall be mailed or e-mailed no later than five (5) days prior to contractual delivery of items:

REUSABLE CONTAINERS

The contractor shall safeguard all received reusable shipping containers that meet the requirements of the AFMC Form 158 and are suitable for return shipment of serviceable assets. The contractor shall dispose of and replace unserviceable containers with new containers that meet requirements of the AFMC Form 158.

Contact the Product Support Manager and Administering Contracting Office (ACO) for disposition instructions and replacement when Long Life Reusable Container(s) are received in an unserviceable condition.

REPORTING DISCREPANCIES

The contractor shall use Web Supply Discrepancy Report (WebSDR) to submit/reply to a Supply Discrepancy Report (SDR), when improperly packed assets are received/shipped in other than the contracted packaging requirements (coded data or Special Packaging Instructions (SPI) drawings) from Department of Defense (DoD) components or contractor activities. Exceptions when no damage has occurred are as follows:

- Items packaged before the current SPIdate.

- An alternate pack may be used, provided the prime Air Force Sustainment Complex, Packaging Office at the Air Logistics Complex (ALC) approves and issues a waiver, deviation number, which would be annotated on the exterior container on the lower right hand portion of the identification side of thecontainer.

- In accordance with Technical Order (TO) 00-85B-3, How to Package Air Force Spares, the contractor may substitute the next larger size fast pack, of the type I or type II to ship transportation priorities 1 and 2 shipments as long as there is no decrease in any of the container dimensions without the requirement of a waiver deviation. DEMONSTRATED PRIOR TO ANY CONTRACT AWARD.

4120012543508AH is a Specification Control Drawing that covers the technical requirements for the design, fabrication, test and performance of the Environmental Control Unit - MMTHS. The Environmental Control Unit- MMTHS is a part of the Transport Erector Environmental Control System, which is a part of the Minuteman III weapon system. This part requires engineering source approval by the engineering source activity, OO-ALC 414 SCMS/GUEA, Hill AFB, UT in order to maintain the quality of the part and ensure personnel safety, mission accomplishment, and functional integrity.

1. Completion of the specified pre-contract award qualification requirement will assure the government that the offeror is capable of manufacturing the part in compliance with the applicable technical specification/data and within the schedule and economic constraints of our contract. There are significant technical and schedule risks that can only be minimized by a completion of the requirements prior to contract award.

SECTION C: QUALIFICATION REQUIREMENTS THAT MUST BE SATISFIED TO

BECOME A QUALIFIED SOURCE AND QUALIFICATION WAIVER

REQUIREMENTS.

Application: Minuteman III InterContinental Ballistic Missile (ICBM)

1. Prequalification Notice: The offeror shall notify Defense Supply Center Richmond (DSCR) of intent to qualify as a source for this item.

2. Manufacture, Test, and Inspection Capability: The offeror shall certify to the engineering source activity (414 SCMS/GUEA) the availability of the necessary test, inspection, diagnostic and manufacturing facilities, equipment and personnel or its provisions to obtain them. If the offeror proposes to obtain them, the offeror will open its related facilities, equipment, tooling procedures, hiring plans and facility and equipment orders to inspection and evaluation by the Government. The offeror must show the availability of facilities to manufacture to the levels required by standards outlined in the data package

3. Data Verification: The offeror shall verify possession of the most recent and complete data package by providing a list of all applicable drawings, specifications, and engineering change notices in his or her possession. The offeror may be required to furnish a copy of these documents.

4. Manufacture: At its own expense, the offeror shall manufacture the item and provide all quality, performance, and environmental evaluations to show compliance with the latest data package requirements. The offeror shall identify its material sources, provide its process control standards, and provide procedures for using any special test or inspection equipment, facilities, or software. The offeror shall prove to 414 SCMS/GUEA that all parts are available to the offeror for manufacturing the item prior to contract award.

5. Test and Evaluation or Verification: For prior approval and at its own expense, the offeror shall submit to 414 SCMS/GUEA a qualification plan/procedure explaining how it will verify compliance with all performance, environmental, mechanical, and quality assurance requirements identified by the latest data package. The Government retains the option to witness any documented testing. The offeror shall notify 414 SCMS/GUEA at least 30 days prior to any testing to be used as a basis for qualifying. After completing qualification testing, the offeror shall submit a test report to 414 SCMS/GUEA for acceptance.

6. Qualification Article: At the offeror's own expense, the offeror shall manufacture and provide a qualification article for evaluation by the Government. This article shall comply with the latest data package, will be subjected to form, fit, and function evaluation to demonstrate compatibility with the weapon system, and will be considered in evaluating the offeror's manufacturing capability. Once the qualification requirements are met, the offeror will be listed as an approved source for this item. Approval, however, does not guarantee subsequent contract award. Subsequent contract awards may require submission of a first article.

7. Estimated Costs: Estimated likely costs for government testing and evaluation, which will be incurred by the potential offeror to become qualified, are $20,000.00.

8. Time Completion Estimate: 414 SCMS/GUEA estimates that completion of this qualification effort should require 180 days. This is based on the timeline required by the various submittals and the time required to test the qualification article. This time estimate is not a deadline, but a notification to a potential offeror of the time that the Air Force thinks will be required to test/evaluate the qualification article. Depending on individual circumstances, the qualification process may require more or less time.

9. Time Limitation: An offeror may not be denied the opportunity to submit and have considered an offer for a contract if the offeror can demonstrate to the satisfaction of the contracting officer that the offeror (or its product) meets these standards for qualification or can meet them before the date specified for award of the contract. The award will not be delayed to provide the offeror with an opportunity to demonstrate its ability to meet the standards specified in this source qualification statement.

I 0. Evaluation of Proposals Not Previously Qualified: The offeror must be fully qualified in order to be eligible for a contract award. Therefore, the offeror must fulfill all of the requirements stated, in writing, in the source qualification statement before the date specified for award of the contract. The Government reserves the right to award a contract, at the time specified for award, to a qualified source even though other sources may not have finished qualifying.

11. Waiver: Any offeror who has had previous experience in the manufacture of this Environmental Control Unit, or other similar items, may apply, to the design control activity, for a waiver of all or part of the qualification requirements. The offeror must still comply with all of the qualification requirements, but may not have to prove that compliance to the extent described in the source qualification statement. A waiver may be granted only if the design control activity can establish, from previous experience and written submittals from the offeror, that the offeror is a qualified manufacturing source. Proof of qualification may be waived to the extent the submittals establish similarities between the items' designs; components, manufacturing/repair processes, and testing procedures, programs and equipment. If the offeror is awarded the contract, a post contract award article may be required.

12. Approval: Once the qualification requirements are met, the offeror will be listed as an approved source for this item. Approval, however, does not guarantee subsequent contract award.
