About this file

This is a Vendor's Statement form pursuant to Kentucky Revised Statute 45A.343, required by the City of Owensboro, Kentucky for contract compliance certification. The form applies to vendors seeking to contract with the City of Owensboro and serves as a declaration of the vendor's compliance history and future obligations regarding Kentucky state regulations. The statement requires vendors to certify that they have not been found in violation of specified Kentucky statutes within the five-year period preceding the submission and to acknowledge their obligation to maintain compliance throughout the contract term. The form does not specify particular products, services, project scopes, quantities, response dates, or contract terms, as it serves as a standard compliance document applicable to multiple procurement opportunities administered by the City of Owensboro.

The vendor statement imposes compliance requirements across seven Kentucky statutory chapters covering corporate taxes, sales and use taxes, income taxes, wage and hour laws, occupational safety, unemployment insurance, and workers' compensation. Vendors must acknowledge that failure to maintain compliance or failure to disclose prior violations constitutes grounds for contract cancellation and disqualification from City of Owensboro contracts for two years. The form requires completion with the vendor's company name, authorized officer signature, title, and date, serving as a certification mechanism rather than establishing pricing terms, set-asides, funding sources, or other contract-specific provisions. This document functions as a prerequisite compliance instrument that may be required in conjunction with other procurement documents but does not independently establish the terms of any specific contract opportunity.
