SCOPE OF WORK
Chinle Service Unit Denture Program 2024
Provision of care given by Geriatric or Prosthetic Dentist Contractor
Description of the work to be performed: The Chinle Service Unit’s Dental Department needs a contractor, with the specified credentials, to provide dental services and prosthetic oral appliances to patients in the Chinle Service Unit.
Background: The Chinle Service unit’s dental clinics are not funded to take care of all the dental needs of the members of its community. This means that dental providers typically are involved only in the treatment of dental needs that fall in the Level I-III priorities for dental care (emergency, preventive, and restorative conditions). Periodically, funds may become available to provide for higher level needs (re-habilitation) such as dentures, but permanent dental staff cannot provide the care as they are busy meeting the more urgent dental needs of patients. Additionally, most general dentists are not proficient in manufacturing a good quality denture. The Indian Health Service does not typically staff dentists with the credentials or expertise to perform Removable Prosthodontics. Geriatric Dentists and Removeable Prosthodontists typically have more training and experience with dentures and provide a much higher rate of treatment success.
Unfortunately, the disease of caries still exists, and Native Americans have teeth removed more than other populations in our country. The loss of teeth is detrimental as it negatively impacts mastication, overall health, and psychosocial well-being. Acquiring a licensed contractor to come and provide full and partial dentures to those who have lost teeth can help improve the ability of patients to properly masticate thereby improving physical health and the mental well-being of patients.
Location of Work: Chinle Comprehensive Health Care Facility; Dental Clinic, Chinle Arizona, 86503
Period of Performance: March 01, 2024 to September 30, 2024.
Deliverable/Schedule: ARO 30 days
Performance Standards/Requirements: The contractor shall:
A. Provide a full range of removable dental prosthetic services for geriatric Diabetic or non-Diabetic patients seen in the Dental Clinic. Contractor will come to Chinle Hospital for 5 days with his laboratory technician to perform a denture exam, obtain impressions, jaw relations, and bite registrations on edentulous and partially edentulous patients for the fabrication of full dentures, partial dentures, and combinations thereof.
B. Will Make appropriate referrals and obtains appropriate consultations with the general dentist if needed for any prior/after treatment for extractions.
C. Materials/equipment will be provided by the contractor; and facilities and staff will be provided by the CCHCF Dental Program, except for the Contractor’s laboratory technician.
D. Contractor will then perform the necessary laboratory procedures in their facilities, and will return to the Dental Department in Chinle Hospital approximately one month later for three days to try in the prosthetic appliances to ascertain correct function, form and esthetics.
E. Contractor will then return to the Dental Department in Chinle Hospital for 2-3 days in approximately four to five weeks to insert the prosthetic appliances.
F. The final visit to Chinle Hospital by the Contractor will occur in approximately six weeks for approximately one day and a half to two days, to make any appropriate post-insertion adjustments.
G. Contractor and Dental Lab Assistant will interact in a professional manner with patients and co-workers
H. Services to be provided for approximately 161 patients.
Supervision: Functions under the general supervision of the service unit’s dental program director.
Special Requirements:
· Work must be accomplished during normally scheduled working hours, Typically 7:30 AM to 6:00 PM.
· Dentist must be a licensed prosthodontist or a Geriatric Dentist.
