About this file
This is a Statement of Work (SOW) for an instrument placement agreement between the Navajo Area Indian Health Service and Alcor Scientific Inc. The contractor shall provide one ISED Elite ESR analyzer (product #112-00222) to the Chinle Comprehensive Health Care Facility under a cost-per-test model. The facility will purchase consumables, quality control materials, and testing supplies separately from the contractor on an as-needed basis, while the service agreement will be procured independently. The analyzer is currently installed and operational, and this represents an annual renewal request.
The period of performance runs from April 1, 2026, through March 31, 2027, with two optional 12-month renewal periods available. The contractor is responsible for shipping and delivering supplies as needed and responding to service requests according to established service agreement terms. Performance standards require proper invoicing for billing in the Integrated Personnel and Payroll System (IPP), with instrumentation replacement or repair handled by the manufacturer following their internal protocols. The Contracting Officer Representative is Colleen Ben, Laboratory Supervisor, at (928) 674-7101 or colleen.ben@ihs.gov. This is a sole-source award to the only organization uniquely qualified to provide the specialized ESR testing capability required to maintain continuity of clinical laboratory operations without interruption to patient care.
